3:45 PM Abstract No. 180 Single-session treatment of lower extremity venous thrombosis using a novel thrombectomy device: results of a pilot study

To study the efficacy of the Jeti thrombectomy device in patients with acute DVT. 30 patients (10 male, 20 female) with mean age of 57 years (± 21 years) with acute lower extremity venous thrombosis were reviewed. The study was approved by the Institutional Review Board (IRB). The primary outcome measure was technical success, defined as reestablishment of inline flow. Secondary outcome measures were percent thrombus removal, total thrombolytic dose, total number of DVT-related procedures, total procedural time, overnight thrombolysis post procedure, ICU length of stay, total hospital length of stay, procedure-related complications. Additional data points included mean symptom duration and venous segments treated. A total of 40 procedures were performed in 30 patients. Technical success was 93.3% (28 of 30 patients). Mean percent thrombus removal was 74.0% (± 28.2%). Mean tPA dose was 7.8 mg (± 8.9 mg). Average number of DVT procedures was 1.3 (± 0.5). Mean procedural time was 88.7 mins (± 44.6 mins). 23 of 30 patients did not require overnight thrombolysis after Jeti thrombectomy. 12 of 30 patients did not require thrombolytics at all. Mean hospital length of stay was 4.9 days (± 5.2 days). Mean ICU length of stay was 1.6 days (± 4.6 days). In the patients who did not require overnight thrombolysis, mean percent thrombus removal was 81.2% (± 24.2%), mean tPA dose was 3.1 mg (± 3.8 mg) and mean ICU length of stay was 1.1 days (± 4.9 days). No major complications were observed. One minor complication included focal contrast extravasation, which was self-limiting. Mean symptom duration was 7.7 days (± 9.2 days). Venous segments included iliocaval (25), iliofemoral (20), femoropopliteal (15). The Jeti aspiration thrombectomy system offers a safe and feasible adjunct to currently available options for peripheral thrombectomy, allowing for significant potential reduction of thrombolytics and ICU length of stay. Further studies to evaluate the safety, efficacy, and cost-effectiveness of the system in the peripheral vasculature are warranted.

Incidence of acute lower extremity venous thrombosis after percutaneous endovascular aneurysm repair

Improvements in endovascular aortic aneurysm repair (EVAR) have led many physicians to embrace a purely percutaneous approach using the suture-mediated "preclose" technique. Whereas there are a number of theoretical benefits, the rate of periprocedural iatrogenic acute deep venous thrombosis (DVT) is unknown. We sought to determine the incidence of acute DVT after percutaneous EVAR (PEVAR). This was a single-center, retrospective review of 52 consecutive patients undergoing elective PEVAR. Demographics, procedural data, and postprocedure lower extremity venous duplex ultrasound data were analyzed by univariate statistical analysis. Among the 52 patients, the average age was 73 years, and the majority were men (n = 44). Only 6% (n = 3) had a prior history of DVT. The majority of procedures were performed under general anesthesia (n = 51 [98%]) with systemic intravenous heparin. Protamine was used in only 13 cases (25%). Sheath sizes ranged from 9F to 20F, with an average of 16F, and closure was achieved using the preclose technique. Postprocedure chemoprophylaxis was administered to 85% of patients (n = 44) during their hospitalization. Median length of stay was 1 day. Acute DVT was identified in 8% of patients (n = 4) on postoperative day 1. Among the 4 DVTs, 25% were femoropopliteal (n = 1) and 75% were calf vein DVTs (n = 3). On follow-up imaging 2 weeks later, 75% of DVTs were resolved. Among the four patients with acute DVT on postoperative day 1, three had associated risk factors: history of DVT (n = 2), active smokers (n = 1), and obesity (body mass index >30; n = 3). The remaining patient had no risk factors but developed an ipsilateral calf vein DVT. The risk of acute DVT after PEVAR is low. Lower extremity venous duplex ultrasound screening is not necessary unless there exist preclinical risk factors or postprocedural clinical indications suggestive of DVT.

Decreased incidence of left common iliac vein compression in patients with abdominal aortic aneurysms

Compression of the left common iliac vein (LCIV) by the right common iliac artery is an anatomic variant that may increase the risk for LCIV thrombosis. The incidence of LCIV compression in patients with abdominal aortic aneurysms (AAA) is unknown, however. The aim of this descriptive anatomic study was to determine (1) the incidence of LCIV compression in patients with and without AAA and (2) if endovascular AAA repair acutely alters the incidence of LCIV compression and, hence, the risk of LCIV thrombosis. A retrospective analysis of medical records and helical computed tomography (CT) scans was conducted in 100 AAA patients and 100 non-AAA patients (n = 200). Medical records were reviewed for symptoms and risk factors for deep vein thrombosis, and data were reported according to the Joint Society Reporting Standards for acute lower-extremity venous thrombosis. The minor diameters of the aorta, inferior vena cava, and common iliac arteries and veins were measured. For AAA patients, measurements were obtained from preoperative and 30-day postoperative CT scans. The mean age of the study cohort was 38 years (range, 17 to 85 years) for non-AAA subjects and 73 years (range, 51 to 89 years) for AAA subjects. The mean acute lower-extremity venous thrombosis risk factor score was low for both patient groups (non-AAA, 0.82 +/- 0.12; AAA, 2.63 +/- 0.14). Mean compression of the LCIV was 37.8% (range, 0% to 74.8%) for non-AAA patients but only 27.3% (range, 0% to 74.2%; P < .0006) for AAA patients. Of non-AAA patients with venous compression, the right common iliac artery was the compressing structure in 87% of cases. However, of AAA patients with venous compression, the left common iliac artery was the compressing structure in 76% of cases. There was no statistically significant change in the degree of compression of the LCIV before and after endovascular repair (27.3% vs 25.1%, respectively) nor was there a change in the structure that compressed the LCIV. Patients with AAA were found to have more tortuous iliac arteries that led to less anatomic compression of the LCIV compared with nonaneurysmal patients. Furthermore, the left common iliac artery was found to compress the LCIV in most of the AAA patients, and the right iliac artery was found to compress the LCIV in most non-AAA patients. Endovascular AAA repair did not acutely alter these anatomic findings. Patients with AAA may therefore be at lower risk of developing LCIV thrombosis owing to the nature of their anatomy.

Iliac vein compression in an asymptomatic patient population

Objective May-Thurner syndrome is a well-recognized anatomic variant that is associated with the development of symptomatic acute venous thrombosis of the left iliac vein. However, the natural frequency of compression of the left iliac vein and its clinical significance in asymptomatic disease has not been established. Therefore the purpose of this descriptive anatomic study was to determine the incidence of left common iliac vein compression in an asymptomatic population. Methods A retrospective analysis of medical records and helical abdominal computed tomography scans was conducted in 50 consecutive patients evaluated in the emergency department because of abdominal pain. Medical records were reviewed for symptoms and risk factors for deep venous thrombosis, and data were collected and reported according to the Joint Society Reporting Standards for acute lower extremity venous thrombosis. All computed tomography was performed with intravenous contrast medium, and 2-mm to 5-mm axial images were obtained. The minor diameter of the common iliac arteries and veins was measured. The technique of transverse image measurement was validated with multiplanar reconstructions and orthogonal diameter measurements in a subset of subjects. Statistical analysis was performed with the Student t test or Spearman rank correlation. Results Mean age of subjects without symptoms was 40 years (range, 19-85 years), and 60% (n = 30) were female patients. The mean acute lower extremity venous thrombosis risk factor score was 1.16 ± 0.23 (range, 0-6; maximum possible score, 28). It was surprising that 24% (n = 12) of patients had greater than 50% compression and 66% (n = 33) had greater than 25% compression. Mean compression of the left common iliac vein was 35.5% (range, −5.6%-74.8%). The structure most often compressing the left common iliac vein against the vertebral body was the right common iliac artery (84%). There was no strong correlation between patient age or common iliac artery size and compression of the left common iliac vein. However, women had greater mean compression of the left common iliac vein (women, 41.2% ± 3.1%; men, 27.0% ± 3.0%; P = .003). Conclusion Hemodynamically significant left common iliac vein compression is a frequent anatomic variant in asymptomatic individuals. Therefore compression of the left iliac vein may represent a normal anatomic pattern that has thus far been thought of as a pathologic condition.

Do Patients with Acute Deep Vein Thrombosis Have Fever?

The purpose of this study was to determine whether those with lower extremity acute venous thrombosis have fever. During a recent 14.5-month period, 1847 patients undergoing lower extremity venous duplex scanning also had their oral temperature measured using a digital thermometer at the time of duplex examination. Patients were 57.8 +/- 17.3 years of age (range, 14 to 99). Temperature was 98.5 +/- 1.1 degrees F. Twenty-three patients had acute inferior vena cava thrombosis, 60 had acute iliac vein thrombosis, 138 had acute femoral venous thrombosis, and 131 had acute popliteal venous thrombosis. Calf vein thromboses were present in 102 patients, and 43 patients had superficial venous thrombosis. A total of 228 patients had acute lower extremity venous thrombosis in one or more of these venous segments. Temperature with acute lower extremity venous thrombosis was 98.7 +/- 1.05 degrees F versus 98.5 +/- 1.10 degrees F in those with no acute thrombosis. Although small, this temperature difference was statistically significant (P < 0.02). Acute deep venous thrombosis (DVT) was defined as acute popliteal or more proximal femoral, iliac, or vena cava thrombosis. The temperature for the 175 patients with acute DVT was 98.7 +/- 1.10 degrees F versus 98.5 +/- 1.10 degrees F for those without DVT (P < or = 0.035). There was no temperature that served to accurately differentiate those who did from those who did not have DVT. The frequency that patients with DVT had fever, defined as a temperature > or = 100 degrees F, was 9.1 per cent (16 of 175) with DVT versus 7.5 per cent (126 of 1678) without DVT (not significant). In the subgroup with a temperature > or = 101 degrees F, 4.6 per cent (8 of 175) with DVT had such a fever versus 3.4 per cent (57 of 1672) without DVT (not significant). Those undergoing venous duplex who were found to have acute lower extremity venous thrombosis, including acute DVT, had statistically higher temperatures, but such temperature differences were minimal. The incidence of fever, defined as a temperature > or = 100 degrees F or > or = 101 degrees F, was not different between those with and those without acute DVT. It appears that the presence of fever may not be a sensitive or specific indicator for the presence of underlying acute DVT.

Reporting standards in venous disease: An update

At the request of the Ad Hoc Committee on Reporting Standards of the Joint Council of the Society for Vascular Surgery and the North American Chapter of the International Society for Cardiovascular Surgery, this report updates and modifies "Reporting standards in venous disease" (J Vasc Surg 1988;8:172-81). As in the initial document, reporting standards for publications dealing with (1) acute lower extremity venous thrombosis, (2) chronic lower extremity venous insufficiency, (3) upper extremity venous thrombosis, and (4) pulmonary embolism are presented. Numeric grading schemes for disease severity, risk factors, and outcome criteria present in the original document have been updated to reflect increased knowledge of venous disease and advances in diagnostic techniques. Certain recommendations of necessity remain arbitrary. These standards are offered as guidelines whose observance will in our opinion improve the clarity and precision of communications in the field of venous disorders.

This report comprises suggested reporting standards for publications dealing with (1) acute lower extremity venous thrombosis, (2) chronic lower extremity venous insufficiency, (3) upper extremity venous thrombosis, and (4) pulmonary embolism. Attempts have been made to set forth numeric grading schemes for disease severity, risk factors, and outcome criteria. Some of the recommendations had to be arbitrary of necessity, but they were judged the most generally acceptable by members of the committee. They are offered not as binding rules but as guidelines whose observance will serve the clarity and precision of communication.
