The effect of diabetes management on the workload of district and community nursing teams in the UK.

The number of older people with diabetes requiring care from district nursing teams is increasing. The role of district nursing teams in diabetes management has expanded to involve diagnosis, treatment and medication administration. As the complexity of caseloads increases, the current model is likely unsustainable. This study aims to understand the current diabetes workload of district nursing teams. An online survey was distributed via social media and key stakeholder networks to district nursing teams. Survey items were designed by the researchers prior to pilot testing with potential participants. Descriptive statistical and qualitative analyses were conducted. Data are median ± IQR. 159 district nursing teams completed the survey. The median caseload per team was 300 (IQR 176-407) patients including 21 with diabetes (IQR 14-40; 8.7% (4-20%)). 1.09 home visits per day per person with diabetes lasting 13.8 minutes (excluding travel time) were needed, with most requiring insulin administration. 96% of nursing teams undertake multiple daily visits for some patients. 91% reported workloads relating to diabetes management had increased over the last 2 years; 76% stated current diabetes workloads were unsustainable. More insulin usage, more referrals and a lack of ability or willingness to self-administer insulin has increased the diabetes workload. Possible solutions include better collaboration between healthcare professionals, simplification of insulin administration and glucose monitoring, better training and upskilling of healthcare assistants and promotion of self-efficacy. Diabetes management forms an increasing component of district nursing workload and is likely to be unsustainable unless new models are found.

An effective secondary personalization file system driven by FileForge module

Digital service platforms provided by academic support departments in Macao assist academic staff and students in various areas such as registry, student affairs, academic activities, and research. As the number of undergraduate students increases and new departments are established, academic staff often face the challenge of dealing with paperwork that contains similar content but different formats. This situation results in redundancies and a waste of time. This paper presents our endeavors to simplify administrative procedures in higher education by automating restructured documentation and developing secondary file systems. The paper presents two case studies: Scenario One focuses on streamlining the publication system for academic staff who submit papers in different formats. At the same time, Scenario Two aims to simplify the daily paperwork process for academic staff. Both cases involve transforming the distribution of administrative documents, transitioning from a standardized form with guidelines to a customized form with concise tips. This approach allows academic staff to handle only the necessary information, which may not be available in the database or requires verification. The case studies serve to demonstrate the effectiveness of this administrative simplification.

526 Administrative Simplification of Committee Reviews through REDCap

OBJECTIVES/GOALS: A Mayo Clinic in Florida committee completes 100+ Scientific Reviews annually through manual e-mail and Excel tracking, placing a manual burden on reviewers and coordinators. REDCap, an electronic data capture system, was leveraged to reduce the administrative burden.#_ftn1 #_ftnref2 METHODS/STUDY POPULATION: Historically, emails were sent by a coordinator to physicians, requesting their initial review and following up with reminders. This process was tracked using Excel, presenting a need to make this process more efficient, so a workgroup was created. To ensure all perspectives were accounted for, the workgroup included the review coordinator, a physician reviewer, and study team members who submit development requests for studies. The process was mapped using the existing Excel spreadsheet, and email templates. Critical data elements were identified, ensuring the database would identify bottlenecks. Two REDCap instruments were then created: one to outline the coordinator workflow and a survey for physician reviewers to complete the scientific reviews. #_ftn2#_ftnref1 RESULTS/ANTICIPATED RESULTS: The workflow is live in REDCap and has effectively processed over 100 scientific reviews in <1 year. The system captures the review status and guides the coordinator through the workflow, capturing dates when tasks are completed. When review criteria are met, the database sends an email to the assigned reviewer. The email includes a link to a REDCap survey, containing all pertinent documents. The reviewer uploads their completed review form within the survey, if this is not completed within a given period, the database sends email reminders. Once the review is complete, a notification is sent to the review coordinator. The review workflow is accessible to the study team who requested scientific review, making them aware of their request status ad giving them access to the review the moment it is completed. DISCUSSION/SIGNIFICANCE: Leveraging REDCap has increased visibility, reduced overall manual processes, and simplified the reviewer burden by providing all the information needed in a single notification. REDCap is a cost-effective, impactful solution to simplifying administrative burden in managing committee reviews.

Upstream and downstream implementation arrangements in two‐level games. A focus on administrative simplification in the Italian National Recovery and Resilience Plan

AbstractThe onset of the European NGEU program represented for European member states a formidable opportunity for post‐pandemic recovery and yet a significant challenge at the same time: to receive and retain EU funds, each state had to promptly draw up a National Recovery and Resilience Plan—NRRP) and commit to a pressing timetable for its implementation. Regarding this challenge, the Italian government's response is a case in point: first, Italy is by far the largest beneficiary of NGEU funds; second, it has long had a reputation for being laggard in both the implementation of European directives and the spending of cohesion policy structural funds; and the formulation of the NRRP, the design of the governance in charge of its implementation, and implementation itself, occurred at a particular moment in the country's political life. Based on these premises, the article examines the upstream process by which the Italian government designed the implementation arrangements for the adoption of simplification policies under the NRRP and their downstream recalibration in the first two years, taking the implementation arrangements as the dependent variable. Analytically, the focus is on the interplay between the pressures of EU timetables and internal political dynamics, be they the legacy or strategic political considerations on the part of national policymakers, in determining the design and eventual re‐design of implementation arrangements as the plan unfolds.

E-GOVERNMENT: IMPLEMENTATION OF ASAN SERVICE MODEL FOR PUBLIC SERVICE REFORM IN NIGERIA AND SUDAN

Despite technological developments, customer acceptability is constrained due to a lack of functional knowledge. E-government growth ambitions do not include administrative simplification, lack of infrastructure and human resource capabilities, there is a mismatch between supply and demand for e-services. the Ineffective government regulations, scarcity of technical skills and exorbitant technological costs. Politicians, civil servants, and all public servants are seen as the actual enemy in these two countries, depriving them of progress and national development. This quantitative study proposed that the Azerbaijan ASAN service model be adopted as an alternative public services reform approach for reimagining Nigerian and Sudanese public services sectors. The findings indicate that Strongly Agree (n=3). Agree (N=21) contradicted disagreed (n=17) and strongly disagree (n=9). To summarize, n=25 respondents genuinely disagree that people in their country were aware of e-government services, which runs counter to (n=24). This result showed that almost everyone who participated in the survey agreed and recognized the value of using the Azerbaijan ASAN services model to make it easier for residents to receive government services in a corrupt-free digital environment. In conclusion, the adoption of e-Government services that are more advantageous, user-friendly, effective, and efficient may be significantly impacted by the study's findings, in our opinion. We recommend that the government to adopt and provide electronic services like ASAN service E-government of Azerbaijan.

Elotuzumab: Empiric analysis of dexamethasone administration schedule.

Elotuzumab in combination with dexamethasone and immunomodulating agents (IMiDs) lenalidomide or pomalidomide is 2nd to 4th line therapy for multiple myeloma. The labelled dosage of dexamethasone for use in conjunction with elotuzumab and IMiDs splits the dexamethasone dose into two administrations, one oral and one intravenous, on the days of each elotuzumab infusion. Administration of split dose dexamethasone on days of elotuzumab administration is based on the registration trials submitted for drug approval and was intended to ensure standard well-timed immunotherapy premedication using pharmacologically equivalent dexamethasone doses for both study arms. Administration of dexamethasone in the manner delineated by the elotuzumab product label adds complexity to the delivery of care. This commentary provides an empirical assessment of established medication safety and effectiveness which supports administration of dexamethasone standard intermittent dose instead of the split dose approach delineated on elotuzumab package insert. Simplification of regimen administration improves medication adherence, reduces the risk of inadvertent omission or duplication of medication therapy, and improves the workflow required for delivery of care.

The impact of decentralization on the speed of provision of administrative services in wartime conditions

The article focuses on the disclosure of the content of administrative services under the impact of decentralisation and the activities of relevant bodies aimed at ensuring human and civil rights and freedoms, interests of legal entities in wartime, and identifying factors affecting the speed of their provision. It has been noted that, in order to improve the quality and speed of administrative service provision in the context of war under the influence of decentralisation, the issue of decentralisation of powers to provide administrative services at the level of local self-government bodies is becoming particularly relevant, in particular through the instruments of delegation, transfer of powers and budget revenues from state bodies to local self-government bodies.
 It has been noted that the legal institution of administrative services is a system of relatively separate and interrelated legal provisions which regulate relations arising in the course of exercising subjective rights of an individual or a legal entity (at their request) in the course of public authority activities of an administrative body. Administrative services as part of public are provided by executive authorities and local self-government bodies. Their provision is related to the exercise of power.
 On the basis of the analysis of scholars’ scientific positions, the terms “public service”, “administrative and legal instrument”, “administrative service” have been studied. The factors affecting the speed of providing quality basic administrative services under the influence of decentralisation in the context of war have been identified, and the general prospects for the development of the administrative service delivery system have been determined, in particular: regulation of payment for administrative services, deregulation and administrative simplification to reduce the number of administrative services and simplify the procedures for providing specific services. The importance of information exchange between electronic registers, the possibility of using electronic services throughout the country, and optimising the distribution of powers between different levels of public administration has been emphasised.

The article discusses the issue of functioning of small and medium-sized businesses in Ukraine, their development in the economic, political and social spheres under conditions of high uncertainty. The current state of small and medium-sized businesses is analyzed and the possibilities of ensuring their sustainability and further development are defined. The approaches to the definition of entities of micro-entrepreneurship, small, medium, and large entrepreneurship in accordance with the legislation of Ukraine are systematized. The main parameters of micro-enterprises, small, medium, and large enterprises are specified. The main problems of the development of small business in Ukraine before the war are highlighted, in particular, the lack of an effective mechanism of the State support and protection of small and medium-sized businesses, the prevalence of corruption, the shadowing of business, raiding, and the imperfection of the tax system, underdevelopment of risk insurance in business, the need to increase the social responsibility of business, etc. The balance of economic entities and the dynamics of the number of operating economic entities (large, medium, small) in Ukraine in 2010–2021 are analyzed. It is concluded that during this period there was a general reduction in the number of enterprises, while the proportion of SMEs in the total number of economic entities remains at the level of 99.97%. It is noted that in the context of the full-scale russian invasion, SMEs continue to be a significant factor in the stability of the State. SMEs are payers of the taxes that Ukraine uses for various security and defense needs. A study of the dynamics of the number of employed employees in the field of small and medium-sized enterprises over the past ten years is carried out, allowing to note an increase in their number to 81.5% of the total number of employees in 2021. The problems and challenges of the development of entrepreneurial activity in modern Ukraine are identified, such as destruction, relocation, search and resumption of work in a new regional niche of the market segment, as well as the search for opportunities to pay taxes, pay wages, engage in volunteering, and receive income for their own development. The authors propose a number of directions of the State policy to maintain the stability of small and medium-sized businesses: creating a favorable environment for the development of small and medium-sized enterprises; expanding access to finance for small and medium-sized enterprises; simplification of tax administration for small and medium-sized enterprises; increasing the competitiveness and innovation potential of small and medium-sized enterprises.

Метод комплексного контроля передачи трафика в программно-конфигурируемых сетях

Modern requirements for traffic processing should possess the properties of mobility, speed, and simplification of network equipment administration. However, the existing classical Ethernet networks with Quality of Service failed to meet these requirements, which led to the emergence of software defined networks (SDN). In SDN, the main protocol is OpenFlow, which enables processing and managing heterogeneous traffic. This article aims to propose a methodology for eliminating the disadvantages inherent in the OpenFlow protocol. A method for complex control of traffic transmission in SDN was developed based on the algorithm of early diagnosis of real-time traffic losses during the transmission of heterogeneous traffic in the OpenFlow switch and the algorithm of real-time traffic transmission using a scheduler and a delivery control function in SDN. Moore digital automata of the proposed transmission algorithms were developed. A description of systems of canonical equations and output functions for digital automata was presented. Tables of transitions of digital automata were constructed, states of digital automata were encoded, and digital automata were synthesized based on D-triggers. The formalization of the developed method and algorithms of traffic transmission in SDN based on digital automata made it possible to design control automata and algorithms on FPGA of the Spartan 6 family and later familie.

The purpose of this study is to find out the implications of the revision of North Korea's Maritime and Port Law for the North Korean economy. North Korea's Maritime and Port Law was classified into three periods, and looked into two frameworks: characteristics of North Korean law and compliance with international norms.
 As a result, North Korea's shipping and port legislation presupposes international trade, so it was created within the framework of international convention from the beginning of its origin. Regardless of the time period, North Korea recognized the shipping and port industry as an important sector for economic growth, and reformed its laws to accept international norms in order to open its economy to the outside world. In addition, policy changes were made according to changes in the internal and external environment, and a causal relationship was shown in which changes in shipping and port laws were accompanied within the framework.
 The enactment and revision of the Shipping and Port Act in the 2010s, a period of expansion of economic opening to foreign countries, was more systematized and concrete compared to the previous period, and the law was reformed in the direction of mitigating discrimination against foreign ships and crew, objectivity, efficiency, and simplification of administration.
 However, in order to achieve a virtuous cycle structure in which law revision leads to industrial development, it is necessary to move towards market socialism through fundamental economic and institutional reforms.

DEVELOPMENT OF THE SYSTEM OF TAX ADMINISTRATORS IN UKRAINE

The aim of the article is to analyze the development of the tax administrators` system in Ukraine in the current conditions of Ukrainian financial system development. The administration of taxes and fees is a dynamic process. The quality of its implementation characterizes the activity of the tax administrators` system with the use of certain tools for the implementation of state tax policy for customs and tax control. In addition, it is a special type of tax activity, a constant and necessary component, the organization of which is due primarily to the needs of public fiscal policy. The analysis of organizational bases of activity of tax payments administrators` system is carried out. It is noted that the functions assigned to public authorities for the administration of tax payments in Ukraine are largely in line with international practice of tax administration. In the activities of the fiscal service, control aimed at ensuring compliance with the requirements of current tax legislation by all participants in tax relations, identifying the facts of its violations and ensuring their elimination is of particular importance. The work of tax authorities to conduct control significantly affects the conditions of doing business in Ukraine. In the first place it is necessary to put the legitimacy of the actions of controllers and the legitimacy of the goals they plan to achieve. Any law aimed at improving the work of tax authorities with taxpayers will be leveled in case of violation of the law by the tax administrator in the performance of control functions. Taxpayers, regardless of their field of activity, should be aware that honest payment of taxes guarantees the protection of their rights and protects them from illegal claims or actions of tax authorities. It is determined that to implement the strategic direction – effective administration of taxes, fees, and customs duties, it is necessary to define the main goals, in particular – simplification of tax and fee administration, expanding the tax base by limiting tax avoidance, improving, and centralizing tax and fee administration procedures. Organizational and Structural Approaches on Administrative Simplification: The Case of Kosovo

Administrative simplification is a particular approach that is considered a key aspect to ensure the quality of regulations and legislation. The appropriate organizational and structural approach to managing, coordinating, and monitoring administrative simplification is an important factor. The institutional framework for managing and coordinating administrative simplification initiatives has been the subject of discussions in the Government of Kosovo for several years. The objective of this paper is therefore to analyze the institutional set up in the central administration, as well as gaps and challenges, and their implications for the implementation of such reforms in Kosovo. It also presents the context of the reforms implemented by the Kosovo state administration to simplify administration and reduce administrative burdens. Particular attention is paid to the importance, status of implementation, and administration of the Law on General Administrative Procedures. Several recommendations and options have been made based on the findings of this study, the authors’ first-hand experience, and best comparative international practice.

The Simplification of Procedures in Portuguese Administrative Law

The present paper intends to provide an overview and a critical assessment of the administrative simplification policies implemented in Portugal over the past 20+ years. Throughout these decades, the major instruments for administrative simplification have been: (i) Decree-Law n. 135/99, of 22 April; (ii) the Simplex programme; (iii) the 2015 new Code of Administrative Procedure and its 2021 amendment; and (iv) the COVID-19 legislation. Although Decree-Law n. 135/99 (i) was the first attempt to specifically address simplification, it was a very thin one. The Simplex programme (ii), created in 2006, was the first cross-sectoral robust policy of simplification; its motto is “the simpler the better” and it encompassed reforms in all government areas aiming at cutting red tape, promoting administrative efficiency, and making the citizens’ and the corporations’ lives easier when dealing with State. The new Code of Administrative Procedure (iii), approved in 2015, brought about specific normative solutions to simplify procedures, namely, prior communications, administrative assistance, procedural conferences, and the electronic one-stop shop. Finally, the COVID-19 legislation (iv) was made necessary by the COVID-19 pandemic and by the fact that public health restrictions made it impossible for citizens to establish personal contact with the administration in many cases.

ЕЛЕКТРОННЕ АДМІНІСТРУВАННЯ ПОДАТКІВ ЯК ЗАСІБ ПІДВИЩЕННЯ ЕФЕКТИВНОСТІ ЇХ СПЛАТИ

The purpose of the article is to present the results of the study of organizational principles of e-administration as an element of the tax administration system. The budget deficit against the background of the crisis in the economy determines the need for active search for tools to increase budget revenues. One of them is tax administration. The article shows how a high level of information technology allows the tax service to solve the problem of justification of VAT refunds from the budget by switching to electronic administration of tax invoices and VAT returns. The main goals of the electronic reform of VAT collection in Ukraine are to restore justice in the tax process: elimination of fictitious suppliers, destruction of "tax pits", "scrolls", conversion centres, "optimizers", combating tax evasion by improving administration, eliminating corruption, elimination of administrative pres-sure from tax authorities, simplification of administration and payment of VAT, etc. It is proved that digital transformations in taxation first stimulate transparency of taxation procedures. It is determined that the efforts of digital transformation lead to tangible results – there is an increase in tax revenues. The natural result of increasing the level of digital maturity is the trans-formation of tax authorities and businesses into an adaptive digital platform. This will minimize the administrative burden: for conscientious taxpayers, tax obligations will be met as part of their business processes, and control and supervision activities will focus only on potential violators or those outside the platform. The system of VAT administration has been studied directly: automatic accounting of VAT paid by taxpayers, automatic free opening of accounts for taxpayers in the bank, compilation of all tax invoices is done electronically, as well as their registration in a single electronic register of tax invoices. The main problems of the organization of the tax administration system in Ukraine are pointed out and the ways to increase its efficiency are outlined.

Розвиток комп’ютеризованих інформаційних систем обліку й оподаткування в умовах глобалізації та цифровізації економічних відносин

The article is aimed at substantiating the influence of computerized information systems for accounting and taxation on the formation of high-quality information in the context of globalization and digitalization of economic relations. It is proved that the formation of the information society involves the subordination of economic growth to the qualitative parameters of social and economic development, since further progress will be determined not so much by the production of goods as by the expanded use of information. It is determined that information is a factor of globalization in the information society, so national strategies for economic development of countries of the world should be built on the specifics and main trends of globalization processes, on the grounds of the conception of sustainable development. It is substantiated that the digital economy consists of three main parts, namely: infrastructure development and support, e-business, and e-commerce. It is noted that for the effective functioning and development of each component of the digital economy, a precise accounting and taxation system is needed, which will provide high-quality, reliable and complete information for making managerial decisions. A list of the main directions of development of digital socioeconomic relations in the conditions of global cooperation is proposed. It is determined that the computerized information system of enterprise management is a set of economic-mathematical methods and models, technical, software, technological instruments and solutions, as well as the availability of specialists for information processing and managerial decision-making. The most popular in Ukraine are the integrated corporate information systems «Parus», «Galaxy», «BAS», SAP R/3 (SAP AG), etc., and the correct choice of the type of information system by an enterprise requires a classification of such systems according to the criterion of complexity of the functions and tasks performed at enterprise. It is proved that the use of computerized information systems in accounting and taxation leads to simplification of administration, ensures the preparation of reliable, understandable and comparable financial and management reporting, aligning internal relations between the departments of accounting, economic, control services and increasing the analytical value of information about the financial and economic activities of the enterprise as a whole and its individual important links in particular. Concerning the system of accounting and taxation, it is proposed to allocate several sections of information about the economic activity of the enterprise, which include accounting, analytical, forecast and management information. Each of these information blocks is sensitive to the influence of a certain group of factors, needs to have a number of problems solved, and has reserves to improve the organizational and quality level. The general algorithm of the automated process of accounting and taxation of any business operations through modeling is constructed. The considered problems of computerization of information systems of accounting and taxation and the proposed recommendations for their solution create a basis for continuing research in the direction of improving the methodology and organization of accounting and taxation of economic activity of enterprises in the context of globalization and digitalization of the economy.

The Only Game in Town? New Steering Models as Spaces of Contestation in 1990s Public Administration

Abstract Reform concepts such as the »New Steering Model« (NSM) became prominent in the German administrative discourse of the 1990s. This article examines the examples of the Speyer Quality Contest and the Carl Bertelsmann Price. These intermunicipal performance contests awarded municipalities for the appropriation of NSM principles, thus popularizing the approach in administrative theory and practice. Simulated competition served as a tool for the relevant stakeholders to negotiate notions of quality and innovation. Actors such as the Joint Office for Administrative Simplification, the University for Administrative Sciences Speyer, and the Bertelsmann Foundation pushed frameworks that conceptualized the municipality as a non-commercial professional service firm. Cities, in turn, hoped to find ways to deliver public services with waning funds. While neither the NSM nor practices of simulated competition dominated the public administrative discourse beyond the early 2000s, they speak to a temporary belief in managerial practices in the public sphere.
