Optical coherence elastography under homolateral parallel acoustic radiation force excitation for ocular elasticity quantification.

Alteration in the elastic properties of biological tissues may indicate changes in the structure and components. Acoustic radiation force optical coherence elastography (ARF-OCE) can assess the elastic properties of the ocular tissues non-invasively. However, coupling the ultrasound beam and the optical beam remains challenging. In this Letter, we proposed an OCE method incorporating homolateral parallel ARF excitation for measuring the elasticity of the ocular tissues. An acoustic-optic coupling unit was established to reflect the ultrasound beam while transmitting the light beam. The ARF excited the ocular tissue in the direction parallel to the light beam from the same side of the light beam. We demonstrated the method on the agar phantoms, the porcine cornea, and the porcine retina. The results show that the ARF-OCE method can measure the elasticity of the cornea and the retina, resulting in higher detection sensitivity and a more extensive scanning range.

In Vivo Evaluation of Corneal Biomechanics Following Cross-Linking Surgeries Using Optical Coherence Elastography in a Rabbit Model of Keratoconus.

Validation of the feasibility of novel acoustic radiation force optical coherence elastography (ARF-OCE) for the evaluation of biomechanical enhancement of the in vivo model of keratoconus by clinical cross-linking (CXL) surgery. Twelve in vivo rabbit corneas were randomly divided into two groups. Both groups were treated with collagenase type II, and a keratoconus model was obtained. Then, the two groups were treated with CXL procedures with different irradiation energy of 15 J and 30 J (CXL-15 J and CXL-30 J, respectively). An ARF-OCE probe with an ultrasmall ultrasound transducer was used to detect the biomechanical properties of cornea. An antisymmetric Lamb wave model was combined with the frequency dispersion relationship to achieve depth-resolved elastography. Compared with the phase velocity of the Lamb wave in healthy corneas (approximately 3.96 ± 0.27 m/s), the phase velocity of the Lamb wave was lower in the keratoconus region (P < 0.05), with an average value of 3.12 ± 0.12 m/s. Moreover, the corneal stiffness increased after CXL treatment (P < 0.05), and the average phase velocity of the Lamb wave was 4.3 ± 0.19 m/s and 4.54 ± 0.13 m/s after CXL-15 J and CXL-30 J treatment. The Young's moduli of the keratoconus regions were significantly lower than the healthy corneas. Moreover, the Young's modulus of the keratoconus regions was significantly higher after CXL-30 J treatment than after CXL-15 J treatment. We demonstrated that the ARF-OCE technique has great potential in screening keratoconus and guiding clinical CXL treatment. This work accelerates the clinical translation of OCE systems using ultrasmall ultrasound transducers and is used to guide CXL procedures.

Quantitative assessment of corneal elasticity distribution after FS-LASIK using optical coherence elastography.

Quantifying corneal elasticity after femtosecond laser-assisted in situ keratomileusis (FS-LASIK) procedure plays an important role in improving surgical safety and quality, since some latent complications may occur ascribing to changes in postoperative corneal biomechanics. Nevertheless, it is suggested that current research has been severely constrained due to the lack of an accurate quantification method to obtain postoperative corneal elasticity distribution. In this paper, an acoustic radiation force optical coherence elastography system combined with the improved phase velocity algorithm was utilized to realize elasticity distribution images of the in vivo rabbit cornea after FS-LASIK under various intraocular pressure levels. As a result, elasticity variations within and between the regions of interest could be identified precisely. This is the first time that elasticity imaging of in vivo cornea after FS-LASIK surgery was demonstrated, and the results suggested that this technology may hold promise in further exploring corneal biomechanical properties after refractive surgery.

Two-dimensional elastic distribution imaging of the sclera using acoustic radiation force optical coherence elastography.

The scleral elasticity is closely related with many ocular diseases, but the relevant research is still insufficient. Here, we utilized optical coherence elastography to carefully study biomechanical properties of the sclera at different positions and under different intraocular pressures. Meanwhile, elastic wave velocity and Young's modulus of each position were obtained using a phase velocity algorithm. Accordingly, the two-dimensional elasticity distribution image was achieved by mapping the Young's modulus values to the corresponding structure based on the relationship between the position and its Young's modulus. Therefore, elastic information in regions-of-interest can be read and compared directly from the scleral structure, indicating that our method may be a very useful tool to evaluate the elasticity of sclera and provide intuitive and reliable proof for diagnosis and research.

Novel acoustic radiation force optical coherence elastography based on ultrasmall ultrasound transducer for biomechanics evaluation of in vivo cornea.

We developed a novel acoustic radiation force optical coherence elastography (ARF-OCE) based on an ultrasmall ultrasound transducer for quantitative biomechanics evaluations of in vivo cornea. A custom single-sided meta-ultrasonic transducer with an outer diameter of 1.8 mm, focal spot diameter of 1.6 mm, central frequency of 930 kHz, and focal length of 0.8 mm was applied to excite the sample. The sample arm of the ARF-OCE system employed a three-dimensional printed holder that allowed for ultrasound excitation and ARF-OCE detection. The phase-resolved algorithm was combined with a Lamb wave model to depth-resolved evaluate corneal biomechanics after keratoconus and cross-linking treatments (CXL). The results showed that, compare to the healthy cornea, the Lamb wave velocity was significantly reduced in the keratoconus, increased in the cornea after CXL, and increased with cross-linked irradiation energy in the cornea. These results indicated the good clinical translation potential of the proposed novel ARF-OCE.

Evaluation of Residual Corneal Stromal Bed Elasticity by Optical Coherence Elastography Based on Acoustic Radiation Force

Despite the rapidly growing popularity of laser vision correction (LVC) in the correction of myopia, its quantitative evaluation has not been thoroughly investigated. In this paper, an acoustic radiation force–optical coherence elastography (ARF-OCE) system was proposed to evaluate LVC by measuring the residual stromal bed (RSB) elasticity, because it is directly relevant to the RSB thickness that is critical to maintaining normal corneal function. As expected, the Young’s modulus of the RSB was calculated, then its relationship with the RSB thickness was determined. More significantly, a specific thickness was revealed in which the Young’s modulus changed dramatically, which may imply that there is a high risk of complication caused by over-cutting of the cornea. To the best of our knowledge, this is the first ARF-OCE imaging of the RSB, which may help to determine the safe RSB thickness and thus may help us to quantitatively assess LVC surgery.

Acoustic radiation force optical coherence elastography: A preliminary study on biomechanical properties of trabecular meshwork.

Evaluating biomechanical properties of trabecular meshwork (TM) is of great significance for understanding the mechanism of aqueous humor circulation and its relationship to some eye diseases such as glaucoma; however, there is almost no relevant study due to the lack of clinical measurement tool. In this paper, an acoustic radiation force optical coherence elastography (ARF-OCE) system is developed with the advantages of noninvasive detection, high resolution, high sensitivity, and high-speed imaging, by which elastic modulus of the porcine and human TMs is accurately quantified. As the first OCE imaging of TM, our study demonstrates that ARF-OCE may be an effective approach to advance the research of diseases related to aqueous humor circulation.

Quantitative Evaluation of In Vivo Corneal Biomechanical Properties after SMILE and FLEx Surgery by Acoustic Radiation Force Optical Coherence Elastography.

The purpose of this study is to quantitatively evaluate the differences in corneal biomechanics after SMILE and FLEx surgery using an acoustic radiation force optical coherence elastography system (ARF-OCE) and to analyze the effect of the corneal cap on the integrity of corneal biomechanical properties. A custom ring array ultrasound transducer is used to excite corneal tissue to produce Lamb waves. Depth-resolved elastic modulus images of the in vivo cornea after refractive surgery were obtained based on the phase velocity of the Lamb wave. After refractive surgery, the average elastic modulus of the corneal flap decreased (71.7 ± 24.6 kPa), while the elastic modulus of the corneal cap increased (219.5 ± 54.9 kPa). The average elastic modulus of residual stromal bed (RSB) was increased after surgery, and the value after FLEx (305.8 ± 48.5 kPa) was significantly higher than that of SMILE (221.3 ± 43.2 kPa). Compared with FLEx, SMILE preserved most of the anterior stroma with less change in corneal biomechanics, which indicated that SMILE has an advantage in preserving the integrity of the corneal biomechanical properties. Therefore, the biomechanical properties of the cornea obtained by the ARF-OCE system may be one of the essential indicators for evaluating the safety of refractive surgery.

Quantitative Assessment of Biomechanical Properties of the Human Keratoconus Cornea Using Acoustic Radiation Force Optical Coherence Elastography.

PurposeQuantification of biomechanical properties of keratoconus (KC) corneas has great significance for early diagnosis and treatment of KC, but the corresponding clinical measurement remains challenging. Here, we developed an acoustic radiation force (ARF) optical coherence elastography technique and explored its potential for evaluating biomechanical properties of KC corneas.MethodsAn ARF system was used to induce the tissue deformation, which was detected by an optical coherence tomography system, and thus the localized point-by-point Young's modulus measurements were achieved. Then, two healthy rabbit eyes were imaged to test the system, after which the human keratoconus cornea was evaluated by using the same method. Three regions were selected for biomechanics analysis: the conical region, the transitional region, and the peripheral region.ResultsYoung's moduli of transitional region ranged from 53.3 to 58.5 kPa. The corresponding values for the peripheral region were determined to be 58.6 kPa and 63.2 kPa, respectively. Young's moduli of the conical region were gradually increased by 18.3% from the center to the periphery, resulting in the minimum and maximum values of 44.9 kPa and 53.1 kPa, respectively. Furthermore, Young's moduli of the anterior and posterior of the center were determined to be 44.9 kPa and 50.7 kPa, respectively.ConclusionsDifferences in biomechanical properties between the three regions and slight variations within the conical region were clearly distinguished. Biomechanical weakening of the keratoconus cornea was mainly localized in the conical region, especially in the vertex position.Translational RelevanceThe system may provide a promising clinical tool for the noninvasive evaluation of local corneal biomechanics and thus may have potential applications in early keratoconus detection with further optimization.

Elasticity measurements of ocular anterior and posterior segments using optical coherence elastography.

The changes of biomechanical properties, especially the elasticity of the ocular tissues, are closely related to some ophthalmic diseases. Currently, the ophthalmic optical coherence elastography (OCE) systems are dedicated either to the anterior segment or to the retina. The elasticity measurements of the whole eye remain challenging. Here we demonstrated an acoustic radiation force optical coherence elastography (ARF-OCE) method to quantify the elasticity of the cornea and the retina. The experiment results show that the Young's moduli of the cornea and the retina were 16.66 ± 6.51 kPa and 207.96 ± 4.75 kPa, respectively. Our method can measure the elasticity of the anterior segment and the posterior segment, and provides a powerful tool to enhance ophthalmology research.

Quantification for biomechanical properties of human cornea by using acoustic radiation force optical coherence elastography

Acoustic radiation force optical coherence elastography (ARF-OCE) has been widely employed for elasticity imaging of ocular tissues, however, it has not been applied to detecting the biomechanical properties of the human cornea. In this paper, we performed the experiments on ex vivo human eyes by using ARF-OCE, where the 2D OCT structural images and elastic wave propagations from human corneas were achieved, and the resulting Young’s modulus agreed well with the results measured by other methods, indicating the feasibility and reliability of ARF-OCE. With the advantages of non-invasive measurement, high resolution, high sensitivity and real-time imaging, ARF-OCE could be an effective way to rebuild the cornea elasticity and may be applied in clinical ophthalmology in the future.

Quantification of biomechanical properties of human corneal scar using acoustic radiation force optical coherence elastography.

Biomechanical properties of corneal scar are strongly correlated with many corneal diseases and some types of corneal surgery, however, there is no elasticity information available about corneal scar to date. Here, we proposed an acoustic radiation force optical coherence elastography system to evaluate corneal scar elasticity. Elasticity quantification was first conducted on ex vivo rabbit corneas, and the results validate the efficacy of our system. Then, experiments were performed on an ex vivo human scarred cornea, where the structural features, the elastic wave propagations, and the corresponding Young's modulus of both the scarred region and the normal region were achieved and based on this, 2D spatial distribution of Young's modulus of the scarred cornea was depicted. Up to our knowledge, we realized the first elasticity quantification of corneal scar, which may provide a potent tool to promote clinical research on the disorders and surgery of the cornea.

Acoustic radiation force optical coherence elastography for ocular biomechanics

The mechanical properties of the cornea, limbus, and sclera provide important information regarding eye health. Optical coherence elastography (OCE) is highly suitable for assessing ocular tissues' mechanical properties due to its high resolution and superior displacement sensitivity. Typically, elastography involves an external mechanical excitation of the tissue. We recently adopted a method of Air-Coupled Acoustic Radiation Force for the generation of elastic waves in ocular tissues. This method shows several clear advantages other than the traditionally used air-puff excitation. However, the safety of this method needs to be studied before its clinical applications. Also, there is increased interest in passive elastography, where the mechanical response to natural physiological forces like heartbeat and respiration is measured. I will also demonstrate examples of using heartbeat for quantification of tissues' mechanical properties in vivo.

2-D Ultrasonic Array-Based Optical Coherence Elastography.

Acoustic radiation force optical coherence elastography (ARF-OCE) has been successfully implemented to characterize the biomechanical properties of soft tissues, such as the cornea and the retina, with high resolution using single-element ultrasonic transducers for ARF excitation. Most currently proposed OCE techniques, such as air puff and ARF, have less capability to control the spatiotemporal information of the induced region of deformation, resulting in limited accuracy and low temporal resolution of the shear wave elasticity imaging. In this study, we propose a new method called 2-D ultrasonic array-based OCE imaging, which combines the advantages of 3-D dynamic electronic steering of the 2-D ultrasonic array and high-resolution optical coherence tomography (OCT). The 3-D steering capability of the 2-D array was first validated using a hydrophone. Then, the combined 2-D ultrasonic array OCE system was calibrated using a homogenous phantom, followed by an experiment on ex vivo rabbit corneal tissue. The results demonstrate that our newly developed 2-D ultrasonic array-based OCE system has the capability to map tissue biomechanical properties accurately, and therefore, has the potential to be a vital diagnostic tool in ophthalmology.

Characterizing blood clots using acoustic radiation force optical coherence elastography and ultrasound shear wave elastography

Thromboembolism in a cerebral blood vessel is associated with high morbidity and mortality. Mechanical thrombectomy (MT) is one of the emergenc proceduresperformed to remove emboli. However, the interventional approaches such as aspiration catheters or stent retriever are empirically selected. An inappropriate selection of surgical devices can influence the success rate during embolectomy, which can lead to an increase in brain damage. There has been growing interest in the study of clot composition and using a priori knowledge of clot composition to provide guidance for an appropriate treatment strategy for interventional physicians. Developing imaging tools which can allow interventionalists to understand clot composition could affect management and device strategy. In this study, we investigated how clots of different compositions can be characterized by using acoustic radiation force optical coherence elastography (ARF–OCE) and compared with ultrasound shear wave elastography (SWE). Five different clots compositions using human blood were fabricated into cylindrical forms from fibrin-rich (21% red blood cells, RBCs) to RBC-rich (95% RBCs). Using the ARF–OCE and SWE, we characterized the wave velocities measured in the time-domain. In addition, the semi-analytical finite element model was used to explore the relationship between the phase velocities with various frequency ranges and diameters of the clots. The study demonstrated that the wave group velocities generally decrease as RBC content increases in ARF–OCE and SWE. The correlation of the group velocities from the OCE and SWE methods represented a good agreement as RBC composition is larger than 39%. Using the phase velocity dispersion analysis applied to ARF–OCE data, we estimated the shear wave velocities decoupling the effects of the geometry and material properties of the clots. The study demonstrated that the composition of the clots can be characterized by elastographic methods using ARF–OCE and SWE, and OCE demonstrated better ability to discriminate between clots of different RBC compositions, compared to the ultrasound-based approach, especially in clots with low RBC compositions.

Quantification of iris elasticity using acoustic radiation force optical coherence elastography

Careful quantification of the changes in biomechanical properties of the iris can offer insight into the pathophysiology of some ocular diseases. However, to date there has not been much information available regarding this subject because clinical detection for iris elasticity remains challenging. To overcome this limitation, we explore, for the first time to our knowledge, the potential of measuring iris elasticity using acoustic radiation force optical coherence elastography (ARF-OCE). The resulting images and shear wave propagation, as well as the corresponding shear modulus and Young's modulus from ex vivo and in vivo rabbit models confirmed the feasibility of this method. With features of noninvasive imaging, micrometer-scale resolution, high acquisition speed and real-time processing, ARF-OCE is a promising method for reconstruction of iris elasticity and may have great potential to be applied in clinical ophthalmology with further refinement.

Acoustic radiation force optical coherence elastography for evaluating mechanical properties of soft condensed matters and its biological applications.

Evaluating mechanical properties of biological soft tissues and viscous mucus is challenging because of complicated dynamic behaviors. Soft condensed matter models have been successfully used to explain a number of dynamical behaviors. Here, we reported that optical coherence elastography (OCE) is capable of quantifying mechanical properties of soft condensed matters, micellar fluids. A 7.5 MHz focused transducer was utilized to generate acoustic radiation force exerted on the surface of soft condensed matters in order to produce Rayleigh waves. The waves were recorded by optical coherence tomography (OCT). The Kelvin-Voigt model was adopted to evaluate shear modulus and loss modulus of soft condensed matters. The results reported that various concentrations of micellar fluids can provide reasonable ranges of elasticity from 65.71 to 428.78 Pa and viscosity from 0.035 to 0.283 Pa·s, which are close to ranges for actual biological samples, like mucus. OCE might be a promising tool to differentiate pathologic mucus samples from healthy cases as advanced applications in the future.

Spatial localization of mechanical excitation affects spatial resolution, contrast, and contrast-to-noise ratio in acoustic radiation force optical coherence elastography.

The notion that a spatially confined mechanical excitation would produce an elastogram with high spatial resolution has motivated the development of various elastography techniques with localized mechanical excitation. However, a quantitative investigation of the effects of spatial localization of mechanical excitation on the spatial resolution of elastograms is still lacking in optical coherence elastography (OCE). Here, we experimentally investigated the effect of spatial localization of acoustic radiation force (ARF) excitation on spatial resolution, contrast, and contrast-to-noise ratio (CNR) of dynamic uniaxial strain elastograms in dynamic ARF-OCE, based on a framework for analyzing the factors that influence the quality of the elastogram at different stages of the elastography workflow. Our results show that localized ARF excitation with a smaller acoustic focal spot size produced a strain elastogram with superior spatial resolution, contrast, and CNR. Our results also suggest that the spatial extent spanned by the displacement response in the sample may connect between the spatial localization of the mechanical excitation and the resulting elastogram quality. The elastography framework and experimental approach presented here may provide a basis for the quantitative analysis of elastogram quality in OCE that can be adapted and applied to different OCE systems and applications.

Quantified elasticity mapping of retinal layers using synchronized acoustic radiation force optical coherence elastography.

Age-related macular degeneration (AMD) is the leading cause of blindness in the elderly (over the age of 60 years) in western countries. In the early stages of the disease, structural changes may be subtle and cannot be detected. Recently it has been postulated that the mechanical properties of the retina may change with the onset of AMD. In this manuscript, we present a novel, non-invasive means that utilizes synchronized acoustic radiation force optical coherence elastography (ARF-OCE) to measure and estimate the elasticity of cadaver porcine retina. Both regions near the optic nerve and in the peripheral retina were studied. An acoustic force is exerted on the tissue for excitation and the resulting tissue vibrations, often in the nanometer scale, are detected with high-resolution optical methods. Segmentation has been performed to isolate individual layers and the Young’s modulus has been estimated for each. The results have been successfully compared and mapped to corresponding histological results using H&E staining. Finally, 64 elastograms of the retina were analyzed, as well as the elastic properties, with stiffness ranging from 1.3 to 25.9 kPa in the ganglion to the photoreceptor sides respectively. ARF-OCE allows for the elasticity mapping of anatomical retinal layers. This imaging approach needs further evaluation but has the potential to allow physicians to gain a better understanding of the elasticity of retinal layers in retinal diseases such as AMD.
