See related article, pages 1247–1255 The dilator function of the vascular endothelium is critical for optimal tissue perfusion, and this is brought into stark reality in a number of diseases, such as hypertension, diabetes, and preeclampsia, in which endothelium-dependent vasodilator function is impaired. Prostacyclin and nitric oxide (NO) were the earliest identified endothelium-dependent vasodilators, but it was soon found that in many vascular beds, particularly in resistance vessels, vasorelaxation persisted when production of these autacoids was suppressed. This residual vasorelaxation was invariably associated with hyperpolarization of the vascular smooth muscle and the entity was dubbed “endothelium-derived hyperpolarizing factor” (EDHF).1 From the outset, the identity of EDHF has been the subject of vigorous debate. Nonetheless, that the “EDHF effect” is blocked by agents that target calcium-activated K+ channels is among the few aspects of EDHF on which there is apparent consensus. There was also the debate on the significance, or otherwise, of EDHF. Is it just the poor cousin of NO, stepping into the breech when the latter is compromised, especially under conditions of enhanced oxidative stress? Then a beacon of light shone out from Japan, with the demonstration that EDHF assumed greater importance as vessel diameter decreased.2 This was a key observation, supporting the importance of EDHF in the region of the vascular system where resistance is primarily determined. This was hotly followed by the revelation that EDHF may be targeted in diseases, diabetes,3–5 hypertension,6 and apolipoprotein E-deficient mice.7 Thus, EDHF, suggested to provide vasodilation when NO was compromised, was also under attack in disease. Regional Differences in the Regulation of Vascular Smooth Muscle Relaxation by the Endothelium in Health and in Diabetes

The potent vasodilator influence of the vascular endothelium is compromised in diabetes, and this contributes to the increased vascular tone and reduced tissue perfusion characteristics of this disease. Regional differences exist in the extent of this vasodilator dysfunction, and in larger arteries the focus has been on nitric oxide (NO). In smaller arteries and arterioles, it is emerging that the actions of endothelium-derived hyperpolarizing factor (EDHF) are diminished in diabetes. In the mesenteric arteries of streptozotocin (STZ)-induced diabetic rats, smooth muscle (SM) hyperpolarization and relaxation attributed to EDHF is halved. It is unclear whether the dysfunction is due to changes in the generation of hyperpolarization in the endothelial cells (EC), or to spread of hyperpolarization to the SM via myoendothelial gap junctions (MEGJ), or in the SM response. In our study, we addressed this question. Responses were compared with those of the femoral artery where the entire endothelium-dependent relaxation is mediated by NO and is resistant to diabetes. Male Wistar rats were injected either with 60 mg/kg STZ dissolved in citrate buffer or with the buffer alone (control) into the tail vein, and 8 weeks after injection they were killed by exsanguination. Segments of the mesenteric and femoral arteries were isolated, secured to a recording chamber (or a myograph), and superfused with warmed oxygenated physiological saline. To isolate the EDHF response, 10 M N-nitro-L-arginine methyl ester and 10 M indomethacin were added to this saline. Intracellular glass microelectrodes with tips filled with 2% Lucifer Yellow CH were used to record the membrane potential of EC and SMC. Blood glucose levels were 6-fold higher in STZinjected rats. Acetylcholine evoked concentrationdependent hyperpolarization in the mesenteric artery EC and SMC of control rats. Hyperpolarization in EC was halved in diabetic rats, and this accounts for the reduced hyperpolarization and relaxation in SMC. Hyperpolarization, selectively evoked in the SMC by the ATP-sensitive potassium channel opener levcromakalim, spread to the EC via MEGJ, and this was not altered by diabetes. Femoral arteries lack MEGJ; the fact that the ability of EC to generate hyperpolarization is not influenced by diabetes is intriguing. Impaired generation of EC hyperpolarization accounts for the reduction in EDHF-mediated SMC hyperpolarization and relaxation in the mesenteric arteries in diabetes. The EC–SMC communication via MEGJ remains functional in diabetes. In contrast, EC hyperpolarization was preserved in the femoral arteries. Thus, there are regional differences in the impact of diabetes on EC hyperpolarization, and this may be related to the existence/absence of electrical and/or chemical contacts between EC and SMC.

Regulation of Vascular Smooth Muscle Relaxation by the Endothelium in Health and in Diabetes (with a Focus on Endothelium-Derived Hyperpolarizing Factor)

Amongst its wide repertoire of functions, the vascular endothelium plays a pivotal role in the regulation of vascular smooth muscle tone and ultimately tissue perfusion. In healthy vessels, the endothelium exerts a vasodilator influence on the underlying smooth muscle cells. In diabetes mellitus, endothelium-dependent vasodilation is impaired in various vascular beds and may contribute to the increased vascular tone and reduced tissue perfusion, which are features of this disease. There are regional variations in the extent of endothelial vasodilator dysfunction in diabetes, and the basis for this variation has yet to be resolved. The complement of vasodilators involved in endothelium-dependent relaxation varies in different vascular beds. In larger arteries and conduit vessels, the role of nitric oxide (NO) has been the focus of human and animal studies on diabetes. Small arteries and arterioles are important in the local regulation of tissue perfusion, and in many of these, another endothelial vasodilator, endothelium-derived hyperpolarizing factor (EDHF), plays an increasingly prominent role in overall endothelium-dependent relaxation. Surprisingly few studies have explored the influence of diabetes on EDHF; however, there is emerging evidence from a diverse range of vascular beds that the actions of EDHF are seriously compromised in diabetes. Vascular disease remains the leading cause of morbidity and mortality associated with diabetes mellitus. A better understanding of the regional differences and mechanisms involved in endothelial function and dysfunction in small arteries may reveal new strategies to aid in the prevention and/or therapeutic management of the vascular complications of diabetes mellitus.

Endothelium-dependent hyperpolarization and relaxation in mesenteric arteries of middle-aged rats: influence of oestrogen.

1. We determined whether gender and/or oestrogen deficiency affect endothelium-dependent hyperpolarization and relaxation in mesenteric arteries isolated from middle-aged (44 - 45 week old) rats. 2. The hyperpolarizing response to acetylcholine (ACh) was significantly greater in females than in males. Ovariectomy caused a marked reduction in ACh-induced hyperpolarization in female arteries, and this was improved by 17 beta-oestradiol replacement therapy. 3. ACh-induced relaxations in female arteries were not significantly different from those observed in male rats, and were unaffected by ovariectomy, regardless of whether indomethacin was present. However, when endothelial nitric oxide synthase (eNOS) was blocked with N(G)-nitro-L-arginine, the sensitivity and maximum relaxant response to ACh was significantly higher in intact females compared with males and ovariectomized females. Treatment with 17 beta-oestradiol prevented the reduced vasorelaxant response in ovariectomized females. 4. Immunohistochemical examination for eNOS showed no apparent difference in eNOS protein expression in the endothelium of arteries between intact and ovariectomized females. 5. Since circulating concentrations of oestrogen were essentially low in middle-aged female rats, the present results suggest that subtle changes from a critical concentration of oestrogen at this age may strongly affect the vascular actions of endothelium-derived hyperpolarizing factor without effect on eNOS expression and activity.

Role of endothelial Ni(2+)-sensitive Ca(2+) entry pathway in regulation of EDHF in porcine coronary artery.

Elevation of intracellular Ca(2+) concentration ([Ca(2+)](i)) in endothelial cells is proposed to be required for generation of vascular actions of endothelium-derived hyperpolarizing factor (EDHF). This study was designed to determine the endothelial Ca(2+) source that is important in development of EDHF-mediated vascular actions. In porcine coronary artery precontracted with U-46619, bradykinin (BK) and cyclopiazonic acid (CPA) caused endothelium-dependent relaxations in the presence of N(G)-nitro-L-arginine (L-NNA). The L-NNA-resistant relaxant responses were inhibited by high K(+), indicating an involvement of EDHF. In the presence of Ni(2+), which inhibits Ca(2+) influx through nonselective cation channels, the BK-induced EDHF relaxant response was greatly diminished and the CPA-induced response was abolished. BK and CPA elicited membrane hyperpolarization of smooth muscle cells of porcine coronary artery. Ni(2+) suppressed the hyperpolarizing responses in a manner analogous to removal of extracellular Ca(2+). EDHF-mediated relaxations and hyperpolarizations evoked by BK and CPA in porcine coronary artery showed a temporal correlation with the increases in [Ca(2+)](i) in porcine aortic endothelial cells. The extracellular Ca(2+)-dependent rises in [Ca(2+)](i) in endothelial cells stimulated with BK and CPA were completely blocked by Ni(2+). These results suggest that Ca(2+) influx into endothelial cells through nonselective cation channels plays a crucial role in the regulation of EDHF.

Human urotensin-II is an endothelium-dependent vasodilator in rat small arteries.

The possible role of the endothelium in modulating responses to human urotensin-II (U-II) was investigated using isolated segments of rat thoracic aorta, small mesenteric artery, left anterior descending coronary artery and basilar artery. Human U-II was a potent vasoconstrictor of endothelium-intact isolated rat thoracic aorta (EC(50)=3.5+/-1.1 nM, R(max)=103+/-10% of control contraction induced by 60 mM KCl and 1 microM noradrenaline). However the contractile response was not significantly altered by removal of the endothelium or inhibition of nitric oxide synthesis with L-NAME (100 microM). Human U-II did not cause relaxation of noradrenaline-precontracted, endothelium-intact rat aortae. Human U-II contracted endothelium-intact rat isolated left anterior descending coronary arteries (EC(50)=1.3+/-0.8 nM, R(max)=20.1+/-4.9% of control contraction induced by 10 microM 5-HT). The contractile response was significantly enhanced by removal of the endothelium (R(max)=55.4+/-16.1%). Moreover, human U-II caused concentration-dependent relaxation of 5-HT-precontracted arteries, which was abolished by L-NAME or removal of the endothelium. No contractile effects of human U-II were found in rat small mesenteric arteries. However the peptide caused potent, concentration- and endothelium-dependent relaxations of methoxamine-precontracted vessels. The relaxant responses were potentiated by L-NAME (300 microM) but abolished in the additional presence of 25 mM KCl (which inhibits the actions of endothelium-derived hyperpolarizing factor). The present study is the first to show that human U-II is a potent endothelium-dependent vasodilator in some rat resistance vessels, and acts through release of EDHF as well as nitric oxide. Our findings have also highlighted clear anatomical differences in the responses of different vascular beds to human U-II which are likely to be important in determining the overall cardiovascular activity of this peptide.

Nitric oxide exerts feedback inhibition on EDHF-induced coronary arteriolar dilation in vivo.

We tested the hypothesis that nitric oxide (NO) inhibits endothelium-derived hyperpolarizing factor (EDHF)-induced vasodilation via a negative feedback pathway in the coronary microcirculation. Coronary microvascular diameters were measured using stroboscopic fluorescence microangiography. Bradykinin (BK)-induced dilation was mediated by EDHF, when NO and prostaglandin syntheses were inhibited, or by NO when EDHF and prostaglandin syntheses were blocked. Specifically, BK (20, 50, and 100 ng. kg(-1). min(-1) ic) caused dose-dependent vasodilation similarly before and after administration of N(G)-monomethyl-L-arginine (L-NMMA) (3 micromol/min ic for 10 min) and indomethacin (Indo, 10 mg/kg iv). The residual dilation to BK with L-NMMA and Indo was completely abolished by suffusion of miconazole or an isosmotic buffer containing high KCl (60 mM), suggesting that this arteriolar vasodilation is mediated by the cytochrome P-450 derivative EDHF. BK-induced dilation was reduced by 39% after inhibition of EDHF and prostaglandin synthesis, and dilation was further inhibited by combined blockade with L-NMMA to a 74% reduction in the response. This suggests an involvement for NO in the vasodilation. After dilation to BK was assessed with L-NMMA and Indo, sodium nitroprusside (SNP, 1-3 microgram. kg(-1). min(-1) ic), an exogenous NO donor, was administered in a dose to increase the diameter to the original control value. Dilation to BK was virtually abolished when administered concomitantly with SNP during L-NMMA and Indo (P < 0.01 vs. before SNP), suggesting that NO inhibits EDHF-induced dilation. SNP did not affect adenosine- or papaverine-induced arteriolar dilation in the presence of L-NMMA and Indo, demonstrating that the effect of SNP was not nonspecific. In conclusion, our data are the first in vivo evidence to suggest that NO inhibits the production and/or action of EDHF in the coronary microcirculation.

Role of gap junctions in acetylcholine-induced vasodilation of proximal and distal arteries of the rat mesentery

We have previously shown that myoendothelial gap junctions are more prevalent in distal than in proximal arteries of the rat mesentery. In the present study we have investigated the role of gap junctions in the mechanism of action of endothelium-derived hyperpolarizing factor (EDHF) in these same vessels following relaxation with acetylcholine. Arteries were pre-constricted with phenylephrine and concentration response curves to acetylcholine were constructed in the presence of N G-nitro- l-arginine methyl ester ( l-NAME; 10 −5 M) and indomethacin (10 −5 M) to prevent effects due to the release of nitric oxide and prostacyclins. Nitric oxide was found to have only a small role in the relaxation of the proximal vessels and was not involved in the relaxations of the distal vessels. 18α-Glycyrrhetinic acid (10 −5 M), a putative gap junction uncoupler, significantly reduced acetylcholine-induced relaxations by 50% in both proximal and distal vessels. Potassium channel antagonists, tetraethylammonium chloride (TEA; 10 −3 M) and barium chloride (10 −4 M), together abolished the dilatory response in the proximal mesenteric arteries, but did not completely block responses in the distal arteries. The data suggest that gap junctions contribute significantly to the acetylcholine-induced relaxation in both proximal and distal arteries of the rat mesentery. We hypothesize that the absence of a correlation between the role of gap junctions and the incidence of myoendothelial gap junctions in these same vessels is due to significant effects of the inhibitors on gap junctions located in the smooth muscle layers of the larger vessels.

Characterization of Endothelium- Dependent Relaxation in Guinea Pig Basilar Artery – Effect of Hypoxia and Role of Cytochrome P450 Mono-Oxygenase

In the guinea pig basilar artery, acetylcholine and the calcium ionophore A23187 induced endothelium-dependent relaxations, which were not significantly affected by the nitric oxide (NO) synthase inhibitor N^ω-nitro-L-arginine (L-NOARG; 0.3 mM) or the guanylate cyclase inhibitor ODQ (1H-[1,2,4]oxadiazolo[4,3-a]quinoxaline-1-one; 1-10 µM), or by these inhibitors combined. However, acetylcholine (10 µM) and A23187 (3 µM) each significantly increased the tissue level of cGMP in the absence but not in the presence of L-NOARG, suggesting that NO is released from the vascular endothelium in this blood vessel. Treatment with the potassium (K) channel inhibitors charybdotoxin (0.1 µM) plus apamin (0.1 µM), a toxin mixture previously shown to inhibit relaxations mediated by endothelium-derived hyperpolarizing factor (EDHF) in this artery, had no effect on the A23187-induced relaxation but slightly inhibited the response to acetylcholine (E_max was reduced by 24%). When the action of EDHF was prevented by these K channel inhibitors, the remaining relaxation was abolished by either ODQ (1 µM) or L-NOARG (0.3 mM), indicating that NO, apart from EDHF, contributes to the endothelium-dependent relaxations. Furthermore, ODQ (10 µM) abolished the relaxation induced by the NO donor S-nitroso-N-acetylpenicillamine. Thus, activation of soluble guanylate cyclase seems to be the only mechanism through which NO causes relaxation in this artery. When vessels were exposed to grave hypoxia (pO₂ = 6 mm Hg), the NO-mediated relaxation (induced by acetylcholine in the presence of charybdotoxin plus apamin) disappeared. In contrast, EDHF-mediated responses (elicited by acetylcholine in the presence of L-NOARG) were only marginally affected by hypoxia (E_max was reduced by 16%). 17-Octadecynoic acid (50 µM) and 5,8,11,14-eicosatetraynoic acid (10 µM), inhibitors of cytochrome P₄₅₀-dependent oxidation of arachidonic acid, failed to inhibit the acetylcholine-induced relaxation in the presence of L-NOARG. The cytochrome P₄₅₀-dependent arachidonic acid metabolite 11,12-epoxyecosatrienoic acid (0.3–3.0 µM) had no relaxant effect per se. In conclusion, EDHF and NO are both mediators of endothelium-dependent relaxations in the guinea pig basilar artery. However, during grave hypoxia, EDHF alone mediates acetylcholine-induced relaxation. The results further suggest that EDHF is not a metabolite of arachidonic acid formed by cytochrome P₄₅₀ mono-oxygenase or generated by another oxygen-dependent enzyme in this artery.

Comparative pharmacology of endothelium-derived hyperpolarizing factor and anandamide in rat isolated mesentery

We have recently proposed that anandamide, or a related cannabinoid, is the endothelium-derived hyperpolarizing factor (EDHF) and have now compared EDHF-mediated responses (induced by carbachol in the presence of both nitric oxide and prostanoid synthesis inhibitors) with those induced by anandamide in the rat isolated superior mesenteric arterial bed. Both EDHF-mediated and anandamide-induced relaxations were inhibited in the presence of high K + (60 mM) and opposed by blockade of K + channels with 10 mM tetraethylammonium. The cytochrome P450 inhibitors, and putative EDHF inhibitors, clotrimazole (10 μM) and proadifen (SKF 525A) (10 μM), opposed both anandamide-induced and EDHF-mediated relaxations and also relaxant responses to the K + channel activator levcromakalim. Therefore, EDHF-mediated and anandamide-induced vasorelaxations show very similar pharmacological characteristics, with both responses being mediated via K + channel activation. Further, the actions of EDHF and anandamide are both sensitive to proadifen and clotrimazole, EDHF antagonists which appear to act through K + channel inhibition. Accordingly, these results support our proposal that an endocannabinoid is an EDHF.

The relative importance of nitric oxide and nitric oxide-independent mechanisms in acetylcholine-evoked dilatation of the rat mesenteric bed.

1. The relative contribution of nitric oxide (NO) to acetylcholine-induced smooth muscle relaxation was investigated in the rat perfused mesenteric vasculature and in isolated segments of second, third and fourth order arterial branches. 2. The EC50 values and maximal relaxation to acetylcholine were not significantly different in the sequential arterial branches, being approximately 0.05 microM and 85%, respectively. 3. The NO synthase inhibitor L-NG-nitro-L-arginine methyl ester (L-NAME; 100 microM) reduced acetylcholine-evoked endothelium-dependent dilatation and relaxation in the perfused mesenteric bed and in isolated arterial segments. The maximum response to acetylcholine in both preparations was reduced by between 35% to 40% while the EC50 values were increased by 5-6 fold. L-NAME had no effect on basal smooth muscle tone in either case. 4. In contrast, endothelium-dependent dilatation of the perfused mesenteric bed evoked by A23187 (0.002-20 nmol), was unaffected by exposure to L-NAME. The EC50 values and maximal responses elicited by A23187 (20 nmol) before and after exposure to L-NAME were 0.96 +/- 0.5 nmol and 67.0 +/- 7.0% (n = 4), and 0.7 +/- 0.4 nmol and 70.0 +/- 5.0% (n = 4; P > 0.01), respectively. 5. Perfusion of the isolated mesenteric bed with raised K(+)-Krebs buffer (25 mM) had no effect on basal tone, but reduced the amplitude of both acetylcholine- and A23187-evoked dilatation. The maximum responses to acetylcholine (2 micromol) and A23187 (20 nmol) were reduced from 67.5 +/- 7.3% and 65.4+/-8.2% to 18.9 +/-11.0% (n=5; P<0.01) and 13.5 +/-12.0% (n=4; P<0.01), respectively.6. Exposure of the mesenteric bed to L-NAME in the presence of raised K+-Krebs further reduced the maximal response elicited by acetylcholine to only 8.9 +/- 2.8% (n =4; P< 0.01).7. These results indicate that acetylcholine-evoked vasodilatation of the rat mesenteric vasculature is mediated by both NO-dependent and -independent mechanisms. The relative contribution made by these mechanisms does not appear to differ in sequential branches of the mesenteric artery. In contrast,A23187-evoked vasodilatation appears to be mediated predominantly by a NO-independent mechanism which is sensitive to increases in the extracellular potassium concentration and may reflect the action of endothelium-derived hyperpolarizing factor (EDHF).
