Healing touch: applications in the acute care setting.

Nursing has been dedicated throughout its history to addressing the physical, psychologic, and spiritual aspects of the patient that influence the healing process. Current nursing practice in acute care is focused increasingly on monitoring equipment, giving medications, and administering medical treatments in a fast-paced environment that affords few opportunities for the deeper human connectedness between the nurse and the one who is ill and suffering. Healing touch (HT) is an energy-based complementary therapy fostering that nurse-patient connection. Nurses are beginning to use HT with their patients to assist in easing pain and anxiety, promote relaxation, accelerate wound healing, diminish depression, and increase a patient's sense of well-being. This article reports a conceptual framework for use of HT in acute care settings, describes specific HT techniques, and reviews numerous studies that have reported positive outcomes of HT as a noninvasive complementary therapy.

Nursing unit meal management maintenance program. Continuation of safe-swallowing and feeding beyond skilled therapeutic intervention.

Feeding, deglutition, and swallowing are the most natural means of alimentation. These means provide nutrition and hydration to the body. Management of dysphagia has become a widespread clinical practice in acute care, rehabilitation, and long-term skilled nursing facilities from rehabilitation and nursing perspectives. Caring for chronic dysphagic and feeding-impaired patients on nursing units is a central concern of this article. The purpose of this descriptive article is to report on a practical meal supervision program that may be used on nursing units to facilitate the continuum of dysphagia care after skilled therapeutic intervention. This program stresses the importance of: (a) identifying patient help level and corresponding nursing care; (b) centralized, supervised dining; (c) organized seating assignments; and (d) an effective, integrated communication system. This article addresses staff training and continuous quality improvement monitoring, and discusses potential investigations concerning compliance.

Infection Control in Home Care

Although home care has expanded in scope and intensity in the United States in the past decade, infection surveillance, prevention, and control efforts have lagged behind. Valid and reliable definitions and methods for surveillance are needed. Prevention and control efforts are largely based upon acute-care practices, many of which may be unnecessary, impractical, and expensive in a home setting. Infectious disease control principles should form the basis of training home- care providers to assess infection risk and develop prevention strategies.

Knowledge and Attitudes Among Dietetic Professionals Regarding Herbal Therapies

A random sample of 250 registered dietitians who are members of the New Jersey Dietetic Association was mailed a questionnaire. The questionnaire was designed to record knowledge of twelve currently popular herbal therapies and five attitude statements using a Likert-type scale. Ninety useable surveys were returned (36% return rate). All of the respondents were female and 73.3% were between 26 and 46 years of age. The highest degrees obtained were 56% bachelors, 40% masters, 3% doctorates; and 97% were registered dietitians. Most had been in practice <5 years (29%), 6-10 years (26%), or 16-25 years (23%) and were working in either acute care (24%), long-term care (21%), or private practice (21%) settings. Fifty percent or more were able to correctly identify the therapeutic indications for garlic, ginger, gingko biloba, St. John's wort, saw palmetto, echinacea, and ginseng. Only 28% to 34% correctly identified the therapeutic indications for dong quai, calendula, goldenseal, chaparral, and evening primrose. Thirty percent of the respondents felt comfortable counseling clients who use both conventional and herbal therapies. Forty-three percent feel there is a place for herbal therapy in disease treatment, while a majority (66%) disagreed with the statement that “there is no place for herbal therapy in disease treatment.” However, only 28% felt comfortable recommending herbal therapies.

Characteristics of Graduates of Dietetic Preparatory Programs

LEARNING OUTCOME: To describe three characteristic differences of graduates of three dietetics preparatory programs. The purpose of this study was to describe characteristics of dietetics graduates. Four characteristics: academic and supervised practice (SP) performances, self-efficacy perceptions in dietetic practice, and demographics were studied in a randomized sample stratified by program: Dietetic Internships (DIs) (n=394), Approved Pre-Professional Practice Programs (AP4 s) (n=280), and Coordinated Programs (CPs) (n=256). Data were collected from subjects, ADA, and SP directors using (a) a researcher-developed instrument, (b) registration examination results, and (c) ADA's performance appraisal form, respectively. No significant differences existed among groups in GPAs, registration examination scores, or SP performances. In the sample, mean GPAs ranged from 3.24 for sciences to 3.65 for major and mean SP Total scores were 75.36, equivalent to a performance rating of “More than Satisfactory.” AP4 s and CPs were significantly older than DIs. After controlling for the demographics effect, significant differences existed for two self-efficacy constructs: Choice of practice as a clinical dietitian, generally, and Choice of clinical practice in acute care and long-term care settings, specifically; and Efficacy Expectation in community practice with the general public. DIs and CPs were significantly more likely than AP4 s to choose a clinical position, generally, and choose clinical roles in acute-care and long-term care settings, specifically. However, AP4 s were significantly more likely than DIs and CPs to judge themselves personally efficacious, or able to perform, in community practice with the general public. These and other results point to important differences in graduates that relate to the future of dietetics practice.
