Fibrinolytics: from the thrombolysis to the processes of blood vessels growth and remodeling, neurogenesis, carcinogenesis and fibrosis

One of the most outstanding scientific achievements in the thrombolysis is the development and administration of fibrinolysin - the first Soviet drug that lyses blood clots. Intracoronary administration of fibrinolysin reduced the mortality of patients with myocardial infarction by almost 20%. For his work in this field Yevgeny Chazov was awarded the Lenin Prize in 1982. Over the next decades, under his leadership, the Cardiology Center established scientific and clinical laboratories that created new generations of drugs based on fibrinolytics for treating patients with myocardial infarction, restoration of blood flow in ischemic tissue, and also studying the mechanisms of remodeling of blood vessels involving the fibrinolysis system. It have been found new mechanisms of regulation of the navigation of blood vessels and nerves growth, tumor growth and its metastasis with the participation of the fibrinolysis system proteins. The review reports the role of the fibrinolysis system in the thrombolysis, blood vessels growth and remodeling, neurogenesis, carcinogenesis and fibrosis. The article is dedicated to the 90th anniversary of academician E.I. Chazov.

Experimental data concerning the effect of fibrinolysin on the coronary blood flow

It was found in experiments on dogs that fibrinolysin, fibrinolysin with heparin, trypsin with heparin, and heparin alone caused a considerable increase in the volume speed of the coronary blood flow. It was revealed that the preparations under study not only restored the level of the blood flow decreased due to pituitrin but also raised it approximately to the same indices as in the experiments without a pituitrin-induced spasm. The largest increase in the blood flow was noted upon a combined administration of fibrinolysin and heparin. At the same time, one could also note a distinct hypotensive effect of the investigated preparations. Fibrinolysin eliminated the spasm of the coronary vessels caused by pituitrin and improved the ECG. The vasodilating action of fibrinolysin discovered in the experiment in addition to its principal thrombolytic activity makes it especially valuable for patients with thrombosis of the peripheral vessels, coronary thrombosis, and for those threatened by these diseases.

HUMAN FIBRINOLYSIN WITH AND WITHOUT UROKINASE: ITS EFFECT ON EXPERIMENTAL ARTERIAL THROMBI IN DOGS

The lytic effects of heat-treated, sterile solutions of spontaneously activated and urokinase-activated fibrinolysin on occlusive experimental arterial clots have been studied. For this purpose methods for the preparation of urokinase and activation of profibrinolysin, the production of traumatic arterial thrombi in dogs, and the recording of clot lysis are described. Single injection as a means of administration was compared with the traditional method of infusion. When used in conjunction with a prompt assay of the in vivo fibrinolytic activity, which was developed for this purpose, the injection method was preferred. The results indicated that spontaneous fibrinolysin, given by single injection, was as effective in lysing clots as were preparations of fibrinolysin that contained activator. The approximate therapeutic dose for both types of fibrinolysin under the conditions described was found to be 225–325 Michigan Department of Health caseinolytic units per kg body weight. Lysis of occluding arterial thrombi was achieved within a few hours after administration of fibrinolysin by injection, and arteries remained patent during the week of posttreatment observation. No toxic effects due to the fibrinolysin or the glycerol present as stabilizer could be demonstrated after the therapeutic dose. The only undesirable side effect of overdose of fibrinolysin was a transient incoagulability of the blood.
