The evolution, facilitators, barriers, and additional activities of acute flaccid paralysis surveillance platform in polio eradication programme Bangladesh: a mixed-method study

ABSTRACT Background The Global Polio Eradication Initiative (GPEI) helped develop the standard acute flaccid paralysis surveillance (AFP) system worldwide, including, knowledge, expertise, technical assistance, and trained personnel. AFP surveillance can complement any disease surveillance system. Objective This study outlines AFP surveillance evolution in Bangladesh, its success and challenging factors, and its potential to facilitate other health goals. Methods This mixed-method study includes a grey literature review, survey, and key informant interviews (KIIs). We collected grey literature from online websites and paper documentation from GPEI stakeholders. Online and in-person surveys were conducted in six divisions of Bangladesh, including Dhaka, Rajshahi, Rangpur, Chittagong, Sylhet, and Khulna, to map tacit knowledge ideas, approaches, and experiences. We also conducted KIIs, and Data were then combined on focused emerging themes, including the history, challenges, and successes of AFP surveillance programme. Results According to the grey literature review, survey, and KII, AFP surveillance successfully contributed to decreasing polio in Bangladesh. The major facilitating factors were multi-sectoral collaboration, Surveillance Immunization Medical Officer (SIMO) network activities, social environment, community-based surveillance, and promising political commitment. On the other hand, high population growth, hard-to-reach areas, people residing in risky zones, and polio transition planning were significant challenges. Bangladesh is also utilizing these polio surveillance assets for other vaccine-preventable diseases. Conclusion As the world is so close to eradicating polio, the knowledge, and other assets of the AFP surveillance, could be used for other health programmes. In addition, its strengths can be leveraged for combating new and emerging diseases.

Thirty Years of Experience of Acute Flaccid Paralysis Surveillance for Polio - China, 1993-2022.

Detecting poliovirus infections proves to be highly challenging due to their asymptomatic nature and infectious potential, highlighting the crucial importance of effective detection methods in the context of polio eradication efforts. In many countries, including China, the primary approach for identifying polio outbreaks has been through acute flaccid paralysis (AFP) surveillance. In this study, we conducted an evaluation spanning three decades (1993-2022) to assess the effectiveness of AFP surveillance in China. Data on all AFP cases identified since 1993 and national-level AFP surveillance system quality indicators aligned with the World Health Organization (WHO) standards were collected for analysis. The quality indicators assess surveillance sensitivity, completeness, timeliness of detection notification, case investigation, and laboratory workup. Surveillance sensitivity is determined by the non-polio AFP (NPAFP) detection rate among children under 15 years of age. Between 1993 and 2022, a total of 150,779 AFP cases were identified and reported. Within this pool, surveillance identified 95 cases of wild poliovirus (WPV) and 24 cases due to vaccine-derived poliovirus. From 1995 onwards, the detection rate of NPAFP cases consistently adhered to the WHO and national standards of ≥1 case per 100,000, falling between 1.38 and 2.76. Starting in 1997, all timeliness indicators consistently achieved the criteria of 80%, apart from the consistency in meeting standards set for the rate of positive specimens sent to the national laboratory. AFP surveillance has been instrumental in China's accomplishment of maintaining a polio-free status. The ongoing adherence to key performance indicators, ensuring sensitivity and prompt specimen collection, demonstrates that AFP surveillance is proficient in detecting poliovirus in China. As we move into the post-eradication phase, AFP surveillance remains crucial for the sustained absence of polioviruses in the long term.

Evaluation of the Acute flaccid paralysis surveillance indicators in Zambia from 2015–2021: a retrospective analysis

BackgroundThe resurgence of poliovirus infection in previously polio free regions and countries calls for renewed commitment to the global polio eradication efforts including strengthening of Acute Flaccid Paralysis (AFP) surveillance systems. Zambia is one of the countries substantially at risk for the importation of poliovirus infection from neighbouring countries including Malawi, Mozambique, and Democratic Republic of the Congo (DRC). This study describes a seven-year AFP surveillance, assesses the surveillance indicators, and highlights areas for improvement.MethodsWe conducted retrospective analysis of the routinely collected AFP surveillance data from January 2015 to December 2022. The AFP surveillance indicators performance was assessed using the World Health Organisation’s recommended minimum AFP surveillance indicators performance.ResultsCumulatively, a total of 1715 AFP cases were reported over the study period. More than half, 891 (52%) of reported cases were aged < 5 years with 917 (53.5%) of males. More than half, 1186 (69.2%) had fever at onset, 718 (41.9%) had asymmetric paralysis and 1164 (67.9%) had their paralysis progressed within 3 days of onset. The non-polio AFP rate ranges from 3.4 to 6.4 per 100,000 children < 15 years old and stool adequacy ranging from 70.9% to 90.2% indicating sensitive surveillance with late detection of cases. The percentage of cases with early stool collection, timely transportation was above the World Health Organisation (WHO) minimum of 80% but with declining proportion of stools arriving in the laboratory in optimal condition. Completeness of 60-days follow-up evaluation was suboptimal ranging from 0.9% to 28.2%.ConclusionThe AFP surveillance system in Zambia is doing well. However, additional efforts are needed to improve early detection of cases; stool sample collection, transportation and monitoring to ensure arrival in good condition in the laboratory; and improve 60-days follow-up evaluation for evidenced-based classification of inadequate AFP cases and proper care.

Acute flaccid paralysis surveillance system data analysis, Pujehun, Sierra Leone, 2011-2020

The Acute Flaccid Paralysis (AFP) surveillance system was established to detect poliovirus and to measure progress toward Poliovirus eradication. Vaccine-derived poliovirus outbreaks have been reported worldwide in 2020, including in Sierra Leone. We analyzed AFP surveillance data to describe the distribution, epidemiological trend, and clinical characteristics of AFP cases. We analyzed secondary data of AFP cases in the Pujehun District for the period from 2011 to 2020. We collected data on key variables such as age, sex, clinical characteristics, outcome, non-AFP rate, stool adequacy, and non-Polio enteroviruses. We analyzed data using Microsoft Excel 2013 and Epi-Info 7 software. Results were reported in tables, graphs, and maps. A total of 60 AFP cases were reported with a median age of 3 years (range 0.7- 14), with no confirmed poliomyelitis. Under-five Children accounted for 46 (76.7%), with 27 females and 33 males. Of the sixty cases,17(28.3%) were hospitalized and 16(27%) were detected in the hospital. Ten (16.7%) cases were classified as non-Polio enteroviruses and 50 (83.3%) as negative. Sudden onset of paralysis was reported in 57 (96.6%) cases, paralysis progressed within 3 days in 44 (74.6%) cases, and 56 (94.6%) reported fever. Out of 33, the left legs 28 (84.9%) were the most affected sites. Majority of the cases, 35(58.1%) had received five and more doses of the Oral Polio vaccine. The stool adequacy was 54(90%) and the non-AFP rate was above 2.0 except in 2017 when it was 1.8. Of the 14 chiefdoms,12 (86%) met the minimum AFP reporting target (1/100,000 &lt; 15 years’ population). None of the AFP cases were confirmed as Poliovirus in Pujehun District. Case detection and reporting rate were high except for two chiefdoms. We recommended maintaining the AFP surveillance system in the district and replicate the strategies used to other districts.

A smartphone‐based surveillance system for acute flaccid paralysis: A technology frames perspective

AbstractThere is a consensus in the information systems (IS) discipline that conflicting views among those implementing IS could result in critical implementation challenges. Prior studies on implementing smartphone‐based disease surveillance systems highlight the emergence of diverse perceptions of the system among stakeholder groups. It is argued that these various perceptions play a role in achieving the project's objectives, particularly when there is a strong incongruence. This study examines key stakeholders' interpretations of a smartphone‐based acute flaccid paralysis (AFP) surveillance system and how these interpretations have influenced the resulting outcomes. We adopt a qualitative interpretive case study method with theoretical underpinnings of technological frames (TF). The findings were drawn from observations, and semi‐structured interviews with project managers (PMs), community health workers (CHWs), informants (CHIs), and disease surveillance and notification officers (DSNOs) in Kano and Adamawa states northern Nigeria. Our findings reveal that training, implementation context, and technical infrastructure use socially shape the smartphone‐based AFP surveillance project. The results also illustrated how TF evolve. We present a model that explains the relationship between these factors and recommendations to manage conflicting views among stakeholder groups.

Acute flaccid paralysis surveillance and management in rural and urban reporting sites in Northern Nigeria

Abstract Background: The study evaluated the significance of acute flaccid paralysis (AFP) surveillance within the polio eradication strategy and its integral role in the overall eradication efforts. Specifically, the research assessed the implementation of AFP surveillance and its management at reporting sites in Northern Nigeria. Materials and Methods: This study utilized quantitative research methods, including administering interviewer-administered questionnaires to health facility staff and caregivers of children within the community. The research was conducted between May and July 2019 and involved 592 participating health facilities enrolled in the AFP surveillance program for polio eradication. These facilities were spread across 11 states in Nigeria’s Northeast and Northwest regions. Data were analyzed using IBM Statistical Package for Social Sciences (SPSS) Statistics for Windows, Version 25.0 (Armonk, New York). Results: A total of 171 AFP cases were reported 6 months prior, with the highest proportion recorded in Kano (18.7%), Bauchi, and Kaduna (13.5% each) states. Most cases were seen in rural areas (73.1%), with an average of 1–3 cases (80.8%). Of the 171 AFP cases reported in the past 6 months, &gt;90% were investigated, &gt;80% had a complete clinical investigation, and &gt;70 were followed up for residual paralysis examination. Most rural health facilities (&gt;80%) had 1–3 trained staff compared with 70.9% of facilities in urban areas. On the other hand, the proportion of facilities in urban areas with 4–6 trained staff was almost double rural area facilities (18.4% vs. 9.8%). It was a surprise that a higher proportion of pastoral facility staff was able to define AFP correctly (94%) than urban facilities with 85.1% (P &lt; 0.05). Also, AFP surveillance and management were better in rural facilities than in urban. Conclusion: According to the research findings, the AFP surveillance system in the northern region demonstrated strong performance. However, urban and rural healthcare providers require regular training in AFP surveillance to maintain practical surveillance standards.

Evaluation of acute flaccid paralysis surveillance system in the River Nile State - Northern Sudan, 2021

BackgroundOne of the four main elements of the worldwide polio eradication strategy is acute flaccid paralysis surveillance (AFP). This system is based on (acute flaccid paralysis (AFP) cases reported and tested at World Health Organization (WHO) accredited laboratories. To measure and monitor performance, indicators were created. The current study aims to evaluate the system components, performance, and efficiency in River Nile State, Northern Sudan, and their compliance with World Health Organization (WHO) requirements for it to be adopted as a good system; its results can be used to certify whether a country is polio-free or not.Material and methodsA facility-based retrospective descriptive study was conducted in the River Nile State, Northern Sudan, from Jan 2017 to Dec 2020. This study included all reporting sites/units, workers who reported acute flaccid paralysis (AFP) cases, and officers at the locality level. A total of 50 health institutions were visited for surveillance, and interviews with 59 health workers who were part of the AFP surveillance system were undertaken. The data were collected from participants using a pre-tested questionnaire designed and constructed by the World Health Organization (WHO) framework, and the data were analyzed using the SPSS version (22).ResultsThe River Nile State’s AFP surveillance system was of high quality in terms of the infrastructure that had been put in place and the effectiveness of the system’s operations, as evidenced by the following statistics: from 2017 to 2020, the reported non-polio acute flaccid paralysis (AFP) cases were at a mean rate of 4.02 per 100,000 children under the age of 15; the majority of AFP reported cases were under 10 years; and males made up 73.3% of reported cases; The completeness of reports and surveillance documents exceeded 80%, and active surveillance was applied in 80% of reporting sites.ConclusionDespite the fact that the surveillance system is capable of detecting cases, Sudan continues to report cases of imported polio from other countries, highlighting the need to strengthen surveillance systems and eradication efforts in these countries.

Experience and findings from surveillance peer review in Nigeria, August 2017-May 2019.

acute flaccid paralysis (AFP) surveillance is the gold standard of the Global Polio Eradication Initiative (GPEI) for detecting cases of poliomyelitis and tracking poliovirus transmission. Nigeria's AFP surveillance performance indicators are among the highest in countries of the World Health Organization (WHO) African Region. The primary AFP surveillance performance indicators are the rate of non-polio AFP among children and the proportion of timely, adequate specimen collection. The surveillance working group of the National Emergency Operations Centre assessed the quality of AFP surveillance data in some reportedly high-performing states. we conducted a retrospective review of AFP surveillance performance indicators in Nigeria for 2010-2019. We also reviewed data in reports from four groups of surveillance peer reviews and validation visits (conducted by in-country GPEI partners) during August 2017-May 2019 in 16 states with high primary AFP surveillance indicators; the validation visits reviewed clinical information and the dates of specimen collection and onset of paralysis with caretakers. there were consistently increasing AFP surveillance primary performance indicators during 2010-2016, followed by declines during 2017-2019. From the data for 16 states with peer reviews conducted from August 2017-May 2019, overall concordance of reported and "true" (validated) AFP indicator data in peer review investigations was highly variable. True AFP concordance ranged from 58%-100%, and stool timeliness concordance ranged from 56%-95%. The most common clinical causes of reported AFP cases that were not true AFP were spastic paralysis, malaria, sickle cell disease, and malnutrition. All the states that participated in peer reviews developed surveillance improvement plans based on the gaps identified. Nigeria has highly sensitive AFP surveillance according to reported primary AFP performance indicators. The findings of peer reviews indicate that the AFP surveillance system needs to be strengthened and well-supervised to enhance data quality.

Trends of key surveillance performance indicators of acute flaccid paralysis: a descriptive analysis, Uganda, 2015–2020

BackgroundPolio is disease caused by poliovirus which can in turn cause irreversible paralytic disease, presenting as Acute Flaccid Paralysis (AFP). A sensitive AFP surveillance system, in which all reported AFP cases are evaluated, first to determine if they are true AFP cases or not, is key for tracking polio eradication. True AFP cases are then later categorized as polio AFP or non-polio AFP (NPAFP) cases. Sensitivity is defined by meeting an annual NPAFP rate/100,000 population < 15 years of ≥ 4/100,000, and an annual stool adequacy (SA) rate of ≥ 80%. We describe Uganda’s AFP surveillance performance between 2015–2020, based on the WHO-recommended indicators, including; NPAFP and stool adequacy rate.MethodsWe performed a descriptive analysis of national AFP surveillance data, 2015–2020 obtained from ministry of health. We evaluated proportion of reported AFP cases that were true AFP, and changes in NPAFP and stool adequacy (SA) rate over the study period. We evaluated the trends in achieving the targeted NPAFP and SA rates from 2015–2020. We used QGIS to illustrate patterns in NPAFP and SA rates across districts and subregions.ResultsAmong 3,605 AFP cases reported and investigated countrywide from 2015–2020, 3,475 (96%) were true AFP cases. All the true AFP cases were non-polio related. District reporting was near-complete (97–100% each year). Overall, the mean NPAFP rate declined from 3.1/100,000 in 2015 to 2.1/100,000 in 2020. Less than 40% of districts met the NPAFP target rate in all years. The proportion of districts achieving the NPAFP target rate of ≥ 4/100,000 significantly declined from 35% in 2015 to 20% in 2020. The mean annual SA rate nationally was 88% from 2015–2020. Only 66% of districts achieved the SA target rate of ≥ 80% in the study period. The proportion of districts with SA rate ≥ 80% significantly increased from 68 to 80% between 2015 and 2020.ConclusionMost districts reported AFP cases. However, there was a decline in the NPAFP rate from 2015–2020 and few districts achieved the target rate. The suboptimal AFP surveillance system performance leaves the country at risk of missing ongoing poliovirus transmission. We recommend health worker training on active AFP searches, intensified supportive supervision, increase the number of environmental surveillance sentinel sites to boost AFP surveillance in the country, and periodic review meetings with districts to assess AFP surveillance performance.

Contribution of Auto-Visual AFP Detection and Reporting (AVADAR) on polio surveillance in South Sudan.

Introductionthe last wild polio virus in South Sudan was documented in 2009. Nonetheless, it was one of the last four countries in the WHO African region to be accepted as a polio-free country in June 2020. In line with this, to accelerate the polio-free documentation process, the country has piloted Auto Visual AFP Detection and Reporting (AVADAR) in three counties. This study examined the contribution of the AVADAR surveillance system to the traditional Acute Flaccid Paralysis (AFP) surveillance system to document lessons learnt and best practices.Methodswe performed a retrospective descriptive quantitative study design to analyze secondary AVADAR surveillance data collected from June 2018 to December 2019 and stored at the WHO AVADAR server.Resultsthe AVADAR community surveillance system has improved the two main AFP surveillance indicators in the piloted counties and made up 86% of the total number of true AFP cases detected in these counties. The completeness and timeliness of weekly zero reporting were 97% and 94%, respectively and maintained above the standard throughout the study, while the two main surveillance indicators in the project area were improved progressively except for the Gogrial West County. In contrast, main surveillance indicators declined in some of the none-AVADAR implementing counties.Conclusionthe AVADAR surveillance system can overcome the logistical and remoteness barriers that can hinder the early detection and reporting of cases due to insecurity, topographical, and communication barrier in rural and hard-to-reach areas to accomplish and sustain the two main surveillance indicators, along with the completeness and timeliness of weekly zero reporting. We recommend extending this application-based surveillance system to other areas with limited resources and similar challenges by incorporating other diseases of public health concern.

Genetic Characterization of Novel Oral Polio Vaccine Type 2 Viruses During Initial Use Phase Under Emergency Use Listing - Worldwide, March-October 2021.

The emergence and international spread of neurovirulent circulating vaccine-derived polioviruses (cVDPVs) across multiple countries in Africa and Asia in recent years pose a major challenge to the goal of eradicating all forms of polioviruses. Approximately 90% of all cVDPV outbreaks are caused by the type 2 strain of the Sabin vaccine, an oral live, attenuated vaccine; cVDPV outbreaks typically occur in areas of persistently low immunization coverage (1). A novel type 2 oral poliovirus vaccine (nOPV2), produced by genetic modification of the type 2 Sabin vaccine virus genome (2), was developed and evaluated through phase I and phase II clinical trials during 2017-2019. nOPV2 was demonstrated to be safe and well-tolerated, have noninferior immunogenicity, and have superior genetic stability compared with Sabin monovalent type 2 (as measured by preservation of the primary attenuation site [domain V in the 5' noncoding region] and significantly lower neurovirulence of fecally shed vaccine virus in transgenic mice) (3-5). These findings indicate that nOPV2 could be an important tool in reducing the risk for generating vaccine-derived polioviruses (VDPVs) and the risk for vaccine-associated paralytic poliomyelitis cases. Based on the favorable preclinical and clinical data, and the public health emergency of international concern generated by ongoing endemic wild poliovirus transmission and cVDPV type 2 outbreaks, the World Health Organization authorized nOPV2 for use under the Emergency Use Listing (EUL) pathway in November 2020, allowing for its first use for outbreak response in March 2021 (6). As required by the EUL process, among other EUL obligations, an extensive plan was developed and deployed for obtaining and monitoring nOPV2 isolates detected during acute flaccid paralysis (AFP) surveillance, environmental surveillance, adverse events after immunization surveillance, and targeted surveillance for adverse events of special interest (i.e., prespecified events that have the potential to be causally associated with the vaccine product), during outbreak response, as well as through planned field studies. Under this monitoring framework, data generated from whole-genome sequencing of nOPV2 isolates, alongside other virologic data for isolates from AFP and environmental surveillance systems, are reviewed by the genetic characterization subgroup of an nOPV working group of the Global Polio Eradication Initiative. Global nOPV2 genomic surveillance during March-October 2021 confirmed genetic stability of the primary attenuating site. Sequence data generated through this unprecedented global effort confirm the genetic stability of nOPV2 relative to Sabin 2 and suggest that nOPV2 will be an important tool in the eradication of poliomyelitis. nOPV2 surveillance should continue for the duration of the EUL.

Acute flaccid paralysis surveillance performance from 2011 to 2020 in Jonglei State, South Sudan: progress and challenges encountered.

IntroductionSouth Sudan reported the last indigenous wild poliovirus (WPV) in 2001 in Unity State, while the country was part of Sudan. In addition, the country reported an imported case of WPV in 2004-2005 and 2008-2009. The WPV circulation in the state was interrupted in 2009 and the last case was reported in Ayod county. The country continues to be at risk of importation of circulating vaccine-derived poliovirus type 2 (cVDPV2). In 2014 and 2020 the country experienced an outbreak of cVDPV2, in which Jonglei state was one of the affected states. Four out of 50 (8%) cVDPV2 cases in 2020 were reported from Jonglei State. The purpose of this study is to review surveillance performance indicators of Jonglei and compare them with the WHO surveillance performance standard and other country´s surveillance performances.Methodsretrospective secondary data analysis was conducted using the Jonglei state Acute Flaccid Paralysis (AFP) surveillance case-based database from 2011 to 2020. The reason for selecting Jonglei is because it is one of the poor performing states and is chronically hit by flood and internal conflicts. Data analyses were carried out using the Microsoft Excel (2016) program, where descriptive analysis frequencies, tables, and graphs were generated.Resultsthe study revealed that 346 AFP cases were reported in the counties of Jonglei state from 2011 through 2020. Out of 11 counties, 11 (100%) of them have reported suspected AFP cases. Children under five years accounted for 275 (79%) of all cases. The male gender accounted for 175 (51%) of all cases. A total of 249 (72%) had received three or more doses of Oral Polio Vaccine (OPV). Non-Polio Acute Flaccid Paralysis (NPAFP) rate varies from 1.2 in 2014 to 4.4 cases per 100,000 children under 15 years in 2018. The stool adequacy ranges from 58% in 2020 to 100% in 2011.Conclusionthe performance of Jonglei´s AFP surveillance system did not meet the WHO recommended target for both major AFP surveillance indicators (non-polio AFP rate and stool adequacy) during the study period.

The sensitivity of acute flaccid paralysis surveillance - the case of South Sudan: retrospective secondary analysis of AFP surveillance data 2014-2019.

IntroductionSouth Sudan has made quite impressive progress in interrupting wild poliovirus and maintaining a polio-free status since the last case was reported in 2009. South Sudan introduced different complementary strategies to enhance acute flaccid paralysis (AFP) surveillance. Hence, the objective of this study is to evaluate the sensitivity of the surveillance system using the WHO recommended surveillance standard and highlight the progress and challenges over the years.Methodswe conducted a retrospective, descriptive, quantitative study design and used the available secondary AFP surveillance database.Resultsthe overall non-polio AFP rate was 6.2/100,000 children under 15 years old in the study period. The stool adequacy was maintained well above the certification level of surveillance. The two main surveillance performance indicators were met at the national level throughout the study period. In contrast, only five out of ten states persistently attained and maintained the two main surveillance performance indicators throughout the study period, while in 2019 all states achieved except for Jonglei state. During the analysis period, no wild poliovirus was isolated except two circulating Vaccine Derived Poliovirus Type 2 (cVDPV2) cases in 2014 and one Immunodeficiency Vaccine Derived Poliovirus Type 2 (iVDPV2) case in 2015. However, on average, three cases were classified as polio compatible with each year of the study.ConclusionSouth Sudan met the two key surveillance performance indicators and had a sensitive AFP surveillance system during the period studied. We recommend intensifying surveillance activities in the former conflict-affected states and counties to maintain polio-free status.

Evaluation of the Acute Flaccid Paralysis Surveillance System From 2016 to 2020: A Retrospective Study

Background Acute flaccid paralysis surveillance played a major role in the global eradication of polio. The World Health Organization adopted this method to monitor the progress toward poliomyelitis eradication. The Expanded Program of Immunization in Jordan has routinely collected acute flaccid paralysis data since 1999, which then attained a polio-free certification. Yet, because of wars in neighboring countries such as Syria and Iraq, there is a risk of polio outbreaks occurring. Objective This study aims to evaluate the acute flaccid paralysis surveillance system in Jordan from 2016 to 2020 and identify areas for improvement. Methods This was a retrospective descriptive study that used data from the acute flaccid paralysis surveillance system in Jordan between January 2016 and December 2020. The World Health Organization standard indicators were used to evaluate the performance of the surveillance system. Results A total of 483 cases of nonpolio acute flaccid paralysis were reported. Most of them (n=478, 99%) were younger than 15 years, and among those, 55.6% were younger than 5 years, and 58% were male. At the national level, the surveillance achieved all the World Health Organization indicators throughout the evaluating period, except for two indicators: the proportion of stool specimens from which nonpolio enterovirus was isolated in 2016 and 2017, and the nonpolio acute flaccid paralysis detection rate per 100,000 of the population younger than 15 years in 2020. At a subnational level (governorates level), the proportion of stool specimens from which nonpolio enterovirus was isolated, as an indicator, was not achieved most of the time. This was frequently observed in each of all evaluation years of 2016 to 2020. Moreover, most indicators were not achieved at the governorate level in 2020. Conclusions There are some gaps that need improvement in the acute flaccid paralysis surveillance system in Jordan, especially at the governorate level. The lower performance during 2020 could have been caused by the COVID-19 crisis and the lockdown during the pandemic. Similar challenges are possible in the future and proper preparation is required.

Descriptive Epidemiology of Acute Flaccid Paralysis Cases in Afghanistan, 2015-2018

Background Polio is on the verge of eradication, while Afghanistan and Pakistan are the only endemic countries remaining where polio is still prevalent. Surveillance for acute flaccid paralysis (AFP) is one of the four cornerstone strategies of the Polio Eradication Initiative. Objective This study aims to describe the epidemiology of AFP cases in terms of time, place, and person. Methods It is a descriptive study whereby we analyzed the secondary data reported by AFP surveillance in Afghanistan. We accessed and used line lists from 2015 to 2018 to describe the epidemiological status of AFP cases in the country. With the use of Epi Info 7 and Microsoft Excel, we calculated descriptive measures, including frequencies, mean, median, SD, generated proportions, tables, and graphs. Results Overall, 11,513 cases were reported in the last 4 years (2015-2018) by AFP surveillance at the Ministry of Public Health. The majority of the cases (29%) were reported in 2018, while 2088 (18%) cases were reported in 2015. The trend of oral polio vaccination has increased from 2015 to 2018 (57%, 64%, 63%, and 68%, respectively). Most of the cases were reported from southern and western regions, and 57% were male cases. The highest (38%) proportions of cases were in those less than 30 months in age. Guillain-Barre syndrome was 38% of all categories. The samples were collected using appropriate procedures. However, the numbers of confirmed cases increased from 13 in 2016 to 14 in 2017, 20 in 2018, and 22 in 2019. Conclusions The AFP surveillance system is well established in the country. Nevertheless, with the increase in the trend of oral polio vaccine coverage, there is also an increase in the number of confirmed polio cases. Hence, the system should be sustained, and strategies should be strengthened to focus on the southern region as being the main engine of polio in the country.

Using the Acute Flaccid Paralysis Surveillance System to Identify Cases of Acute Flaccid Myelitis, Australia, 2000‒2018.

Since 2012, the United States has reported a distinct syndrome of acute flaccid paralysis (AFP) with anterior myelitis, predominantly in children. This polio-like syndrome was termed acute flaccid myelitis (AFM). Australia routinely conducts AFP surveillance to exclude poliomyelitis. We reviewed 915 AFP cases in Australia for children <15 years of age during 2000‒2018 and reclassified a subset to AFM by using the US Council of State and Territorial Epidemiologists case definition. We confirmed 37 AFM cases by using magnetic resonance imaging findings and 4 probable AFM cases on the basis of cerebrospinal fluid pleocytosis. Nonpolio enteroviruses were detected in 33% of AFM cases from which stool samples were tested. Average annual AFM incidence was 0.07 cases/100,000 person-years in children <15 years of age. AFM occurred sporadically in Australia before 2010 but regularly since then, indicating sustained, albeit rare, clinical manifestation in children. The AFP surveillance system in Australia is well-positioned to identify future AFM cases.

Enhancing acute flaccid paralysis surveillance system towards polio eradication: reverse cold chain monitoring in Nigeria, 2017 to 2019.

IntroductionHighly sensitive acute flaccid paralysis (AFP) surveillance is critical for detection of poliovirus circulation and documentation for polio-free certification. The reverse cold chain (RCC) is a system designed to maintain stool specimens in appropriate temperature for effective detection of poliovirus in the laboratory. We monitored the RCC of AFP surveillance in Nigeria to determine its effectiveness in maintaining viability of enterovirus.MethodsA descriptive cross-sectional study was conducted from November 2017 to December 2019. We included AFP cases from 151 Local Government Areas and monitored RCC of paired stool specimens from collection to arrival at laboratories. The national guideline recommends RCC temperature of +2 to +8°C and a non-polio enterovirus (NPENT) detection rate of ≥10%. We analyzed data with Epi Info 7, and presented results as frequencies and proportions, using Chi-square statistic to test for difference in enterovirus isolation.ResultsOf the 1,042 tracked paired stool specimens, 1,038(99.6%) arrived at the laboratory within 72 hours of collection of second specimen, 824(79.1%) were maintained within recommended temperature range, and 271(26%) yielded enteroviruses: 200(73.8%) NPENT, 66(24.4%) Sabin, 3(1.1%) vaccine derived poliovirus type 2 and 2(0.7%) mixture of Sabin and NPENT. The NPENT and Sabin rates were 19.2% and 6.7% respectively. Twenty-five percent of 824 specimens maintained within recommended temperature range, compared with 29.8% of 218 specimens with temperature excursion yielded enteroviruses (P=0.175).Conclusionthe RCC of AFP surveillance system in the study area was optimal and effective in maintaining the viability of enteroviruses. It was unlikely that poliovirus transmission was missed during the intervention.

Sensitivity of the acute flaccid paralysis surveillance system for poliovirus in South Africa, 2016-2019.

IntroductionGlobal poliovirus eradication is a public health emergency of international concern. The acute flaccid paralysis (AFP) surveillance programme in South Africa has been instrumental in eliminating polioviruses and keeping the country poliovirus free.Gap statementThe sensitivity of surveillance for polioviruses by every African country is of global interest in the effort to ensure global health security from poliovirus re-emergence.AimTo describe the epidemiology of polioviruses from AFP cases and environmental samples in South Africa and to report the performance of the AFP surveillance system for the years 2016–2019 against targets established by the World Health Organization (WHO).MethodsStool specimens from AFP or suspected AFP cases were received and tested as per WHO guidelines. Environmental samples were gathered from sites across the Gauteng province using the grab collection method. Concentration was effected by the two-phase polyethylene glycol method approved by the WHO. Suspected polioviruses were isolated in RD and/or L20B cell cultures through identification of typical cytopathic effects. The presence of polioviruses was confirmed by intratypic differentiation PCR. All polioviruses were sequenced using the Sanger method, and their VP1 gene analysed for mutations.ResultsData from 4597 samples (2385 cases) were analysed from the years 2016–2019. Two cases of immunodeficiency-associated vaccine-derived poliovirus (iVDPV) type 3 were detected in 2017 and 2018. A further 24 Sabin type 1 or type 3 polioviruses were detected for the 4 years. The national surveillance programme detected an average of 3.1 cases of AFP/100 000 individuals under 15 years old (2.8/100 000–3.5/100 000). The stool adequacy of the samples received was 53.0 % (47.0–55.0%), well below the WHO target of 80 % adequacy. More than 90 % of results were released from the laboratory within the turnaround time (96.6 %) and non-polio enteroviruses were detected in 11.6 % of all samples. Environmental surveillance detected non-polio enterovirus in 87.5 % of sewage samples and Sabin polioviruses in 12.5 % of samples.ConclusionThe AFP surveillance programme in South Africa is sensitive to detect polioviruses in South Africa and provided no evidence of wild poliovirus or VDPV circulation in the country.

Acute Flaccid Paralysis surveillance in Gaza Strip, Palestine

Acute Flaccid Paralysis (AFP) is a clinical syndrome characterized by rapid onset of weakness, in the respiratory muscles, swallowing and limbs; and death because of respiratory muscles failure. AFP surveillance is crucial, the clinicians are required to notify AFP cases among children below 15 years old to the epidemiology department with completion of AFP surveillance form. In this study, the researcher analyzed AFP surveillance investigation forms to determine the clinical picture, viral etiology and the incidence of AFP among children under 15 years. Material and method (Study design): The research study conducted through cross sectional study, based on AFP investigation forms in the epidemiology department. Results: The final diagnosis of the cases were as follow, 71.1% “27 cases” were GBS, 10.5 % were Encephalitis, 7.9% were Neuropathy, 2.6% “ one case only” for each of the following Aseptic meningitis, Transverse myelitis, Tuphus fever and Subspinal muscular atrophy. The incidence rate among children less than 15 years old which range from 0.58 / 100.000 in 2006 to 1.2 / 100.000 in 2012; Most of the cases 89.5 % had fully vaccinated and 10.5% had not fully vaccinated due to their ages less than one year, 65.8 “25 cases” had residual weakness, all of the cases classified as discarded according to the expert committee decision; no one case dead, all of the samples sent to the Tal-Hashomir laboratory in the occupied land in 1948 for viral isolation; most of the results 81.6% revealed that no virus isolated, 11.8% isolated ECHO virus, 2.6% isolated Coxsachie virus and 4 % isolated NPEV. Conclusion: No wild polio virus in Palestine which is polio free since 1984 and certified in 2006, our success being attributed to: start of 2 doses of IPV and 5 doses of OPV since 1978, high immunization coverage more than 95% at district level and effective AFP surveillance system in Palestine. The study recommended to encourage polio immunization and all clinicians are required to report all AFP cases and submitting at least two stool samples to the epidemiology department (at least 24 hours apart within 14 days of onset of paralysis).

Evaluation of acute flaccid paralysis surveillance indicators in Sokoto state, Nigeria, 2012\u20132019: a secondary data analysis

BackgroundNigeria, the last endemic country in the WHO African Region, was certified free of Wild Polio Virus (WPV) in 2020. However, due to low immunity in some communities in Sokoto, outbreaks of the circulating Vaccine Derived Polio Virus (cVDPV) occur. The aim of this study is to evaluate the Acute Flaccid Paralysis (AFP) surveillance indicators in Sokoto state, Nigeria.MethodsThis retrospective study was an analysis of routinely collected AFP surveillance data between 2012 and 2019 by the Sokoto state surveillance network. We assessed the Sokoto state AFP surveillance system using the AFP surveillance performance indicators. We performed all analyses using Microsoft Excel 2019.ResultsCumulatively, 3001 Acute Flaccid Paralysis (AFP) cases were reported over the evaluation period, out of which 1692 (56.4%) were males, and 2478 (82.4%) were below five years. More than half, 1773 (59.1%), had a fever at the beginning of the disease, and 1911 (63.7%) had asymmetric paralysis. The non-polio AFP rate (9.1 to 23.5% per 100,000 children < 15 years old) and stool adequacy rate (92.5 to 100%) indicate high sensitivity. The proportion of cases that had stool samples collected early, timely transported to the laboratory and arrived at the laboratory in optimal condition were all above the World Health Organization (WHO) minimum standard of 80%. There was inadequate profile documentation of some suspected cases.ConclusionsSokoto State has exceeded the WHO minimum standards in most of the AFP surveillance indicators. The performance of the system is sufficient enough to detect any reintroduction of WPV into the state. However, there is a need for improvement in data quality.
