Comparison of three artificial intelligence algorithms for automatic cobb angle measurement using teaching data specific to three disease groups

Spinal deformities, including adolescent idiopathic scoliosis (AIS) and adult spinal deformity (ASD), affect many patients. The measurement of the Cobb angle on coronal radiographs is essential for their diagnosis and treatment planning. To enhance the precision of Cobb angle measurements for both AIS and ASD, we developed three distinct artificial intelligence (AI) algorithms: AIS/ASD-trained AI (trained with both AIS and ASD cases); AIS-trained AI (trained solely on AIS cases); ASD-trained AI (trained solely on ASD cases). We used 1612 whole-spine radiographs, including 1029 AIS and 583 ASD cases with variable postures, as teaching data. We measured the major and two minor curves. To assess the accuracy, we used 285 radiographs (159 AIS and 126 ASD) as a test set and calculated the mean absolute error (MAE) and intraclass correlation coefficient (ICC) between each AI algorithm and the average of manual measurements by four spine experts. The AIS/ASD-trained AI showed the highest accuracy among the three AI algorithms. This result suggested that learning across multiple diseases rather than disease-specific training may be an efficient AI learning method. The presented AI algorithm has the potential to reduce errors in Cobb angle measurements and improve the quality of clinical practice.

Comparison Between Resident and Attending Surgeons as Assistants on Adolescent Idiopathic Scoliosis Surgery: No Differences in Outcomes, Complications Rate, or Pedicle Screw Placement Accuracy.

Retrospective cohort study. The aim of the present study was to determine if the level of training of the first assistant (resident or attending surgeon) has an influence on the radiographic outcome of AIS surgery and on the accuracy rate of the pedicle screws placement. Adolescent idiopathic scoliosis (AIS) surgery is a challenging procedure that requires a dedicated team of skilled professionals. Therefore, understanding the learning curve is of outstanding importance to guarantee the best outcomes and the highest safety to the patients. A retrospective analysis of patients who underwent surgery for AIS with a minimum follow-up of 2 years was conducted. All patients were operated by an experienced spine surgeon, assisted by and attending surgeon (group A) or a senior resident (group B). Radiographic outcomes were assessed. Through postoperative CT scan, accuracy of pedicle screw placement was measured (using Gertzbein-Robbins classification). Groups were then statistically compared. A total of 120 patients were included (mean age 15.3±3.39y, major curve Cobb 60.7±11.9 degrees). No difference was found between groups in terms of preoperative characteristics and postoperative radiographic (correction rate, thoracic kyphosis, screw density) outcomes or complications. Operative time and estimated blood loss were significantly higher in Group B (P=0.045 and P=0.024, respectively). Of the 2746 pedicle screws inserted (1319 group A and 1427 group B), 2452 had a perfect intrapedicular trajectory (absolute accuracy of 89.29%) and 2697 had a breach <2mm (relative accuracy of 97.56%). No difference was found among groups in terms of absolute or relative accuracy (P=0.06 and P=0.23, respectively). AIS cases assisted by senior residents have longer operative time and higher blood loss, but this does not negatively affect the overall radiographic outcome and does not place the patient at increased risk of complications. III.

Severe (>100 Degrees) Thoracic Adolescent Idiopathic Scoliosis - A Comparison of Surgical Approaches.

Retrospective. Severe curves >100° in adolescent idiopathic scoliosis (AIS) are rare and require careful operative planning. The aim of this study was to assess baseline, perioperative, and 2-year differences between anterior release with posterior instrumentation (AP), posterior instrumentation with posterior column osteotomies (P), and posterior instrumentation with 3-column vertebral osteotomies (VCR). Two scoliosis datasets were queried for primary cases of severe thoracic AIS (≥100°) with 2-year follow-up. Pre- and 2-year postoperative radiographic measures (2D and estimated 3D kyphosis), clinical measurements, and SRS-22 outcomes were compared between three approaches. Sixty-one patients were included: 16 AP (26%), 38 P (62%), 7 VCR (11%). Average age was 14.4 ± 2.0 years; 75.4% were female. Preoperative thoracic curve magnitude (AP: 112°, P: 115°, VCR: 126°, P = 0.09) and T5-T12 kyphosis (AP: 38°, P: 59°, VCR: 70°, P = 0.057) were similar between groups. Estimated 3D kyphosis was less in AP vs P (-12° vs 4°, P = 0.016). Main thoracic curves corrected to 36° in AP vs 49° and 48° for P and VCR, respectively (P = 0.02). Change in estimated 3D kyphosis was greater in AP vs P and VCR (34° vs 13°, P = 0.009; 34° vs 7°, P = 0.046). One incomplete spinal cord injury had residual deficits (P; 1/61, 1.6%). All SRS-22 domains improved postoperatively. All approaches obtained satisfactory coronal and sagittal correction, but AP had smaller residual coronal deformity and greater kyphosis restoration than the other approaches. This information may help inform the decision of whether to include an anterior release for large thoracic AIS curves.

Two AIS Cases, 2 Surgeons, 1 Operating Room, 1 Day: Faster and Safer Than 1 Case in a Day.

To lessen surgical times for patients with adolescent idiopathic scoliosis (AIS) undergoing posterior spinal instrumentation and fusion (PSIF), our department developed a quality improvement initiative where 2 AIS cases were completed in 1 day by the same 2 surgeons operating together in 1 operating room (OR). We describe the results of this initiative, comparing operative times and outcomes to cases of these surgeons operating individually. From 2017 to 2023, patients aged 10 to 18 years with AIS undergoing PSIF were prospectively enrolled for "Two Spine Tuesday." Patients were matched by age, sex, curve severity, and number of levels fused to historical AIS controls. Outcomes included surgery time, total OR time, estimated blood loss (EBL), volume of cell saver transfused, allogenic blood transfusion, length of stay, 90-day readmissions, Clavien-Dindo-Sink Complication Classification System complication rates, and percentage who achieved the minimal clinically important difference (MCID) for SRS-22. Fifty-five patients composing the 2-spine group (group 2) were compared with 55 historical sex-matched and age-matched controls (group 1). Major coronal curve and average number of levels fused were similar between groups. Overall surgery time (203 vs. 296min, P <0.001), total OR time ( P <0.001), and EBL (400 vs. 550mL, P <0.001) were lower for group 2. Group 2 had fewer complications [n=17 (31%) vs. n=28 (51%), P =0.03]. Performing 2 AIS cases in 1 OR by 2 surgeons the same day resulted in shorter surgery times, less total time in the operating room, lower complication rates, and less blood loss compared with single-surgeon matched controls. Comparative Analysis of Monoaxial and Polyaxial Pedicle Screws in the Surgical Correction of Adolescent Idiopathic Scoliosis.

Background: Although several biomechanical studies have been reported, few clinical studies have compared the efficacy of monoaxial and polyaxial pedicle screws in the surgical treatment of adolescent idiopathic scoliosis (AIS). This study aims to compare the radiological and clinical outcomes of mono- and polyaxial pedicle screws in the surgical treatment of AIS. Methods: A total of 46 AIS patients who underwent surgery to treat scoliosis using pedicle screw instrumentation (PSI) and rod derotation (RD) were divided into two groups according to the use of pedicle screws: the monoaxial group (n = 23) and polyaxial group (n = 23). Results: The correction rate of the main Cobb's angle was higher in the monoaxial group (70.2%) than in the polyaxial group (65.3%) (p = 0.040). No differences in the rotational correction of the apical vertebra were evident between the two groups. SRS-22 scores showed no significant differences according to the type of pedicle screws used. Conclusions: The use of polyaxial pedicle screws resulted in coronal, sagittal, and rotational correction outcomes comparable to those associated with the use of monoaxial pedicle screws for surgical treatment using PSI and RD to treat moderate cases of AIS.

Range of international surgical strategies for adolescent idiopathic scoliosis: Evaluation of a multi-center survey.

Surgical treatment of adolescent idiopathic scoliosis (AIS) is very complex and modern instrumentation techniques offer multiple possibilities. Despite numerous publications, there is no clear consensus on the optimal strategy for the correction of scoliotic deformities. The goal of this study was to summarize the current surgical strategies for specific AIS cases within various countries. Thirty-two experienced scoliosis surgeons from 15 countries were asked to plan surgeries on 12 representative AIS cases. All AIS cases had an indication for surgery. A questionnaire was provided to document surgical planning. The surgeons were provided with the patients' age and sex, together with radiographs in the lateral and sagittal planes during upright standing and in lateral bending to the left and right, as well as with clinical images. The angles of the main spinal curvatures were specified in the questionnaire. The surgeons were asked to specify their preferred classification system, their surgical approach, the planned fusion length, the type of implants, the rod type, and the resection steps. The data were analyzed with respect to the inter-rater variability, which was quantified using the Fleiss-Kappa Method. There was a good agreement (k = 0.61) between the surgeons in choosing the Lenke curve type, and a moderate agreement for the lumbar (0.41) and sagittal (0.56) modifiers. The most frequently planned resection procedure was complete facetectomy (67%). The posterior approach was the most commonly (91%) selected strategy to treat AIS. Anterior approaches were chosen most for Lenke 5 type with a rate of 20%. The upper instrumented vertebra (UIV) varied most for Lenke 1, 5, and 6 cases, with a vertebral level discrepancy of up to 10 levels at Lenke 6. The lowest instrumented vertebra varied most for Lenke 1 and 4 by up to five levels. Polyaxial screws were chosen most (56%), followed by monoaxial (20%) and uniplanar (19%) screws and hooks (5%). The results highlight the commonalities and discrepancy in the surgical treatment of AIS in between surgeons. The selected LIV and UIV can vary depending on the curve type and surgeon. Hook constructs appear to be generally replaced by transpedicular screws. The survey indicates open questions in the AIS treatment and in the understanding of scoliosis biomechanics.

The effect of surgical time on perioperative complications in adolescent idiopathic scoliosis cases. A propensity score analysis.

Posterior spinal instrumentation and fusion (PSIF) for adolescent idiopathic scoliosis (AIS) can be lengthy and complication-ridden. The aim of this study was to evaluate the effect of surgical time on perioperative complications in this procedure when controlling for confounding variables with propensity score analysis. This was an IRB-approved review of electronic health records from 2010 to 2019 at a single tertiary care children's hospital. Patients undergoing PSIF were grouped into "short" (< 6h) or "long" (≥ 6h) surgical time groups. Outcome measures were estimated blood loss (EBL), cell saver transfusions, packed red blood cell (pRBC) transfusions, length of stay (LOS), intraoperative monitoring (IOM) alerts, hematocrit, ICU transfer, neurologic loss, surgical site infection, and 90-day readmissions. We controlled for age, sex, BMI, curve severity, number of segments fused, and surgeon factors. After propensity score matching there were 113 patients in each group. The short surgical time group had lower EBL (median 715, IQR 550-900 vs median 875, IQR 650-1100cc; p < 0.001), received less cell saver blood (median 120, IQR 60-168 vs median 160, IQR 97-225cc; p = 0.001), received less intraoperative pRBCs (median 0, IQR 0-0 vs median 0, IQR 0-320, p = 0.002), had shorter average LOS (4.8 ± 1.7 vs 5.4 ± 2.5days; p = 0.039), and fewer IOM alerts (4.3% vs 18%, p = 0.003). Patients with shorter surgical times had less blood loss, received less transfused blood, had a shorter LOS, and fewer IOM alerts compared to patients with longer surgical times. Surgical times < 6h may have safety and efficacy advantages over longer times. III.

Aborted AIS spinal fusion due to persistent loss of IONM: which patients are at greatest risk?

Determine peri-operative risk factors predictive for prematurely stopping surgery prior to completion of deformity correction due to intra-operative neuromonitoring changes. A single institution retrospective review of adolescent idiopathic scoliosis (AIS) patients that underwent spinal fusion for curves greater than 70°. Cases aborted due to persistent loss of IONM were compared to completed cases. Demographic, radiographic, neurologic, and surgical information was reviewed. There were 453 total cases. Nine (9/453: (2%)) cases were aborted due to persistent loss of IONM, and 4 (4/453; (0.88%)) awoke with a neurologic deficit. Comparing to the 444 completed cases, pre-operative risk factors associated with case abortion were older age (15.3 vs. 13.8 years; p = 0.02), sex (male) (66.7% vs. 20.3%, p = 0.004), and larger cobb angles (87.6° vs. 79.2°; p = 0.01). Being male increased the risk of case abortion: 7.9X. Intraoperative risk factors associated with case abortion were combined anterior/posterior approach (ASF/PSF) (44.4% vs. 7.2%; p = 0.003) and increased index procedure EBL (1127 vs. 769mL; p = 0.043). ASF/PSF increased the risk: 10.3X. Four (4/9;44%) of the aborted cases awoke with neurologic deficit. Motor strength returned at 2.3days (0-18). Aborted cases returned to the OR after 12.6 ± 7.0days (1-23) which was related to time to regain motor strength. Pre-operative risk factors for AIS case abortion due to persistent loss of IOMN are older age, males, with larger Cobb angles. Intraoperative risk factors are combined ASF/PSF and increased index procedure EBL. Independent risk factors were sex (male) and ASF/PSF which increased the risk 7.9X and 10.3X, respectively.

Exploring the Effectiveness of Hatha Yoga as a Complementary Treatment for Adolescent Idiopathic Scoliosis: Clinical Effect and Future Research Directions.

Adolescent idiopathic scoliosis (AIS) is a common spinal deformity that primarily affects adolescents during the key period of growth and development. While traditional treatment methods often involve bracing or surgery, Hatha yoga, a millennia-old practice rooted in Indian tradition, has emerged as a complementary option for AIS cases. This paper explores the potential benefits of Hatha yoga for adolescents with AIS. It also discusses the limitations of existing research, such as the lack of large-scale randomized controlled trials (RCTs), varying yoga protocols, and challenges in blinding participants and researchers. To address these limitations, I propose future research directions, including conducting large-scale RCTs, long-term follow-up studies, standardized yoga protocols, and assessing safety concerns. I also highlight the need for tailored interventions and comparative effectiveness studies to better understand the potential of Hatha yoga in the holistic treatment of AIS in adolescents.

Can We Control Lateral Shoulder Balance Through Proximal Thoracic Curve Correction in Lenke Type 2 Adolescent Idiopathic Scoliosis?

Proximal thoracic curve (PTC) correction has been considered to prevent lateral shoulder imbalance in Lenke Type 2 adolescent idiopathic scoliosis (AIS) patients; however, postoperative shoulder imbalance (PSI) commonly occurs despite these strategies with PTC correction. We investigated the hypothesis that PTC correction would not directly affect PSI in the majority of Lenke type 2 AIS cases. Furthermore, we investigated the risk factors for lateral PSI after corrective surgery. This study examined the records for AIS patients with Lenke type 2 who underwent corrective surgery and followed up for >2 years. Patients were categorized into PSI (-); radiologic shoulder height (RSH)<15mm, and PSI (+); RSH≥15mm. Repeated measures analysis of variance was performed at preoperatively, postoperatively, 1 month, and final follow-up. Postoperative lateral shoulder imbalance was predicted by the identification of univariate analysis and multivariate analysis. Among the 151 patients reviewed, 29 (19.2%) showed PSI at final follow-up. Lateral shoulder balance parameters showed different directionalities between PSI (-) and (+) groups at postoperatively, 1 month, and final follow-up (P<0.01 each). Preoperative PTC, middle thoracic curve (MTC) curve and MTC correction showed strong correlations with the RSH (P=0.01, 0.03, and 0.04, respectively). However, PTC correction did not show a significant correlation with the RSH. Moreover, only a smaller MTC curve and larger MTC correction rate were related to lateral PSI in multivariate analysis. In Lenke type 2 AIS curves, the MTC curve and its correction predominantly influence lateral shoulder imbalance after corrective surgery, irrespective of the PTC correction extent. Consequently, overemphasizing the correction of the PTC curve may not necessarily lead to an improved lateral shoulder balance. When MTC curve is smaller, surgeons should be more careful for MCT overcorrection leading to a lateral shoulder imbalance. Level III.

Range of surgical strategies for individual adolescent idiopathic scoliosis cases: evaluation of a multi-centre survey

PurposeSurgical treatment of adolescent idiopathic scoliosis (AIS) is very complex, involves many critical decisions and modern instrumentation techniques, and offers multiple possibilities. It is known that the surgical strategy may vary strongly between surgeons for AIS cases. The goal of this study was to document, summarize, and analyse the current biomechanical relevant variabilities in the surgical treatments of individual AIS patient cases.MethodsEight experienced scoliosis surgeons from different hospitals were asked to plan surgeries on 12 representative patients with AIS. The surgeons were provided with radiographs during upright standing in the coronal and sagittal plane, as well as lateral bending images to the left and right. The surgeons were asked to specify the Lenke type, their surgical approach, the resection steps, the planned fusion length, and the type of implants. The data were analysed with respect to the inter-rater variability, which was quantified using the Fleiss Kappa method.ResultsIn the selection of the surgical approach, the surgeons concurred most with Lenke curve types 2 (κ = 0.88) and 4 (κ = 0.75). The largest differences were shown at Lenke 1 (κ = 0.39) and 5 (κ = 0.32). Anterior approaches were selected in the majority of cases at Lenke types 5, with an average of 50%. The strongest deviation in fusion length was documented at Lenke curve type 6.ConclusionThe survey highlighted differences in the surgical strategy depending on the Lenke curve type, the direction of the surgical approach, and the surgeon. The main discrepancies between the surgeons were found for Lenke 1, 5, and 6 curves, and consistencies for Lenke 2, 3, and 4. The documented discrepancies indicate the remaining open questions in the surgical treatment and understanding of scoliosis biomechanics.

Association between lean mass and adolescent idiopathic scoliosis: a meta-analysis

ObjectiveThe objective of this meta-analysis was to evaluate the association between lean mass and adolescent idiopathic scoliosis (AIS).MethodsEnglish databases CENTRAL (The Cochrane Library and the Cochrane Back Review Group Trials Register), MEDLINE, EMBASE, PubMed, Web of Science and Chinese databases CBM, CNKI, VIP, WANGFANG DATA were searched for the relevant case control studies and cross-sectional studies. Two authors selected studies and extracted data independently. Data analysis was performed by Stata15.0.ResultsEight studies were included, with a total of 1771 cases of AIS and 6340 controls. AIS group had a lower lean mass compared to control group [MD = − 1.95, 95% CI (− 2.96, − 0.93)]. In the subgroup analysis, female AIS patients had a lower lean mass than the control group [MD = − 1.76, 95% CI (− 2.63, − 0.88)]. The mean difference of lean mass between AIS patients and control group in studies with adults [MD = − 3.96, 95% CI (− 7.26, − 0.67)] is much greater than studies without adults [MD = − 1.04, 95% CI (− 1.59, − 0.49)]. There was not statistically significant in European studies [MD = − 2.10, 95% CI (− 4.35, 0.14)], but in Asian studies lean mass in AIS patients was lower than the control group [MD = − 2.26, 95% CI (− 3.98, − 0.54)]. Study type, gender, age, and geography condition were thought to have no effect on the primary outcome of lean mass by subgroup analysis.ConclusionIn the meta-analysis, AIS group had a lower lean mass compared to control group, which indicated that lean mass may be involved in the pathogenesis of AIS. But limited by the number of studies we included; the above conclusions need to be validated by more high-quality studies.

Variations in Duration of Clinical Follow-up After Spinal Fusion for Adolescent Idiopathic Scoliosis: A Survey of POSNA and SRS Membership

Background: There are currently no evidence-based guidelines addressing the optimal duration of follow-up after posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS). Despite the safety and efficacy of PSF for AIS, long-term complications exist, including infection, pseudoarthrosis, adjacent segment disease, deformity progression, persistent pain, and junctional deformities. In this study, we describe practice variation existing among surgeons regarding duration and intervals of patient follow-up after surgical treatment of AIS in addition to factors influencing provider recommendations for duration of radiographic and clinical follow-up. This investigation of practice variation is important, as opportunities exist for both limiting unnecessary radiation exposure, while also identifying opportunities for timely intervention to help decrease the morbidity of late complications arising after spinal fusion.Methods: An anonymous online survey was created and subsequently distributed to members of POSNA and SRS to assess practice demographics and surgeon opinions surrounding duration of surveillance following surgery for AIS. Only surgeons who treated at least five operative AIS cases within the past year were included. Descriptive statistics and comparative sub-analyses are presented.Results: Forty-nine participants met inclusion criteria. Respondents were mainly pediatric orthopaedic surgeons (92%) in practice for 21-50 years (49%) who performed approximately 21-50 operative AIS cases per year (49%). Forty-eight percent of providers had an age limit in their practice, and 52% regularly followed operative AIS patients over 18 years of age. Sixty-two percent of surgeons followed operative AIS patients for 2-5 years postoperatively, whereas only 4% followed for more than 10 years. The most cited factors impacting follow-up recommendations were junctional deformities, adjacent segment disease, and symptomatic implants. There were no significant associations between years in practice, operative volume, and recommendations for duration of follow-up after routine operative AIS cases.Conclusions: Significant variability in duration of follow-up after PSF for AIS exists. Although most patients are clinically followed for 2 years after surgery, only a small percentage of providers follow AIS patients for more than 10 years postoperatively. Numerous AIS revisions occur more than 5 years after the index surgery. Further investigations to determine the optimal duration of surveillance following PSF for AIS should be conducted.Level of Evidence: V Key Concepts•Late complications following posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS) may occur 5-10 years after the index surgery and currently, there are no guidelines that outline the optimal duration of postoperative follow-up.•Significant variability exists in pediatric orthopaedic provider recommendations for long-term follow-up after PSF for AIS and is not associated with surgeon experience or case volume.•Most providers follow operative AIS patients for 1-2 years postoperatively, and only a small minority follow operative AIS patients for more than 10 years after the index surgery.•Persistent back pain, junctional deformity, and symptomatic implants are the most common factors affecting provider recommendations for duration of follow-up.•These survey findings may be useful for pediatric orthopaedic providers to operatively manage AIS patients in determining the need and frequency for routine radiographic and/or clinical follow-up.

The association between intraoperative fluid management and perioperative allogenic blood transfusion during adolescent idiopathic scoliosis surgery.

Bleeding and transfusion remain important concerns during surgical correction of scoliosis even when multiple conservative strategies, such as preoperative recombinant erythropoietin and/or antifibrinolytic agents, are used. The current work aimed to determine the impact of other potential risk factors, especially the volume of intraoperative fluid intake, on the perioperative risk of allogenic transfusion during surgical correction of adolescent idiopathic scoliosis. This prospective study included all cases of adolescent idiopathic scoliosis operated in a single center during 2 years (2018-2020). Predictors analyzed were as follows: body mass index, preoperative hemoglobin concentration, thoracoplasty, preoperative halo-gravity, volume of intraoperative crystalloid administration, use of esophageal Doppler (for goal-directed fluid therapy), and duration of surgery. Statistical analyses were performed using a multivariable logistic regression model. Two hundred patients were included in the analysis. Multivariable analysis found: an increased volume of intraoperative crystalloid administration as a significant predictor of allogenic blood transfusion. Receiving operator characteristics analysis found the model exhibiting an area under the curve of 0.85 (95% confidence interval: 0.75-0.95). Optimizing stroke volume using esophageal Doppler was associated with a decrease in intraoperative crystalloid intake. These results indicate a statistical association between the increase in crystalloid intake and the risk of allogenic blood transfusion during surgical correction of adolescent idiopathic scoliosis. Controlled studies are needed to address the causative relation between intraoperative fluid intake and the risk of allogenic transfusion.

Biomechanical analysis of rod contouring in posterior spinal instrumentation and fusion for 3D correction of adolescent idiopathic scoliosis.

To biomechanically evaluate 3D corrective forces and deformity correction attributable to key parameters of rod contouring in posterior spinal fusion (PSF) for adolescent idiopathic scoliosis (AIS). Computerised patient-specific biomechanical models of six AIS cases were used to simulate PSF and evaluate the effects of 5.5-mm cobalt-chrome rod contouring angle (concave-convex angles: 30°-15°, 45°-15° and 60°-15°), length (spanning 4 and 7 vertebrae), and apex location (T7, T9). 3D correction and bone-implant forces were computed and analysed. By increasing the concave rod contour from 30° to 60°, thoracic kyphosis (TK) increased from 18° ± 2° (15°-19°) to 24° ± 2° (22°-26°), apical vertebra rotation (AVR) correction increased from 41%(SD8%) to 66%(SD18%) whilst the main thoracic curve (MT) correction decreased from 68%(SD6%) to 56%(SD8%). With a contouring length of 4 vs. 7 vertebrae, the resulting TK, AVR and MT corrections were 22° ± 1° (19°-26°) vs. 19° ± 10° (15°-22°), 57%(SD18%) vs. 50%(SD26%) and 59%(SD1%) vs. 69%(SD35%), respectively. With the rod contouring apex at T7 (vs. T9), AVR corrections were 69% (SD19%) vs. 44% (SD9%), with no significant difference in TK and MT corrections, and with comparatively 67% of screw pull-out forces. Corrective forces were more evenly shared with fixation on 7 vs. 4 vertebrae. Rod contouring of a greater angulation, over a shorter portion of the rod, and more centred at the apex of the main thoracic curve apex improved AVR correction and allowed greater restoration of TK, but resulted in significantly higher screw pull-out forces and came at the expense of less coronal plane correction.

Finite element simulation of growth modulation during brace treatment of adolescent idiopathic scoliosis.

Adolescent idiopathic scoliosis (AIS) is a spine deformity whose progression during growth is affected by asymmetrical loads acting on the spine. The conservative brace treatment aims to limit the deformity's progression until the end of skeletal growth. This study's objective was to develop a patient-specific finite element model (FEM) simulating immediate in-brace correction and subsequent growth modulation over 2 years of treatment. Thirty-five retrospective AIS cases with documented correction over 2 years were analyzed. For each case, a patient-specific FEM was built, and in-brace correction was simulated. Vertebral growth and its modulation were modeled using simulated pressures on epiphyseal vertebral growth plates, including a compliance factor representing the recorded brace wear. The simulated Cobb angles, thoracic kyphosis, lumbar lordosis and apical vertebral rotation were compared with the actual measurements immediately in-brace and out-of-brace at the 2-year follow-up. Treatment outcomes according to simulated compliance scenarios of no brace-wear vs. full brace-wear were also computed. The average immediate in-brace difference between the simulated and actual Cobb angle was 4.9° (MT) and 3.7° (TL/L). 2-year out-of-brace, it was 5.6° (MT) and 5.4° (TL/L). The no brace-wear and full brace-wear compliance scenarios resulted respectively in 15/35 (43%) and 31/35 (89%) simulated spine deformities progressing by less than 5° over 2 years of treatment. Clinical significance: the FEM's ability to simulate the final correction with an accuracy on the order of the radiological measurements' interoperator reproducibility, combined with its sensitivity to brace-wear compliance, provides confidence in the model's predictions for a comparative context of use like improving a brace's design before its application. Surgical Site Infection Following the Correction of Adolescent Idiopathic Scoliosis With ApiFix: A Retrospective Study Analyzing Its Incidence and Recurrence.

Background and objective ApiFix (OrthoPediatrics, Warsaw, IN) is an internal brace used for the correction of adolescent idiopathic scoliosis (AIS) classified as Lenke 1 or 5 with a Cobb angle of 35-60 degrees that decreases to ≤30 degrees on lateral side-bending radiographs. Since the indications are very specific, it is not a common procedure. Our study aimed to evaluate the incidence of surgical site infection (SSI) and its recurrence following treatment with ApiFix. Materials and methods A retrospective study of 44 cases of AIS treated at our center from 2016 to 2022 with ApifiX was conducted. Two patients who presented with SSI were initially treated with irrigation and debridement (I&D) following antibiotic therapy. Results A total of 44 patients with a mean age of 15.1 years were evaluated. Two of our patients presented with early-onset infection while one of them presented after the end of treatment with a skin ulcer due to septic screw loosening. The removal of the ApiFix implant revealed a pedicle abscess during the screw removal. Conclusions In this study of 44 patients, we observed two cases of infection and one case of reinfection. Given the limited muscle detachment and short operating time needed for Apifix, statistics suggest that the risk of SSIis always present. Further randomized trials are needed to gather more evidence on this subject.

Academic-related factors and daily lifestyle habits associated with adolescent idiopathic scoliosis: a case-control study

Adolescent idiopathic scoliosis (AIS) is the most prevalent spinal deformity, which may have long-term negative consequences on adolescents. The research on the etiology is of great importance for identifying high-risk population and formulate tailored prevention. This study aimed to evaluate the association between academic-related factors and daily lifestyle habits and AIS. In this population-based case-control study, 491 AIS cases and 1,346 healthy controls that frequency-matched by age and sex were recruited in Shenzhen, Southern China. AIS was diagnosed as a Cobb angle ≥ 10° on standing posteroanterior radiographs of the whole spine. The academic-related factors (e.g., reading and writing posture) and daily lifestyle habits (e.g., intake of milk and dairy products) were collected by a self-reported questionnaire. The logistic regression analysis was performed. After adjusting for potential confounding factors, multivariable logistic regression models demonstrated that academic-related factors were associated with AIS. Individuals with poor reading and writing posture were more likely to have AIS (AOR: 2.06, 95%CI: 1.58-2.68). Moreover, there was a significant association between heavy school bags and AIS (AOR: 2.22, 95%CI: 1.50-3.31). Additionally, adolescents who reported daily screen time on weekdays over 2 hours were more likely to develop AIS (P < 0.001). Regarding daily lifestyle habits, individuals without the habit of taking milk and dairy products had a higher risk of developing AIS (AOR: 1.87, 95%CI: 1.29-2.71). Academic-related factors and daily lifestyle habits were associated with AIS among Chinese adolescents. Schools, families, and related facilities are recommended to take actions on developing effective prevention and management strategies that integrates "Student-Family-School-Education-Health-Sports" for AIS.

Radiation in Adolescent Idiopathic Scoliosis Management: Estimated Cumulative Pre-Operative, Intra-Operative, and Post-Operative Exposure.

Adolescent Idiopathic Scoliosis (AIS) is the most common type of scoliosis affecting adolescents, with approximately 2-4% of children being diagnosed. Crucial to the diagnosis and management are radiographic imaging, which allow physicians to assess and treat - from initial visits through surgical planning and post-operative management. While initial stages require low levels of exposure to radiation, via x-rays, as patients progress in disease severity exposure becomes larger with pre-operative, intra-operative and post-operative CT scans. While many studies have evaluated exposure during AIS treatment, few have assessed the cumulative radiation exposure adolescents receive during their evaluation. The purpose of our study is to complete a comprehensive review on cumulative radiation exposure and determine what stages in AIS treatment expose patients to the highest level of radiation over a duration of 2 years. A retrospective chart review of 109 AIS cases (29M, 80F), mean age 14.9 ± 2.3 years was completed to assess and quantify each imaging modality used. Employing a radiation detector, each imaging modality was then assessed, and estimated radiation exposures were determined. Statistical analysis was completed utilizing averaged patient exposures during each selected period in AIS management. Mean estimated radiation doses (StDev) were 60.94 mrem (±0.609 mrem) for two x-rays (full-length AP and lateral radiograph of the entire spine), 12.92 mrem (±1.292 mrem) for each fluoroscopy exposure, and 1340.60 mrem (±13.406 mrem) per CT scan. Based on these values, estimated subject exposures were calculated. The total estimated radiation exposure over a 2-year period was 5572.74 mrem (±1428.88 merm) or 2786.37 mrem (±714.43 mrem) per year. The two-year cumulative radiation exposure is below the recommended exposure by Nuclear Regulatory Commission and OSHA. As expected, CT exposure presents the largest radiation exposure to patients with AIS throughout their operative management. III, retrospective study.

The role of pineal gland volume in the development of scoliosis.

Adolescent idiopathic scoliosis (AIS) is believed to be caused by genetic, neurological, osseous growth anomalies, histological variables including muscle fiber percentage and core structure changes, metabolic and hormonal dysfunction, vestibular dysfunction, and platelet microarchitecture. The objective of this study was to contribute to the determination of the cause of AIS by analyzing the changes in pineal gland volume in AIS cases. Study (AIS) and control group were each comprised of 26 patients who met the inclusion requirements. Scoliosis radiograph and MRI of the pineal glands were used for radiological examinations. The distribution of age, gender, Risser grading for skeletal radiological development, and sexual maturation according to Tanner categorization were uniform and statistically insignificant between groups. When the pineal gland volumes of the cases were evaluated according to age, the AIS group was found to have significantly reduced pineal gland volumes in all age groups. The pineal gland volume was found to be 38.1% lower in the AIS group compared to the control group (p˂0.001). In the AIS group, patients aged 13years had the lowest pineal gland volume (77.2 ± 13.86 mm3), while patients aged 15years had the highest volume (97.9 ± 16.47 mm3). Changes in pineal gland volume support the role of the pineal gland in the etiopathogenesis of AIS.
