A pilot study on the "ScoliBrace" in the treatment of adolescent idiopathic scoliosis.

Bracing for adolescents with idiopathic scoliosis (AIS) is a treatment option to prevent curve progression to surgical level. This study aimed to assess the efficacy of a 3D fully customized over corrective brace, "ScoliBrace," an orthosis treatment for AIS. This was a prospective pilot study of AIS female patients with inclusion criteria followed recommended Scoliosis Research Society (SRS) Guidelines. Cobb angles measured at: baseline (T0), 21months (T5-2), skeletal maturity (T6), 6months post-brace (T7), along with hours of brace wear using a thermal sensor and health-related quality of life (HRQoL) using the SRS-22r questionnaire. A total of 30 female AIS patients with mean age 11.85 ± 0.68years, predominantly Risser 0 (70%), and median Cobb angle 29° were recruited; 21 patients were included for the final analysis. Results showed significant difference in Cobb angle between T0 and T5-2 (median = 22.5° vs. 28.5°, p = 0.0082). 57.14% had reduction in Cobb angle by ≥ 5° at skeletal maturity. Cobb angle reduced 0.794° for each additional hour of dosage (p = 0.036, 95% CI = - 1.532°, - 0.056°). Although pain level was increased at T6 (4.37 ± 0.51vs.4.70 ± 0.41, p = 0.014), patients reported significantly greater satisfaction with management of their condition (3.90 ± 0.90vs.3.29 ± 0.88, p = 0.020). Results show similar findings to the BRAIST study, whereby curves remained under surgical threshold and showed improvement. More than half had curve reduction of ≥ 5° at skeletal maturity. Increased dose was also associated with improved outcomes. Using "ScoliBrace" as a non-surgical treatment, maintained curves below surgical threshold and showed curve reduction, improving patient satisfaction with management.

Outcome measures for young people with adolescent idiopathic scoliosis: A qualitative exploration of healthcare professionals' perceptions and practices.

Limited knowledge exists on current use of patient reported outcome measures (PROMs) and performance measures for adolescents with idiopathic scoliosis (AIS), as well as health care professionals' (HCPs) perceived barriers and facilitators towards their use. This study's objectives were: 1) to explore current practice of HCPs when assessing outcomes for AIS 2) to understand perceived barriers and facilitators of HCPs to use PROMs 3) to understand perceived barriers and facilitators of HCPs to use performance measures. A qualitative study recruited a purposive sample of HCPs from a tertiary hospital in the United Kingdom. Mean years of experience managing individuals with AIS was 11.8 years; and included surgeons, physiotherapists and nurses, educated at Bachelor, Masters and Doctoral level. Consent to participate and demographic information were collected in advance of the interviews. In-depth, virtual semi-structured interviews were informed by a topic guide based on current evidence. Interviews of approximately 45 minutes were audio and video recorded and transcribed verbatim alongside written field notes. Data were coded and analysed using inductive thematic analysis, involving researchers with topic and methodological expertise and input from a patient representative. Two themes emerged regarding current practice of using PROMs routine practice and personal evaluations. Four themes emerged as barriers to using PROMs for individuals with AIS: priority and support (e.g., HCPs focus on providing care), practical challenges (e.g., inadequate PROMs), patient-related challenges (e.g., patient preferences) and knowledge, education, and perceived value. Two themes emerged as facilitators: quality existing measure (e.g., sufficient psychometric properties), and priority and support (e.g., research department/culture). Themes for barriers to use performance measures were practicality (e.g., need physical space) and perceived value and knowledge (e.g., PROMs are more important), while the one theme for facilitators was practical consideration (e.g., acceptability). Although HCPs perceived the value of using outcome measures, current practice indicates limited use for individuals with AIS. The findings revealed different barriers and facilitators to implement PROMs in practice. Adopting performance measure are limited due to lack of knowledge and perceived value alongside the practicality, while considering practical factors can improve the use of these measures in practice.

Effect of orthopedic insoles on spinal deformity and walking in adolescents with idiopathic scoliosis summary.

To observe the effects of scoliosis-specific exercise therapy combined with braces and orthopedic insoles on improved spinal deformity and walking ability in adolescents with idiopathic scoliosis (AIS). From September 2019 to September 2020, 60 outpatient AIS patients were distributed into brace group (n = 30) at random and brace combined orthopedic insole group (n = 30). Both groups underwent brace dryness, and the observation group used scoliosis-specific exercise therapy combined with brace therapy, and on this basis, orthopedic insole intervention was added for 8 h per day for 2 months. At the same time, 20 adolescents of the same age with normal spinal development were recruited as a healthy group. GaitScan instruments were used to collect gait and plantar pressure measurements from study subjects. First, the gait and plantar pressure data of AIS patients and healthy groups were compared horizontally to ascertain the abnormal indicators, and then the spinal deformity and the above abnormal indicators were compared between the brace group and the brace combined orthopedic insole group. The plantar pressure center drift index (CPEI) in the AIS group was higher than that in the healthy group (F = 3.120, P < 0.05), and there were significant differences in the ratio of medial and lateral heel pressure (M/l) and total foot pressure (P < 0.05) between the AIS group and the healthy group, and no noticeable variations were found in the support phase period, walking speed, and proportion of each phase (P > 0.05). After treatment, the Cobb angle was significantly reduced in both the brace group and the brace combined with orthopedic insole group (P < 0.05), and there was no significant difference between the groups (P > 0.05). There were no significant changes in the pressure ratio of CPEI, M/l and bilateral full foot in the brace group (P > 0.05). The CPEI decreased in the brace combined with orthopedic insole group (P < 0.05), and the pressure ratio of M/l and bilateral full foot tended to 1 (P < 0.05), and was better than that in the brace group (P < 0.05). Patients with AIS may have local and worldwide asymmetric changes in plantar pressure distribution. The addition of orthopedic insoles has limited effect on improving scoliosis deformity in patients with AIS, but it can effectively improve the abnormal biomechanics of patients with AIS, so that the patient's force tends to be balanced.

How Schroth Therapists Vary the Implementation of Schroth Worldwide for Adolescents with Idiopathic Scoliosis: A Mixed Methods Study.

(1) Background: Schroth is a type of physiotherapeutic scoliosis specific exercise (PSSE) prescribed to adolescents with idiopathic scoliosis (AIS). Studies have investigated the effectiveness of Schroth but are yet to elucidate how Schroth is applied clinically and the factors that influence their prescription. (2) Methods: A mixed methods design was used comprising an anonymous survey and semi-structured interviews of Schroth therapists who treated AIS and who were publicly listed on the Barcelona Scoliosis Physical Therapy School or the International Schroth 3-dimensional Scoliosis Therapy School websites. The survey included 64 questions covering demographics, session and treatment characteristics, and whether therapists included other treatment modalities in their clinical practice. A convenience sample of survey participants were invited to participate in a semi-structured interview to further explore the factors that influenced their prescription of Schroth for AIS. Results from the survey were analyzed descriptively (n, %), whereas inductive thematic analysis was used for the interviews. (3) Results: of the 173 survey respondents (18% response rate), most were from Europe and North America (64.0%), female (78.6%), physiotherapists (96.0%), and worked in private settings (72.3%). Fifty-two per cent of participants used other types of PSSE as an adjunct to Schroth, the Scientific Exercise Approach to Scoliosis (SEAS) being the most frequently used (37.9%). Non-PSSE methods were used 'at some point' as an adjunct by 98.8% of participants, including massage and other soft tissue techniques (80.9%), Pilates (46.6%), and Yoga (31.5%). The Schroth techniques used by all survey respondents included breathing and pelvic corrections. Seven participants were interviewed, but data saturation was achieved after only four interviews. Thematic analysis revealed four, inter-related broad themes describing the factors that influenced Schroth prescription for AIS: (1) the adolescent as a whole, including physical, emotional and mental characteristics, and patient goals, (2) family, including parent relationship with the adolescent and the motivation of parents in regard to Schroth, (3) the systems within which the treatment was being offered, such as vicinity to the clinic and the presence of financial insurance support, and (4) therapist characteristics, such as their training and experience. (4) Conclusions: Schroth therapists worldwide use a variety of adjunctive methods to treat AIS. Therapists prescribing Schroth exercises to AIS consider the complex interplay of intra-, inter- and extra-personal factors in clinical practice. These considerations move beyond the three components of evidence-based practice of research, patient preferences, and clinical expertise, towards a systems-based reflection on exercise prescription.

Investigation of Static Balance Differences between Adolescents with Idiopathic Scoliosis and Healthy Age-matched Adolescents: A Cross-sectional Study

Background: The aim of this study was to compare postural control between Adolescents with Idiopathic Scoliosis (AIS) and healthy adolescents.&#x0D; Methods: In this cross-sectional study, 20 AIS adolescents with a right thoracic curvature between 10 and 45 degrees and 20 healthy adolescents were matched to compare postural control. Postural control assessments included Center of Pressure (COP) range and COP velocity in the Anteroposterior (AP) and Mediolateral (ML) directions, and the COP sway area in the standing position with opened eyes and closed eyes by the force plate.&#x0D; Results: This study showed that the postural control variables in AIS adolescents and healthy adolescents in terms of COP range, COP velocity, and COP sway area in standing positions with open and closed eyes were significantly different with those of AIS adolescents being worse than those of healthy adolescents (all p&lt;0.001). The interaction between Group and Condition (eyes open and eyes closed) was not significant for all variables (all p&gt;0.05).&#x0D; Conclusion: Differences in the postural control measures indicate postural instability in adolescents with AIS compared to healthy adolescents.

Predictors of Relevant Changes in Pain and Function for Adolescents with Idiopathic Scoliosis following Surgery.

Retrospective analysis of longitudinal data. To evaluate clinically relevant change in surgical outcomes for Adolescents with Idiopathic Scoliosis (AIS), comparing those who achieved smallest detectable change (SDC) in pain and function at 1-year post-surgery and those who did not, and to evaluate the influencing factors. The SDC is recommended to evaluate the surgical outcomes of AIS. However, little is known about the use of SDC in AIS and its influencing factors. This was a retrospective analysis of longitudinal data from patients who underwent surgical correction at a tertiary spinal centre from to 2009-2019. Surgical outcomes were assessed at short-term (6wk, 6mo) and long-term (1- and 2-years) post-surgery using the Scoliosis Research Society (SRS-22r). The difference between 'successful' (≥ SDC) and 'unsuccessful' (< SDC) groups was assessed using an independent t-test. Univariate and logistic regression analyses enabled the assessment of influencing factors. All SRS-22r domains decreased in the short term, except for self-image and satisfaction. In the long term, self-image increased by 1.21 and function increased by 0.2, and pain decreased by 0.1. In all SRS-22r domains 'successful' group had low pre-surgery scores and were statistically different to the 'unsuccessful group'. The difference remained statistically significant at 1-year for most SRS-22r domains. Being older and having low pre-surgery SRS-22r scores increased the chances of achieving SDC function at 1-year. Achieving SDC in the pain domain was significantly associated with age, sex, length of hospital stay, and pre-surgical scores. Notably, the self-image domain showed the largest change compared to other SRS-22r domains. A low preoperative score increases the likelihood of clinical benefit from surgery. These findings demonstrate the utility of SDC for assessing the benefits and factors that may underpin surgical benefit in AIS.

Predictors of Clinically Meaningful Results of Bracing in a Large Cohort of Adolescents with Idiopathic Scoliosis Reaching the End of Conservative Treatment

We need good outcome predictors to maximize the treatment efficiency of adolescents with idiopathic scoliosis (AIS). The in-brace correction has shown an important predictive effect on brace failure, while the influence of other variables is still debated. We aimed to identify new outcome predictors from a big prospective database of AIS. Design: Retrospective analysis of prospectively collected data. AIS between 21 and 45°, Risser 0-2, brace prescription during the observation, treatment conclusion. All of the participants followed a personalized conservative approach according to the SOSORT Guidelines. End of growth below 30°-40°-50°. The regression model included age, BMI, Cobb angle, ATR, TRACE score, real brace wear (RBW), and in-brace correction (IBC). A total of 1050 patients, 84% females, ages 12.1 ± 1.1, 28.2 ± 7.9° Cobb. IBC increased by 30%, 24%, and 23% the odds of ending treatment below 30°, 40°, and 50°, respectively. The OR did not change after the covariate adjustment. Cobb angle and ATR at the start also showed a predictive effect. The systematic evaluation of IBC in clinics is useful for individuating the patient response to brace treatment more accurately, even in relation to the Cobb angle and ATR degrees at the start. Further studies are needed to increase the knowledge on predictors of AIS treatment results.

Vitamin D and adolescent idiopathic scoliosis, should we stop the hype? A cross-sectional observational prospective study based on a geometric morphometrics approach

PurposeThere is strong evidence supporting the presence of fluctuating asymmetry (FA) in Adolescents with Idiopathic Scoliosis (AIS). Additionally, recent research investigating the relationship between vitamin D and AIS found a relation between them. We hypothesize a negative correlation between FA and vitamin D.MethodsWe performed a surface scan of the torso of 53 AIS patients, a blood test to measure vitamin D and the radiographic Cobb angle. A correlation analysis between vitamin D and FA was carried out to test our hypothesis, and a regression of vitamin D on 3D shape was performed to observe shape differences between the vitamin D deficiency and insufficiency groups.ResultsThere was no correlation between vitamin D and FA. We found a strong negative correlation between vitamin D and the Cobb angle only in the premenarche group (n = 7; r = − 0.92). Differences in shape were observed between the deficiency and insufficiency groups, and that differences were related to the width of the torso, but not the rotation or lateral flexion.ConclusionsOur results do not support the massive screening of vitamin D in AIS. Shape analysis revealed differences between the shape of the deficiency and insufficiency groups related to robustness. However, this finding had no relation with the scoliosis characteristics, it just reflected different body composition, and its importance should be explored in future.

Effect of Brace Treatment on Craniovertebral to Lumbopelvic Sagittal Parameters in Adolescents with Idiopathic Scoliosis: A Systematic Review.

The current study was carried out systematically by conducting a review of the literature. The purpose of this study was to conduct a systematic review of the literature to determine the effects of brace wearing on sagittal parameters in adolescents with idiopathic scoliosis (AIS). In this study, PubMed/MEDLINE (National Library of Medicine), Scopus, Ovid, CINAHL, PEDro, Google Scholar, and the Cochrane Library were accessed and searched using the patient, intervention, comparison, outcome, and study design model. We included studies that looked at the effects of brace treatment on sagittal spinopelvic parameters in AIS patients over the age of 18. The studies were chosen for their cross-sectional, retrospective, or prospective observational designs, and they were published in English. Review articles, case reports, case study designs, and conference abstracts were excluded from consideration. The methodological quality of the remaining articles was assessed using the Newcastle Ottawa Scale. A total of 12 studies were chosen, and 995 participants were evaluated, with 3 (25%) and 9 (75%) having high and moderate quality, respectively. The studies were classified based on the length of follow-up. Long-term, short-term, and immediate effects of brace wearing on sagittal spinopelvic parameters were reported in four, five, and three studies, respectively. The results of nine studies showed a significant decrease in Cobb angle after wearing the brace, which contradicted the findings of the other two. The cervical and sagittal pelvic parameters, thoracic kyphosis (TK), lumbar lordosis (LL), spinopelvic parameters, and sagittal balance were all evaluated in the intended studies, yielding varying results. According to the available literature, wearing a brace flattens the TK and LL. According to this systematic review, brace treatment may affect sagittal spinopelvic parameters in adolescents with AIS, particularly in TK and LL. The cervical and pelvic parameters yielded inconclusive results. This study backs up the idea that brace design and structure can influence sagittal parameter changes. The limitations of this study include different methods of parameter measurement, variations in the brace types and wear time, varying follow-up duration, and differences in participant characteristics.

ABSTRACT Objective: To verify the correlation between the thoracic and lumbar Cobb angle and the type of foot, and the parameters of plantar support during gait in adolescents with idiopathic scoliosis. Material and Methods: Sixty adolescents with idiopathic scoliosis (AIS) were divided into three groups: normal foot (n=20), cavus foot (n=20), and flat foot (n=20). The Cobb angles of thoracic kyphosis and lumbar lordosis were evaluated by radiographic examination. The plantar arch was recorded the podoscope and calculated by the ratio between the midfoot and the total foot area. The adolescents performed the march on a 20-meter track, with their feet resting on the pressure platform, totaling an average of 12 steps of the foot (right and left). The variables evaluated were: contact area, peak pressure, and maximum force on the four regions of the feet: hindfoot (medial and lateral), midfoot, and forefoot. Results: There was a positive correlation between the Cobb angle of lumbar lordosis and the arch plantar cavus (r=0.40; p=0.048) and flat (0.25; p=0.004), with no significant correlations for the Cobb angle thoracic (p&gt;0.005). The pressure peak strongly correlated with the cavus plantar arch (r=0.92, p=0.001) in the lateral hindfoot and forefoot region, while the flat foot with the midfoot region. Conclusion: The Cobb lumbar lordosis angle positively correlates with the plantar arch height and the plantar support pattern during gait in adolescents with idiopathic scoliosis. Virtual Scoliosis Surgery Using a 3D-Printed Model Based on Biplanar Radiographs.

The aim of this paper is to describe a protocol that simulates the spinal surgery undergone by adolescents with idiopathic scoliosis (AIS) by using a 3D-printed spine model. Patients with AIS underwent pre- and postoperative bi-planar low-dose X-rays from which a numerical 3D model of their spine was generated. The preoperative numerical spine model was subsequently 3D printed to virtually reproduce the spine surgery. Special consideration was given to the printing materials for the 3D-printed elements in order to reflect the radiopaque and mechanical properties of typical bones most accurately. Two patients with AIS were recruited and operated. During the virtual surgery, both pre- and postoperative images of the 3D-printed spine model were acquired. The proposed 3D-printing workflow used to create a realistic 3D-printed spine suitable for virtual surgery appears to be feasible and reliable. This method could be used for virtual-reality scoliosis surgery training incorporating 3D-printed models, and to test surgical instruments and implants.

A preliminary study in classification of the severity of spine deformation in adolescents with lumbar/thoracolumbar idiopathic scoliosis using machine learning algorithms based on lumbosacral joint efforts during gait

To assess the severity and progression of adolescents with idiopathic scoliosis (AIS), radiography with X-rays is usually used. The methods based on statistical observations have been developed from 3D reconstruction of the trunk or topography. Machine learning has shown great potential to classify the severity of scoliosis on imaging data, generally on X-ray measurements. It is also known that AIS leads to the development of gait disorder. To our knowledge, machine learning has never been tested on spine intervertebral efforts during gait as a radiation-free method to classify the severity of spinal deformity in AIS. Develop automated machine learning algorithms in lumbar/thoracolumbar scoliosis to classify the severity of spinal deformity of AIS based on the lumbosacral joint (L5-S1) efforts during gait. The lumbosacral joint efforts of 30 individuals with lumbar/thoracolumbar AIS were used as distinctive features fed to the machine learning algorithms. Several tests were run using various classification algorithms. The labeling consisted of three classes reflecting the severity of scoliosis i.e. mild, moderate and severe. The ensemble classifier algorithm including k-nearest neighbors, support vector machine, random forest and multilayer perceptron achieved the most promising results, with accuracy scores of 91.4%. This preliminary study shows lumbosacral joint efforts can be used to classify the severity of spinal deformity in lumbar/thoracolumbar AIS. This method showed the potential of being used as an assessment tool to follow-up the progression of AIS as a radiation-free method, alternative to radiography. Future studies should be performed to test the method on other categories of AIS.

The hybrid feature extraction method for classification of adolescence idiopathic scoliosis using Evolving Spiking Neural Network

Scoliosis is the most commonly occurring spinal condition in adolescents. Photogrammetry is an alternative to radiography for examining adolescents with idiopathic scoliosis (AIS) to measure the curvature of the human back and avoid radiography exposure. Currently, a manual approach is applied to detect the spine curve. Machine Learning (ML) models are introduced to overcome this issue and reduce human error. Thus, an appropriate Features Extraction (FE) method is crucial to producing a good ML classification model. Local Binary Pattern (LBP) is capable of providing the desired features from the surface of a human body. However, it produces a high number of parameters for each image, affecting the classification performance. Therefore, a new FE method has been proposed to reduced the number of parameters. A fusion of LBP and 1DCNN (1-Dimensional Convolutional Neural Network) FE methods that can reduce the number of parameters by 50% is proposed. These features are classified using Evolving Spiking Neural Network (LBP-1DCNN-ESNN) and achieve an accuracy of 90.00%. It leads to faster and more accurate results. ESNN can produce efficient and fast decisions through a one-pass learning process generated from the neuron repository. The proposed LBP-1DCNN-ESNN model benefits healthcare when quick solutions are demanded.

Impact of the Free-Pelvis Innovation in Very Rigid Braces for Adolescents with Idiopathic Scoliosis: Short-Term Results of a Matched Case-Control Study.

We introduced pelvis semi-rigid material (ethylene vinyl acetate) (Free-Pelvis) to improve the comfort and adaptability of very rigid braces (VRBs) for adolescents with idiopathic scoliosis (AIS), but this can also negatively impact the corrective forces on the trunk. Study Design: This was a matched retrospective cohort study. The inclusion criteria were AIS, age 10–16, VRB 23 h/day, X-rays available, primary curve 36°–65°, and angle of trunk rotation 7–23°. The cases were Sforzesco VRB with Free-Pelvis (FPB). The controls included classical Sforzesco VRB matched for Risser (range 0/4), menarche age (10/15), weight (33.5/83 kg), height (140/180 cm), BMI (13.5/29 kg/sqm), aesthetics (TRACE 4/12), plumbline distances (S1: −60/35; C7 + L3: −10/115 mm), and referred brace use (22/24 h/day). Statistics: predictors of the results have been tested with linear and logistic regression according to the outcome variable type. We performed logistic regression for improved vs. worsened. The explanatory variable was brace type. We included 777 VRB and 25 FPB, age 13 ± 1, 47° ± 8° Cobb, and 11% men. The few baseline statistical differences were not clinically relevant. We achieved in-brace corrections of 15.2° ± 7.7° and 17.4° ± 6.5° for VRB and FPB, respectively (p = 0.21); out-of-brace corrections at 5 ± 2 months were 7.8° ± 0.2° for VRB and 8.1° ± 1.3° for FPB (p = 0.83). The type of brace did not influence the Cobb angle at either time interval or affect the odds of improvement. Free-Pelvis innovation, introduced to improve comfort and adaptability, does not change the in-brace or short-term results of classical VRB and consequently can be safely applied.

Digitization of three-dimensional spine curvature profile in adolescent idiopathic scoliosis using anatomical palpation

ABSTRACT The need for spaced radiological follow-up of adolescents with idiopathic scoliosis (AIS) stimulates the development of radiation-free methodologies to document spinal curvature parameters. The aim of the study was to develop a methodology, referred to as A-Palp, using index finger pulp palpation to assess 3D spinal curvature in the standing position in patients and control group. To estimate repeatability for 2D/3D curvature angles, three manual palpations of spinal curvature were performed by one examiner using an index finger pulp calibrated in an optoelectronic system to acquire these curvature angles in the sagittal and frontal planes and in the 3D plane of maximum curvature. The intrarater repeatability was analysed by generalisability theory and the dependability index ϕ combined with a decision D-study to judge the number of desirable repetitions. The A-Palp/radiograph concurrent validity of 2D/3D apex angles was studied by linear regression and Bland–Altman plot. For the scoliosis angle in AIS patients, the ϕ index was excellent, above 0.9, with a standard error of measurement and minimal detectable change below 3° and 9°, respectively. For A-Palp/radiograph comparison, 95% of 2D and 3D measurement results were between 8° and 11° below and 12° and 5° above the radiographic apex angle, respectively, with a bias below 3°. The good repeatability and concurrent validity show that this innovative radiation-free 3D methodology seems suitable for the assessment of AIS patients when the same examiner repeats the measurement two or three times according to the D-study.

Predicting final results of brace treatment of adolescents with idiopathic scoliosis: first out-of-brace radiograph is better than in-brace radiograph-SOSORT 2020 award winner.

In-brace radiograph of adolescents with idiopathic scoliosis (AIS) has been shown to reflect brace efficacy and the possibility of achieving curve correction. Conversely, the first out-of-brace radiograph could demonstrate the patient's ability to maintain the correction. We aimed to determine which of the two radiographs is the best predictor of the Cobb angle at the end of treatment (final radiograph). Retrospective cohort study of a prospective dataset. The population was selected based on the following inclusion criteria: AIS, age 10-18years; Risser score 0-2; Cobb angle 25-40°; brace treatment; availability of all radiographs. Pearson correlations provide a first exploration of data. The univariate and multivariate logistic regression model tested the predictors. Finally ROC curve provided a check of model accuracy. A total of 131 patients were included (mean age 13.0 ± 1.3, Cobb angle 33.2 ± 5.5°; 78% females). At the end of treatment, 56% had stabilised, 9% had progressed, and 44% had improved. The difference between the in-brace and final radiographs was 8.0 ± 6.0°, while the difference between the first out-of-brace and final radiographs was 1.8 ± 5.2°. The best predictor of final outcome was the first out-of-brace radiograph (0.80), compared to in-brace (0.68) and baseline (0.59) radiographs. The best cut-offs to predict avoidance of progression were 30% and 10% of the correction rates for the in-brace and first out-of-brace radiographs, respectively. The first out-of-brace radiograph predicts end results better than the in-brace radiograph. It offers an excellent clinical reference for clinicians and patients. The first out-of-brace radiograph should be considered an essential element of future predictive models. Asymmetrical thoracic-lumbar coordination during trunk rotation between adolescents with and without thoracic idiopathic scoliosis.

Cross-sectional comparative study. To compare thoracic-lumbar kinematic changes and coordination based on coupling angles (CAs) in two different directions of trunk rotation between adolescents with idiopathic scoliosis (AIS) and control subjects. Altered three-dimensional (3D) deviations are often apparent in AIS groups during functional activities, such as gait. However, there is a lack of consistent evidence on coordinated motions during different directions of trunk rotation. This study included 14 AIS and 17 age-matched control subjects who were all right limb dominant. A motion capture system was utilized to analyze the spinal segment motions. The outcome measures included range of motion (ROM) at the first thoracic (T1), seventh thoracic (T7), and first lumbar (L1) spinous processes as well as the sacral tubercle (S1). The CAs compared in-phase (rotation from right to left) and anti-phase (rotation from left to right) trunk rotations. Although there was no significant association with the spinal segments in the control group, the Cobb angle demonstrated significant positive correlations with anti-phase at T7 and L1 as well as in-phase at L1. Regarding the CAs, the groups demonstrated a significant interaction with both phases (F = 4.7, p = 0.04). The AIS group demonstrated positive correlations with ROM during in-phase at L1 and anti-phase at T7 and L1. The coordination based on the CAs of the lumbar spine relative to the thoracic spine significantly decreased during left to right trunk rotation in the AIS group. These results indicated that the AIS group demonstrated directional dissociation toward the dominant side of lumbar rotation. III.

Intra- and inter-rater reliabilities and differences of kyphotic angle measurements on ultrasound images versus radiographs for children with adolescent idiopathic scoliosis: a preliminary study.

To develop a new method based on 3D ultrasound information to measure the kyphotic angle (KA) on ultrasound (US) images in adolescents with idiopathic scoliosis (AIS) and to evaluate the intra-rater and inter-rater reliabilities and accuracy of the US measurements. Twenty subjects with AIS (17F, 3M, aged 13.7 ± 2.2years old) were recruited. One 20 + years experienced rater (R3) measured the KA on radiographs twice using the Cobb method. Two raters (R1, R2), both have at least 1-year experience measured US images twice using the new spinous processes method. The intraclass correlation coefficients (ICC[2,1]) of the intra-rater and inter-rater reliabilities of US KA measurements were calculated. An equation based on US KA measurements to calculate the radiographic KA was generated. The intra-rater reliability ICC[2,1] (R3) of the X-ray measurement was 0.92 and US KA measurements for R1 and R2 were 0.94 and 0.95, respectively. The inter-rater reliability ICC[2,1] for R1 versus R2 were 0.85 and 0.86, respectively. The mean absolute differences (MAD) of US versus radiography measurements were 4.2 ± 3.0° (R1 vs R3) and 5.0 ± 4.1° (R2 vs R3), respectively. The radiographic equivalent KA = 0.82 × US KA - 5.6°. When using this equation, the overall MAD between US and radiographic KA was 2.9 ± 1.6°. The ultrasound spinous process method was reliable to measure the KA. Although there was a systematic bias on the US measurements, after the correction, the MAD of the US and radiographic KA was 2.9 ± 1.6°. Using US allows clinicians to monitor KA without exposing children to ionizing radiation.

The Italian Spine Youth Quality of Life questionnaire: reliability and validity of the Persian version in adolescents with spinal deformities

To assess the reliability and validity of the Persian version of the Italian Spine Youth Quality of Life (P-ISYQOL) questionnaire. Forward/backward translations and cultural adaption processes of the ISYQOL questionnaire into Persian were conducted by an expert committee. We recruited 178 adolescents with idiopathic scoliosis (AIS) and Scheuermann's kyphosis (SK) and 103 age-matched adolescents with no spinal deformity. Reliability was assessed by evaluating internal consistency and test-retest reliability using Cronbach's alpha and intraclass correlation coefficient (ICC). Convergent construct validity was assessed by measuring the association between the ISYQOL and revised 22-item Scoliosis Research Society (SRS-22r) questionnaires scores. Known-groups construct validity was assessed regarding curve magnitude, deformity, age, sex, and treatment type. Cronbach's alpha and ICC for P-ISYQOL total score was 0.8 and 0.9, respectively. An acceptable association was observed between P-ISYQOL and SRS-22r total scores (r = - 0.5, p < 0.01). The P-ISYQOL could discriminate between adolescents with spinal deformities and adolescents with no spinal deformity as well as those with different types of deformity and treatment (p < 0.01). The P-ISYQOL is a reliable and valid survey to assess the outcomes of adolescents with spinal deformities. Level I-diagnostic studies.

Coronal deformity angular ratio as a predictive factor for in-brace curve correction and long-term outcome of brace treatment in adolescents with idiopathic scoliosis.

To investigate the relationship between coronal deformity angular ratio (C-DAR) and in-brace correction (IBC) and their role in predicting the long-term bracing outcome in adolescents with idiopathic scoliosis (AIS). In this retrospective multicenter study, the patient's sex, age, primary curve Cobb angle (at initiation of brace treatment, best in-brace, before spinal fusion, and final follow-up), curve pattern, duration of brace treatment, brace type, and C-DAR at initiation of bracing were recorded. The C-DAR values were classified as < 5, 5 ≤ to ≤ 6, and > 6. The IBC values were classified as ≥ 50%, 40% ≤ to ≤ 49%, and < 40%. We classified the patients into two groups of success and failure according to the Cobb angle at the final follow-up. A total of 164 patients (25 boys and 119 girls) were included. Bracing was successful in 60.4% of them. There was a significant association between C-DAR and bracing outcome (p < 0.0001). 63.9% of the patients with C-DAR < 5 had an IBC ≥ 50%. However, when C-DAR was 5 ≤ to ≤ 6 and > 6, 29.2% and 16.9% of the patients had an IBC of ≥ 50%, respectively. For patients with IBC ≥ 50%, the success rate of bracing was 89.2%. Results of logistic regression analysis revealed that the strongest predictor for brace treatment outcome was the C-DAR, with an odds ratio of 2.11. C-DAR may be used as a predictive factor for the long-term outcome of brace treatment in AIS. IV.
