Providing adolescent-friendly sexually transmitted infection screening and treatment services.

There are high rates of sexually transmitted infections (STIs) worldwide. Adolescents and young adults (AYA) ages 15-24 years remain one of the populations that is most vulnerable to STIs. The goal of this review is to summarize recent international updates in adolescent STI screening and treatment. Normalizing sexual history taking and STI testing, and advocating for adolescents to receive comprehensive sexuality education improves stigma surrounding sexual health. The global rise in syphilis is pervasive and includes high rates of infection among AYA and women of reproductive age - universal screening may be indicated depending on local epidemiology. Gonococcal antimicrobial resistance remains a significant public health concern worldwide, thus judicious use of antimicrobials and reporting cases of resistance is crucial. Sexual health services are increasingly using virtual platforms, which may be an effective strategy for STI testing and treatment among AYA. Specific areas of focus to address the STI epidemic among AYA include reducing stigma surrounding sexual health, screening, and treatment of STIs, especially with the global rise in syphilis and high rates of gonorrhea resistance, in addition to increased use of telehealth services as effective education and intervention strategies.

Sexual health in adolescents with inflammatory bowel disease - The paediatric gastroenterologists' point of view.

Although sexual health (SH) impairment and sexually transmitted infections (STI) are occasionally encountered in patients with inflammatory bowel disease (IBD), paediatric gastroenterologists (PedGI) do not often discuss these issues. Literature about SH in the paediatric IBD population is limited. We aimed to assess PedGI knowledge and common practice related to sexual advice and STI workups in patients with IBD. A questionnaire comprising 25 questions addressing sexual activity in youth, SH, recommendations, and workup for STI in adolescents with IBD was sent to all registered PedGI in Israel. Fifty-two physicians completed the questionnaire (27 males,52%). Only 50% correctly predicted the mean age that Israeli youth start practicing sex. Seventy-five per cent responded that providers should discuss sexual activity with their patients, but only 19% do so, most often in response to a patient's query. Ninety six percent answered that they do not have enough knowledge about SH in IBD. Finally, only 2% obtain rectal swabs for STI in patients with refractory proctitis. Sexual issues and recommendations are not routinely discussed by the majority of PedGI in paediatric IBD clinics. Providers should obtain more knowledge in the field and initiate discussion of these issues with adolescent patients with IBD.

Leveraging Artificial Intelligence to Address Adolescent Sexually Transmitted Infections: A Systematic Review

Background: The integration of Artificial Intelligence (AI) into daily life provides a unique opportunity to address significant health concerns. In particular, the tech-savvy adolescent population could benefit from AI-enhanced access to reproductive health services, especially for the prevention, screening, and treatment of Sexually Transmitted Infections (STIs). Objective: This research aims to evaluate the impact of AI technology on improving adolescents' access to reproductive health services related to STIs. The study involves a systematic review of literature published from 2020 to 2024 across various databases. Methods: A systematic review methodology was employed, utilizing databases such as Google Scholar, PubMed, Semantic Scholar, Science Direct, and IEEE-XPLORE. Keywords used in the search included "artificial intelligence," "adolescents OR teenagers," and "sexually transmitted infections OR sexually transmitted diseases." Results: The review identifies AI as a pivotal tool in sexual education, particularly through the use of interactive and engaging chatbots. AI facilitates innovative educational interventions, allowing vulnerable and marginalized groups, including adolescents, to discuss and learn about sensitive topics like STIs. Conclusion: The study highlights the significant potential of AI in improving sexual health education for adolescents. The limited availability of research in this area underscores the importance of this study in advancing knowledge and addressing gaps in the application of AI for adolescent STI prevention and treatment.

Prevalence of sexually transmissible infections in adolescents treated in a family planning outpatient clinic for adolescents in the western Amazon.

Sexually transmitted infections (STIs) are among the most common public health problems worldwide, especially among adolescents and young adults, who account for almost 50% of all STI patients. Studies on the subject in the western Amazon are limited. This study aimed to evaluate the prevalence of STIs (chlamydia, gonorrhea, trichomoniasis, herpes simplex virus, syphilis, human immunodeficiency virus [HIV], hepatitis B, and hepatitis C) in adolescents treated at a family planning outpatient clinic in the western Amazon: Porto Velho, Rondônia, Brazil. A total of 196 adolescents were enrolled. During the gynecological examination, endocervical samples were collected to test for four STIs (chlamydia, gonorrhea, trichomoniasis, and herpes simplex virus), and blood samples were collected for the detection of HIV, syphilis, and hepatitis B and C. The mean age was 17.3 ± 1.5 years, the age at sexarche was 14.4 ± 1.6 years, and 54.6% of participants had their first sexual intercourse at 14 years or younger. Only 1.0% of the adolescents used condoms in all sexual relations, and 19.9% had casual partner(s) in the last year. In the evaluation of prevalence, we found that 32% of the adolescents had at least one STI, with the most prevalent being chlamydia (23%), followed by trichomoniasis (5.6%), herpes simplex (4.6%), and gonorrhea (3.1%). No positive cases of hepatitis B, hepatitis C, or HIV were detected, but 1% of the adolescents tested positive for syphilis. These indicators will support more effective health care strategies aimed at improving the quality of life of populations in this region of the western Amazon. In conclusion, our findings demonstrated high rates of STIs in the studied patients, reinforcing the need to expand epidemiological studies to implement more appropriate public policies and intervention strategies to prevent STIs in adolescents and other vulnerable populations in the western Amazon.

HUBUNGAN TINGKAT PENGETAHUAN DAN SIKAP DENGAN PERILAKU TERHADAP INFEKSI MENULAR SEKSUAL PADA REMAJA DI COMORO DILI TIMOR-LESTE

Sexually Transmitted Infections (STIs) are global public health problems. Adolescents are a vulnerable age group of STIs. This study aims to determine the relationship between knowledge and attitudes and behaviors towards STIs in adolescents in Comoro, Dili, Timor-Leste. This study was descriptive with a cross-sectional approach conducted in Canossa highschool and Externato de São Jose highschool from April-June 2022. The independent variables were knowledge and attitude, the dependent variable was adolescent behavior. The sample of this study was adolescents in 15-18 years old in Canossa highschool and Externato de São Jose and willing to become the respondent, 85 adolescents were selected as respondent. The results showed that 71.8% of respondents have good knowledge, 67.1% of respondents have good attitudes and 75.3% of respondents have good behavior in relation to STIs. The results of the analysis of the relationship between knowledge and attitudes to risk behavior for STIs 83.6% of respondents were categorized as good knowledge and have good behavior in relation to STIs and 84.2% of respondents were having good attitudes and have good behavior in relation to STIs. The results of the chi-square statistical test on knowledge and attitude variables obtained a p-value of 0.01, therefore, there is a significant relationship between the knowledge and attitudes of respondents regarding risk behavior for STIs. Comprehensive education regarding sexual health is needed for adolescents in Comoro Dili Timor-Leste.

Progress and challenges of HIV and other STIs in adolescents and young adults

In 2020, more than 50% of incident sexually transmitted infections (STIs),1 and approximately 27% of incident HIV cases, were estimated to occur among adolescents and young adults.2 The syndemics of HIV and other STIs interact to undermine health, especially in adolescent and young adult populations. Risk factors among adolescents and young adults include both behavioural and identity factors (early initiation of sex, injection drug use, injection drug use by sexual partners, sex with multiple partners, being a man who has sex with men, or being transgender) and social determinants (poverty and inequality, reduced access to education, stigma, ignorance, sex work, and political opposition to effective HIV prevention programmes such as needle exchange and comprehensive sexuality education).

Comparison of the Clinical Profile of Prepubertal versus Pubertal Female Child Sexual Abuse in a Tertiary Hospital

Objective. This study aimed to compare the clinical profile of prepubertal and pubertal female child sexual abuse.
 Methods. A cross-sectional analytical study involving 43 sexually abused children was done. Each patient was categorized as prepubertal or pubertal based on the Tanner stage. Medical history was gathered using a structured interview and physical examination done with proper documentation after obtaining consent. Blind review by child protection specialists was done on genital images. Descriptive statistics were utilized for all variables.
 Results. This study showed 11.97% and 22.22% prevalence for prepubertal and pubertal child sexual abuse, respectively. Most prepubertal children disclosed digital vaginal penetration by the father and non-relative household members, while most pubertal children reported penile-vaginal penetration by the boyfriend. Fondling was common to both groups. The majority were repeated abuse and usually happened at the perpetrator’s house. Behavioral changes and genital symptoms were common in prepubertal children. Findings of hymenal trauma were found in 25% of prepubertal girls and half of the pubertal adolescents. The interobserver variability of these hymenal findings was fair. The prevalence of sexually transmitted infections (STIs) was low, none for the prepubertal children, and only 6% of the adolescent had STI, which was Hepatitis B.
 Conclusion. The clinical profile of sexually abused prepubertal children differed from that of pubertal adolescents. There was a higher incidence of hymenal trauma and STIs in adolescents.

Sexually transmitted infections (STIs) are a public health problem that have been progressively increasing in recent decades. They are especially worrying in adolescents, given their vulnerability. In this protocol, we will discuss the diagnostic and treatment protocol for the most frequent STIs in adolescents: gonorrhea, chlamydia, syphilis, herpes, and human papillomavirus (HPV) lesions.

Acceptability and Feasibility of Peer-to-Peer Text Messaging Among Adolescents to Increase Clinic Visits and Sexually Transmitted Infection Testing: Interrupted Times-Series Analysis

BackgroundAdolescents are disproportionately affected by sexually transmitted infections (STIs), including HIV. Many youths with asymptomatic STI or related symptoms do not seek treatment and may not be screened if accessing the health care system for other reasons.ObjectiveWe examined intervention completion and changes in the number of new patients, the number of STI or HIV tests, and the sexual risk profile of patients over time to determine the feasibility and acceptability of a peer-driven text messaging strategy to connect youth to STI and HIV services.MethodsThe intervention enlisted consecutive patients at an adolescent medicine clinic to send a text message to 5 peers they believed were sexually active and lived in the clinic’s service area. The intervention was evaluated using an interrupted time-series design in which baseline clinic service levels were documented during a 35-week lead-in period, followed by a 20-week intervention implementation period, and a 16-week period of continued clinic observation. Clinic and patient data were obtained through chart abstraction from intake forms that occurred during the entire study period. Analyses conducted in 2015 used a generalized linear mixed model.ResultsOf the 153 patients approached to participate, 100 agreed to send SMS text messages. Most (n=55, 55%) reported no concerns with sending the text message. No adverse events or negative outcomes were reported. Adolescent STI testing, positive test results, and reported risk behavior increased post intervention, although this was not statistically significant, likely because of the small sample size.ConclusionsGiven low youth uptake of health care services, and STI/HIV screening, in particular, new strategies are needed to address access barriers. Common approaches for reaching youth are resource-intensive and often miss those not connected to school or community programs. The peer-based text messaging strategy showed promise for both increasing the number of youths accessing health services and finding youths engaging in sexual risk behaviors and most in need of sexual health screening and services.

Comprehensive Sexuality Education to Reduce Pregnancy and STIs in Adolescents in the United States: A Systematic Review and Meta-Analysis

The United States has a higher rate of teen pregnancy than any other developed country with 30% of American girls becoming pregnant before the age of 20. Laws regarding the inclusion and content of sexuality education vary across the country, which are associated with differences in pregnancy and sexually transmitted infection (STI) rates between states. This systematic review aims to determine whether comprehensive sexuality education (CSE) is more effective than abstinence-only or no sexuality education at reducing teenage pregnancy. Secondary objectives include analyzing the effect of CSE on STI incidence, sexual activity, safe-sex behaviors, and social discomfort. We searched multiple databases for studies published from 1990 to 2021. Twenty-nine studies met our inclusion criteria. Seven included pregnancy as an outcome, with three fitting our meta-analysis criteria. There was a decrease in pregnancy rates for participants in the CSE intervention compared to the control (n = 3, risk ratio = 0.89, 95%CI 0.79–1.00, I2 = 0%). We found that CSE is likely to reduce pregnancy rates; however, there was limited data available on this outcome. CSE increased safe-sex behaviors but did not have a notable impact on sexual activity or STIs. This review can serve as evidence for the implementation of CSE in the US.

ANALISIS PERILAKU TERHADAP SIKAP PENCEGAHAN INFEKSI MENULAR SEKSUAL PADA REMAJA DI DESA NEGERI BARU KETAPANG

Adolescents are the age group with the highest number of STIs compared to other age groups. The high number of people with STIs in adolescents and the younger generation really needs our attention because of the dangers and broad impacts. The highest number of STIs cases among adolescents in June 2019 was 29 cases of HIV-AIDS. The purpose of the study was to determine the effect of behavior on attitudes regarding the prevention of sexually transmitted infections (STIs) in adolescents.This research study is descriptive with a questionnaire interview survey of 25 respondents with simple regression analysis. Results: the behavioral variable has the lowest score of 9 and the highest score of 15. With an average value of the behavioral variable of 13.08 with a standard deviation of 1.352 and the value of the STIs prevention attitude variable in adolescents is 46.16 with a standard deviation of 3.923. And STIs prevention attitude variable in adolescents has the lowest score of 37 and the highest score of 54. The average value of the STIs prevention attitude variable in adolescents is 46.16 with a standard deviation of 3.923. Based on the results of the t test, the t-count value is 2.181 with a sig value of 0.040. In this study, the results of the t test are significant. Conclusion: there is an influence of behavior on adolescent attitudes in preventing STIs. Suggestions increase knowledge about STIs in all adolescents.

Predicting the impact of sexual behavior change on adolescent STI in the US and New York State: a case study of the teen-SPARC tool

PurposeAdolescents aged 13–18 years bear a large burden of sexually transmitted infections (STIs) and changing adolescent sexual risk behavior is a key component of reducing this burden. We demonstrate a novel publicly available modeling tool (teen-SPARC) to help state and local health departments predict the impact of behavioral change on gonorrhea, chlamydia, and HIV burden among adolescents. MethodsTeen-SPARC is built in Excel for familiarity and ease and parameterized using data from CDC's Youth Risk Behavior Surveillance System. We present teen-SPARC's methods, including derivation of national parameters and instructions to obtain local parameters. We model multiple scenarios of increasing condom use and estimate the impact on gonorrhea, chlamydia, and HIV incidence, comparing national and New York State (NYS) results. ResultsA 1% annual increase in condom use (consistent with Healthy People 2020 goals) could prevent nearly 10,000 cases of STIs nationwide. Increases in condom use of 17.1%, 2.2%, and 25.5% in NYS would be necessary to avert 1000 cases of gonorrhea, 1000 cases of chlamydia, and 10 cases of HIV infection, respectively. Additional results disaggregate outcomes by age, sex, partner sex, jurisdiction, and pathogen. ConclusionTeen-SPARC may be able to assist health departments aiming to tailor behavioral interventions for STI prevention among adolescents.

Office-Based Screening for Sexually Transmitted Infections in Adolescents.

Almost 1 in 4 adolescents have a sexually transmitted infection (STI). These infections are preventable through safe sexual practices and routine screening. Pediatricians are the first line of clinical care for adolescents and are well positioned to offer sexual and reproductive health care counseling and services to their patients; yet, there is a paucity of sexual health screening provided at routine health supervision visits. This article addresses the epidemiology of STIs in adolescents, reviews the evidence of current clinical practice, presents recommended STI screening from government and medical agencies, and offers strategies to address barriers to providing care for adolescents and for sexual health screening in primary care.

Examining HIV and STIs Related Knowledge Among Male Adolescents in Saudi Arabia

Background: Sexually Transmitted Infection (STI) is a major global cause of acute illness, long-term disability and death, with serious medical and psychological consequences. Knowledge and awareness about the transmission of sexual disease play an important role in the prevention of the disease. Very little is known about HIV and STI in adolescents and associated factors in Saudi Arabia. Objective: The aim of this study was to examine the association between self-rated STI knowledge and HIV and STI knowledge test scores, as well as to investigate the association between adolescents’ HIV and STI knowledge and their demographic background and sexually related behaviors. Methods: A multi-stage cross-sectional study was conducted using a self-administered questionnaire, recruiting 453 male adolescents (15 and 20 years) from private and public male high schools in Riyadh, the capital city of Saudi Arabia. Results: More than half (53.4%) of the adolescents reported their STI knowledge as excellent or good (42%). However, based on the total score scale of HIV and STI knowledge, 64.3% had a poor level of knowledge. The sexual information sources commonly cited were friends (70%) and internet (40.0%); and the least cited sources were fathers (16%) and mothers (15%). The logistic regression analysis revealed that adolescents’ father and mother education (>12 years) were positively associated with adolescent’s HIV and STI knowledge level (OR= 1.6, 95% CI: 1.1-2.4, p= 0.040) and (OR= 1.8, 95% CI: 1.2-2.8, p= 0.010). No significant association was found between adolescents’ HIV and STI knowledge and their sexual behaviors and attitudes. Conclusion: Findings suggest that a high proportion of male adolescents have poor HIV and STI related knowledge. Poor knowledge and reliance on potentially unreliable informational sources coupled with an over-estimate perception of STI expertise suggests that improvements in STI and HIV education should be addressed. Additional research is needed to help identify factors (beyond knowledge) associated with sexual behavior and attitudes that may increase the risk for STIs and HIV among adolescents.

A review of current guidelines and research on the management of sexually transmitted infections in adolescents and young adults.

Adolescents and young adults are at high risk for sexually transmitted infections (STIs). Providers have the responsibility to accurately manage these infections to prevent medical complications and the spread of STIs. This article will review the epidemiology, screening recommendations, diagnostic tests, and treatment guidelines for STIs most commonly encountered in this population: Chlamydia trachomatis, Neisseria gonorrhoeae, Herpes simplex virus, and Trichomonas vaginalis, as well as the sexually associated infection bacterial vaginosis. This review will discuss ongoing research that explores ways to improve the management of STIs in adolescents and young adults.

Sexually Transmitted Infection Testing in Adolescents: Current Practices in the Hospital Setting

PurposeAdolescents are disproportionately affected by sexually transmitted infections (STIs), and evidence supports expanding sexual health services to the hospital setting. Because STI testing practices in the hospital setting are poorly understood, we sought to describe current STI testing practices among adolescents seen in children's hospitals. MethodsWe performed a retrospective analysis of adolescents (14–18 years old) from 45 children's hospitals in 2015–2016, excluding visits with a billing code for sexual abuse/assault. We calculated rates of STI testing and investigated differences in STI testing by patient and hospital characteristics using generalized linear mixed modeling. ResultsOf the 541,714 adolescent encounters, 59,158 (10.9%) underwent STI testing. After adjusting for demographic characteristics, those with an STI test were more likely to be female (adjusted odds ratio [aOR] 1.61; 95% confidence interval [CI] 1.58–1.64), of non-Hispanic black race/ethnicity (aOR 1.20; 95% CI 1.17–1.23), or from the lowest median household income quartile (aOR 1.21; 95% CI 1.17–1.24). There was substantial inter-hospital variation in adjusted rates of STI testing (3%–24%), but strong correlation was observed between STI testing rates in the ED and inpatient settings within individual hospitals (adjusted R2 .99). ConclusionsOnly one in ten adolescents seen in children's hospitals underwent STI testing with wide variation in testing patterns across hospitals. There are critical opportunities to increase adolescent STI testing in this setting. Our findings highlight potential disparities in STI testing rates and patterns that warrant further exploration from the patient, provider, and health system perspective.

The growing epidemic of sexually transmitted infections in adolescents: a neglected population.

Sexually transmitted infection (STI) incidence is on the rise in the United States. The increase is especially pronounced in adolescents (15-24 years of age). Despite making up only a quarter of the population, adolescents account for approximately half of new STIs in the United States every year. This review summarizes recent developments in the field of STIs, excluding HIV, in adolescents. In this review, we examine the epidemiology, screening, management, and prevention of STIs in adolescents. STI rates in adolescents have been rising since 2014, with young women and MSM at particularly high risk. Barriers to STI screening for adolescents include confidentiality concerns and lack of access to health services. Prevention through STI vaccines represents a promising way to combat the epidemic. STIs are a growing concern for adolescents. Routine screening and management are of critical importance. Furthermore, prevention efforts such as human papillomavirus vaccination should be prioritized. Much of the current literature on STIs does not address the unique nature of STIs in adolescents, and additional research into effective prevention and treatment strategies of STIs in adolescents is urgently needed.

STI Knowledge in Berlin Adolescents.

Sexually transmitted infections (STIs) pose a significant threat to individual and public health. They disproportionately affect adolescents and young adults. In a cross-sectional study, we assessed self-rated and factual STI knowledge in a sample of 9th graders in 13 secondary schools in Berlin, Germany. Differences by age, gender, migrant background, and school type were quantified using bivariate and multivariable analyses. A total of 1177 students in 61 classes participated. The mean age was 14.6 (SD = 0.7), 47.5% were female, and 52.9% had at least one immigrant parent. Knowledge of human immunodeficiency virus (HIV) was widespread, but other STIs were less known. For example, 46.2% had never heard of chlamydia, 10.8% knew of the HPV vaccination, and only 2.2% were aware that no cure exists for HPV infection. While boys were more likely to describe their knowledge as good, there was no general gender superiority in factual knowledge. Children of immigrants and students in the least academic schools had lower knowledge overall. Our results show that despite their particular risk to contract an STI, adolescents suffer from suboptimal levels of knowledge on STIs beyond HIV. Urgent efforts needed to improve adolescent STI knowledge in order to improve the uptake of primary and secondary prevention.

A Computerized Sexual Health Survey Improves Testing for Sexually Transmitted Infection in a Pediatric Emergency Department

To assess whether clinical decision support, using computerized sexually transmitted infection (STI) risk assessments, results in increased STI testing of adolescents at high risk for STI. In a 2-arm, randomized, controlled trial conducted at a single, urban, pediatric emergency department, adolescents completed a computerized sexual health survey. For patients assigned to the intervention arm, attending physicians received decision support to guide STI testing based on the sexual health survey-derived STI risk; in the usual care arm, decision support was not provided. We compared STI testing rates between the intervention and usual care groups, adjusting for potential confounding using multivariable logistic regression. Of the 728 enrolled patients, 635 (87.2%) had evaluable data (323 intervention arm; 312 usual care arm). STI testing frequency was higher in the intervention group compared with the usual care group (52.3% vs 42%; aOR 2 [95% CI 1.1, 3.8]). This effect was even more pronounced among the patients who presented asymptomatic for STI (28.6 vs 8.2%; aOR 4.7 [95% CI 1.4-15.5]). Providing sexual health survey-derived decision support to emergency department clinicians led to increased testing rates for STI in adolescents at high risk for infection, particularly in those presenting asymptomatic for infection. Studies to understand potential barriers to decision support adherence should be undertaken to inform larger, multicenter studies that could determine the generalizability of these findings and whether this process leads to increased STI detection. A New Strategy for Trichomonas Testing Female Adolescents in the Emergency Department

Study ObjectiveSensitive trichomonas diagnostic testing has become available, including nucleic acid amplification tests (NAATs) and a rapid antigen test. The study purpose was to determine if adding sensitive trichomonas testing to routine female sexually transmitted infection (STI) evaluations would increase trichomonas identification and treatment. DesignTwo study time periods. Study time 1 (T1) was used for a retrospective review. Study time 2 (T2) was used for a prospective study. SettingEmergency Department. ParticipantsSymptomatic female patients aged 13-20 years (N = 447). InterventionsImplementation of routing trichomonas testing in the Emergency Department during T2. Main Outcome MeasuresTrichomonas diagnosis and treatment rates were compared during T1 and T2. ResultsDuring T1 31 of 234 of eligible patients (13%) were trichomonas-tested. Laboratory-confirmed trichomonas was identified in 3 of 234 (1.3%). During T2, 212 of 213 of eligible patients (99.5%) were trichomonas-tested; 39 of 212 tested trichomonas-positive (18.4%); 29 of 212 tested rapid trichomonas antigen test-positive (13.6%; P < .001), and 33 of 188 tested trichomonas NAAT-positive (15.5%; P < .001). Trichomonas treatment was given to 3 of 3 laboratory-confirmed trichomonas cases during T1 (100%) compared with 37 of 39 during T2 (95%; P = .688). During T1, 14 of 17 women who received trichomonas treatment (82.4%) did not have a laboratory-confirmed trichomonas diagnosis and during T2 13 of 52 women without a laboratory-confirmed trichomonas diagnosis (25%) were treated for trichomonas (P < .001). Rapid trichomonas antigen tests and trichomonas NAATs were concordant in 178 of 188 patients (94.6%). ConclusionIncorporating trichomonas rapid antigen tests and NAATs into routine female adolescent STI testing significantly increased the number of laboratory-confirmed adolescent trichomonas diagnosis and treatment and are useful Emergency Department STI screening tools.
