Contemporary Isolated Bearing Exchange for the Management of Hip Instability Following Primary Total Hip Arthroplasty

BackgroundIsolated ball and liner exchanges (IBLEs) can be performed to increase hip joint stability, but historical results have been mixed due to a lack of head size options or dual mobility articulations. The purpose of this study was to evaluate the contemporary results of IBLEs in patients who have instability following primary total hip arthroplasty (THA). MethodsWe retrospectively reviewed 65 primary THAs from 2016 to 2020 with hip instability undergoing IBLE or conversion to dual mobility articulation. There were 31 men and 34 women who had an average age of 70 years (range, 26 to 92). The mean time to revision from primary was 40.1 months (range, 1 to 120). In 52 cases, IBLE was performed using conventional bearings, while 13 hips were converted to dual mobility. Radiographic factors, including acetabular component orientation, reproduction of hip joint offset, leg lengths, and outcomes such as recurrent instability requiring subsequent revision and patient-reported outcome measure, were recorded and compared. ResultsThere were 12 (18.4%) hips that experienced subsequent instability and required another revision (17.3% ball and liner exchange versus 23.1% dual mobility articulation, P = .615). The mean time to rerevision for instability was 17.1 months. There were no significant differences in either acetabular component anteversion (P = .25) or restoration of hip joint offset (P = .87) in patients who required another revision for instability compared to those who did not, respectively. At 1 year, patients undergoing conventional bearing exchange reported higher Hip Dysfunction Osteoarthritis Outcome Score for Joint Replacements (P = .002) and Veterans Rand physical component (P = .023) scores compared to those who underwent a conversion to dual mobility articulation. Only age > 75 years at the time of surgery was associated with increased risk for dislocation (odds ratio 7.2, confidence interval 1.2 to 43.7, P = .032). ConclusionsIsolated bearing exchanges for instability following THA remained at high risk for subsequent instability. Conversion to dual mobility articulations did not reduce the risk of reoperation.

Is the Acetabular Cup Orientation Different in Robot-Assisted and Conventional Total Hip Arthroplasty With Right-Handed Surgeons Using an Anterolateral Approach?

Introduction Total hip arthroplasty (THA) is one of the most successful orthopaedic procedures. Survival rates from 90% at 10 years to 93% at 20 years have been reported in different studies. Differences in implant and patient characteristics can undoubtedly explain some of this variability observed in prosthesis durability, but the effect of surgical technique and implant orientation cannot be ignored. Therefore, many intraoperative methods (anatomic landmarks, intraoperative x-ray, fluoroscopy, navigation, and robotic surgery) have been attempted to avoid acetabular component malpositioning.Although postoperative computed tomography (CT) is accepted as the gold standard for the measurement of acetabular anteversion, it remains controversial in respect of costs and radiation exposure. The aim of this study was to examine how acetabular component orientation was affected in robotic and conventional THA operations performed by two surgeons with right-hand dominance. Material and methods The study included 113 primary THA operations performed on 113 patients between 2017 and 2022 in two groups: (i) robotic THA (Mako, Stryker Corporation, Kalamazoo, Michigan, United States) (55 patients) and (ii) conventional THA (58 patients). The patients comprised 51 males and 62 females. THA was performed on 54 right-side hips and 59 left-side hips. The operations were performed by two orthopaedic surgeons, each with 20 years of arthroplasty experience, on all the patients in the lateral decubitus position with an anterolateral approach. In all the cases, the orientation of the acetabular component was 40° inclination and 20° anteversion. Difficult THA procedures (patients with developmental dysplasia of the hip (DDH), a history of hip surgery, revision THA, defect or deformity of the acetabulum, a history of scoliosis or lumbar posterior surgery, or those requiring proximal femoral osteotomy) were excluded from the study. Using the Liaw and Lewinnek methods, the acetabular component anteversion was measured on the radiographs taken in the optimal position postoperatively and the acetabular cup inclination angles were measured on the pelvis radiographs. The groups were compared using the Kolmogorov-Smirnov, Pearson Chi-square and Mann-Whitney U statistical tests. The limits were accepted as 40±5° for inclination and 20±5° for anteversion. Results No statistically significant difference was determined between the groups in respect of age, gender, or operated side. No statistically significant difference was determined between the optimal acetabular cup inclination angles of the robotic and conventional THA groups (p = 0.79). No statistically significant difference was determined between the optimal acetabular cup anteversion angles of the left and right conventional THA groups. Statistically significantly better results were determined in the robotic group in respect of acetabular cup anteversion (p<0,001). Conclusion The optimal orientation of the acetabular component is a key factor for successful THA. Otherwise, revision surgery is inevitable for reasons such as instability, impingement, or increased wear. The results of this study demonstrated that robotic surgery was superior to the conventional method in the placement of the acetabular component in the desired orientation.

Application of transverse acetabular ligament in total hip arthroplasty: a systematic review

IntroductionIn total hip arthroplasty (THA), the correct position of the acetabular component directly determines the outcome of the surgery, or the success of the surgery. Therefore, how to accurately locate the position of the acetabular component has become a very critical step in THA. As an important anatomical structure of the hip joint, the transverse acetabular ligament (TAL) is helpful for acetabular component orientation in THA. The aim of this systematic review was to investigate application of TAL in THA.Materials and methodsA systematic literature search of PUBMED, EMBASE, and Cochrane Library was performed (January and February 2023) using keywords “total hip arthroplasty,” “total hip replacement,” “total hip replacements,” “total hip arthroplasties,” “total hip prosthesis,” and “transverse acetabular ligament” in all possible combinations. Reference lists of included articles were reviewed. Study design, surgical approach, patient demographics, TAL identification rate, appearance of the TAL, anteversion and inclination angle and rate of dislocations were recorded.ResultsIn total, 19 studies met the screening criteria. Study designs were prospective cohorts (42%), retrospective cohorts (32%), Case series (21%), and randomized controlled trial (5%). Twelve of the 19 (63.2%) studies investigated the application of TAL as an anatomical landmark for locating acetabular component position in THA. Analysis revealed that TAL is a reliable anatomical landmark for acetabular component orientation within the safe zone in THA.ConclusionsTAL can reliably be used to align the acetabular component in the safe zone for anteversion and inclination in THA. However, TAL has individual variation influenced by some risk factors. More randomized controlled studies with larger numbers of patients are needed to investigate the precision and accuracy of TAL as an intraoperative landmark in THA.Level of evidenceIV.

POSITIONING OF ACETABULAR AND FEMORAL COMPONENT IN PRIMARY TOTAL HIP ARTHROPLASTY USING CT EVALUATED PATIENT NATIVE ANATOMY

In total hip arthroplasty, component orientation is an important task for stability. Acetabular and femoral component orientation is the key factor for outcome of total hip arthroplasty. The aim of this study is to assess the clinical and radiological effectiveness of positioning of acetabulum and femoral components in primary total hip replacement using CT evaluated patient native anatomy. Subjects of this study were 36 patients with nonunion neck of femur fractures or arthritic hip based on inclusion and exclusion criteria and treated with cemented or uncemented total hip arthroplasty. Pre operative CT scan with 3D reconstruction was done for all patients and the acetabular inclination and version was measured in coronal and axial planes. Femoral anteversion was measured in mid axial image. Transverse acetabular notch (TAN) and anterior acetabular notch (AAN) were identied intra operatively to reproduce the pre operative angles. Post operatively CT scan was done and all angles were measured as before. All patients were followed up and analyzed using Harris Hip Score. The mean preoperative Harris Hip Score is 34.5. The mean postoperative Harris Hip Score is 88.19. Mean acetabular cup inclination is 39.80°, Mean cup anteversion is 15.1° and mean femoral anteversion is 14.75°. No Dislocation was noted during follow up. No hips show clinical and radiological evidence of femoral and acetabular component loosening. All Postoperative CT measurements of angles are within the safe zone by Lewinnek et al. Preoperative CT guided parameters, intra operative anatomical landmarks (TAN&amp;AAN) and the digital protractor device can aid better positioning of the acetabular and femoral component of THA and is independent of the position of the patient. It also provides a precise planning for component placement resulting in less intra operative time thereby reducing the chances of infection and decreases the dislocation rate and wear and tear and improves the functional outcome

Three-Dimensional Morphometry of the Canine Pelvis: Implications for Total Hip Replacement Surgery.

Two-dimensional measurements of acetabular geometry are widely used for the assessment of acetabular component orientation following total hip replacement (THR). With the increasing availability of computed tomography scans, there is an opportunity to develop three-dimensional (3D) planning to improve surgical accuracy. The aim of this study was to validate a 3D workflow for measuring angles of lateral opening (ALO) and version, and to establish reference values for dogs. Pelvic computed tomography scans were obtained from 27 skeletally mature dogs with no radiographic evidence of hip joint pathology. Patient-specific 3D models were built, and ALO and version angles were measured for both acetabula. The validity of the technique was determined by calculating intra-observer coefficient of variation (CV, %). Reference ranges were calculated and data from left and right hemipelves were compared using a paired t-test and symmetry index. Measurements of acetabular geometry were highly repeatable (intra-observer CV 3.5-5.2%, inter-observer CV 3.3-5.2%). Mean (± standard deviation) values for ALO and version angle were 42.9 degrees (± 4.0 degrees) and 27.2 degrees (± 5.3 degrees) respectively. Left-right measurements from the same dog were symmetrical (symmetry index 6.8 to 11.1%) and not significantly different. Mean values of acetabular alignment were broadly similar to clinical THR guidelines (ALO of 45 degrees, version angle of 15-25 degrees), but the wide variation in angle measurements highlights the potential need for patient-specific planning to reduce the risk of complications such as luxation.

Computer-Assisted Navigation Accurately Delivers Preoperative Targets for Acetabular Component Orientation During Direct Anterior Approach Total Hip Arthroplasty.

Despite its success, total hip arthroplasty (THA) remains associated with potentially significant complications associated with component malposition. Preoperative planning can mitigate some of these potential concerns; however, the accurate intraoperative delivery of preoperative targets can be challenging. Computer-assisted navigation may assist with intraoperative target delivery, although the integration of these two technologies is relatively uncommon. We retrospectively reviewed cases of THA planned with a computed tomography-based preoperative planning software and performed with the use of an imageless, computer-assisted navigation system. Postoperative acetabular component orientation from radiographs was compared with preoperative targets and intraoperative navigation measurements. A total of 76 patients were included in the analysis. The mean anteversion target (20.0°±3.1°) did not differ significantly from the mean intraoperative navigation measurement (20.5°±3.3°; P=.30; mean difference, 2.2°±2.3°). The mean radiographic measurement (26.6°±6.5°) differed from the target by a mean of 7.5°±6.1° (P<.001). The mean inclination target (38.4°±1.9°) did not differ significantly from the mean intra-operative measurement (38.0°±1.5°; P=.20; mean difference, 1.3°±1.7°) but differed from the radiographic measurement by a mean of 5.2°±4.2° (41.8°±5.6°; P<.001). No adverse events were reported in the 90-day period following the index procedure. Our study demonstrated that an imageless navigation system can accurately deliver computed tomography-derived preoperative targets for acetabular component orientation. Differences noted on radiographs may be due to the difference in patient positioning for the postoperative imaging (standing) as compared with preoperative imaging or surgery itself (supine). Cup placement in primary total hip arthroplasty: how to get it right without navigation or robotics.

Acetabular component orientation and position are important factors in the short- and long-term outcomes of total hip arthroplasty.Different definitions of inclination and anteversion are used in the orthopaedic literature and surgeons should be aware of these differences and understand their relationships.There is no universal safe zone.Preoperative planning should be used to determine the optimum position and orientation of the cup and assess spinopelvic characteristics to adjust cup orientation accordingly.A peripheral reaming technique leads to a more accurate restoration of the centre of rotation with less variability compared with a standard reaming technique.Several intraoperative landmarks can be used to control the version of the cup, the most commonly used and studied is the transverse acetabular ligament.The use of an inclinometer reduces the variability associated with the use of freehand or mechanical alignment guides.

Differences in the setting of acetabular component alignment guides between the supine and lateral positions for total hip arthroplasty

The acetabular component orientation in total hip arthroplasty is of critical importance to clinical results. Although navigation systems and surgical robots have been introduced, most surgeons still use acetabular component alignment guides. This study aimed to compare the accuracy between modern acetabular component alignment guides for the lateral position and those for the supine position. Thirteen alignment guides for the lateral position and 10 for the supine position were investigated. All the lateral position alignment guides indicated cup alignment in operative definition, and the supine position alignment guides indicated cup alignment in radiographic definition. For lateral position alignment guides, the anteversion actually indicated by the alignment guide itself was smaller than that indicated by the manufacturer by a mean of 6° (maximum, 9°), and the inclination actually indicated by alignment guides themselves was larger than that by the manufacturer (p < 0.01) by a mean of 2° (maximum, 4°). For supine position alignment guides, the inclination and anteversion indicated by the alignment guide itself were identical with those indicated by the manufacturer. The current study showed that the angles actually indicated and those stated by manufacturers were not identical for lateral position alignment guides.

Mechanical and Anatomical Alignment Guide Techniques Are Superior to Freehand in Achieving Target Orientation of an Acetabular Component.

BackgroundAchieving accurate and consistent acetabular component orientation remains a major challenge in total hip arthroplasty.MethodsWe used a pelvic model to compare freehand techniques vs mechanical and anatomical alignment guides in achieving a target operative inclination (OI) and operative anteversion (OA). Thirty subjects comprising consultant orthopedic surgeons, orthopedic trainees, and nonsurgical staff positioned an acetabular component in a pelvic model using 3 different methods for guiding inclination and another 3 for guiding version.ResultsUsing either a standard mechanical alignment guide (MAG) or a spirit level MAG technique eliminated outliers from target OI, while the freehand method resulted in 46.7% of measurements outside the OI target range. The spirit level MAG technique significantly outperformed the standard MAG technique in median unsigned deviation from target OI (0.8° vs 2.1°, P < .001). Either method of referencing the transverse acetabular ligament for version yielded lower deviations from target OA than the freehand method and fewer outliers from the ±5° target range. Surgical experience was not a significant factor for accurately achieving target OI and OA.ConclusionsEven in an idealized in vitro model, a wide range of OI and OA is seen with the freehand technique of cup placement by subjects of all levels of surgical experience. Using either a standard MAG or a spirit level MAG reduces deviations in target OI, with the spirit level MAG method yielding the best accuracy. Using the transverse acetabular ligament to guide cup anteversion yields more accurate OA.

Well-Placed Acetabular Component Oriented Outside the Safe Zone During Weight-Bearing Daily Activities.

Background: A comprehensive and thorough understanding of functional acetabular component orientation is essential for optimizing the clinical outcome after total hip arthroplasty (THA). This study aimed to quantify the functional acetabular anteversion and inclination of unilateral THA patients during walking and static standing and to determine whether the functional acetabular orientation falls within the Lewinnek safe zone.Methods: Seventeen patients with unilateral THA received a CT scan and dual fluoroscopic imaging during level walking and static standing to evaluate in vivo hip kinematics. The pelvic functional coordinate system of the 3D CT-based computer model was defined by the line of gravity and anterior pelvic plane (APP) to measure functional acetabular anteversion and inclination in different postures. The Lewinnek safe zone was used to determine the acetabular malposition during functional activities.Results: The THA side demonstrated an average of 10.1° (± 9.6°, range –7.5° to 29.9°) larger functional anteversion and 16.0° (± 9.2°, range –7.2° to 29.9°) smaller inclination than native hips during level walking. Functional acetabular anteversion in the THA side during level walking and static standing was significantly larger than anatomical measurements (p < 0.05). Acetabular orientation of most well-placed THA components anatomically in the Lewinnek safe zone fell outside the safe zone during more than half of the gait cycle and static standing.Conclusion: The current study revealed that an anatomically well-placed acetabular cup does not guarantee a well-functional orientation during daily activities. The in vivo mechanical performance and loading conditions of the THA component during other weight-bearing activities should be investigated in further studies.

Direct Anterior Total Hip Arthroplasty with Fluoroscopy to Mitigate the Risk of Complications in Patients with Legg–Calve–Perthes Disease

AbstractA minority of adult patients with childhood Legg–Calve–Perthes disease (LCPD) will develop secondary arthritis with marked leg length discrepancy (LLD) and multiplanar hip deformity. During total hip arthroplasty (THA), these technical challenges increase the perioperative risks of nerve injury, leg length inequality, and implant malposition. The technique of direct anterior total hip arthroplasty (DA-THA) in combination with fluoroscopic imaging was evaluated to mitigate these risks. We performed a retrospective review of 11 DA-THA procedures performed for patients with LCPD. The mean preoperative LLD for the patient cohort was 17 mm (range, 2–54). The mean postoperative LLD was 6 mm (range, 0–28). Acetabular component orientation was precise and accurate with a mean abduction angle of 44 degrees (range, 42–46) and mean anteversion of 20 degrees (range, 16–24). Clinical outcomes demonstrated a mean hip disability and osteoarthritis outcome score for joint replacement (HOOS, Jr) of 94 points. No patients had leg lengthening more than 26 mm and no nerve palsies were identified. We conclude that DA-THA with fluoroscopic guidance may be a valuable method to improve component placement precision and procedural safety in this potentially high-risk patient group.

Impact of Cementless Zweymüller Stem Anteversion on Resistance to Periprosthetic Fracture in Total Hip Arthroplasty

SUMMARYTotal hip arthroplasty implies proper orientation of both acetabular and femoral components with a range of 25-40° of combined anteversion. The aim of the study was to examine resistance to periprosthetic fracture of the axially loaded cross section rectangular femoral stem (Zweymüller) with respect to different degrees of anteversion, implanted in the artificial bone model, in laboratory conditions. Femoral bone models with implanted femoral stems were divided into 3 groups depending on the degree of stem anteversion (A, control group 13-17°; B, stem retroverted 0°-4°; and C, stem anteverted 26-30°). The amount of axial load leading to periprosthetic fracture (PPFx) of the artificial bone model was determined experimentally for each construct. The results showed that the load at which the PPFx occurred significantly increased with the increase of the endoprosthesis anteversion angle. In our clinical practice, we are often unable to place the acetabular component in an ideal grade of anteversion for intraoperatively determined reasons. The results of this experimental study suggested that increasing rectangular femoral (Zweymüller) stem anteversion lowered the risk of PPFx. This study was limited by experimental design (laboratory conditions, artificial bone) and should be clinically verified.

Lewinnek Safe Zone References are Frequently Misquoted.

BackgroundOptimal acetabular component orientation in total hip arthroplasty (THA) is a necessity in achieving a stable implant. Although there has been considerable debate in the literature concerning the safe zone, to date, there has not been any review to determine if these references are consistent with the definition applied by Lewinnek et al. in 1978. Therefore, this article aims to examine the available literature in the PubMed database to determine how often a correct reference to the safe zone as defined by Lewinnek was applied to discussions regarding THA.MethodsA search for literature in the PubMed database was performed for articles from 1978 to 2019. Search criteria included terms ‘Lewinnek,’ ‘safe zone,’ and ‘total hip arthroplasty.’ Exclusions included abstract-only articles, non-English articles, articles unrelated to THA, and those lacking full content.ResultsA review of literature yielded 147 articles for inclusion. Overall, only 11% (17) cited the Lewinnek article correctly. Forty-five percent (66) of articles referenced measurements in the supine position, 18% (26) referenced other positions, and 37% (55) did not specify. Nineteen percent (28) reported measurements of the acetabular cup orthogonal to the anterior pelvic plane, while 73% (108) did not, and 7% (11) did not specify. Twenty-three percent (34) measured from computed tomography scans instead of other methods.ConclusionsIn the discussion of the safe zone regarding THA, only 11% of articles listed are consistent with the definition established by Lewinnek. This warrants further investigation into a consistent application of the term and its implications for THA implant stability and dislocation rates.

The Direct Anterior Approach Total Hip Arthroplasty Reliably Achieves "Safe Zones" for Combined Anteversion.

BackgroundIn total hip arthroplasty (THA), component position is critical to avoid instability and improve longevity. Appropriate combined femoral and acetabular component anteversion is important for improved THA stability and increased impingement-free range of motion. In direct anterior THA (DA-THA), concern has been expressed regarding the accuracy of femoral component positioning. This study seeks to quantify acetabular, femoral, and combined component orientation relative to the accepted “safe zones” in patients who have undergone DA-THA.MethodsTwenty-nine patients who had THA performed via direct anterior approach had postoperative computerized tomography scans done to assess femoral anteversion. Stem rotational alignment was measured relative to the transepicondylar axis (TEA) and the posterior condylar axis (PCA) of the femur at the knee. Acetabular abduction and version were recorded on anteroposterior pelvis radiographs.ResultsThe mean stem anteversion was 17.5° (standard deviation = 10.8°) from the TEA and 21.7° (standard deviation = 11.3°) from the PCA. Ten of 30 cups were appropriately anteverted; however, all the cups had appropriate abduction. Combined version when using the TEA resulted in 79% (23/29) of patients within the “safe zone” of 25°-50°. Pearson correlation coefficients were high for both stem anteversion from the TEA (R = 0.96) and PCA (R = 0.98); however, interobserver reliability for combined component anteversion was greater for the TEA (kappa, 0.83 vs 0.65).ConclusionsCombined anteversion within the “safe zone” was achieved 79% of the time with DA-THA. Interestingly, most of the “excessive” combined anteversion appears to be related to increased anteversion of the acetabular component with only 10 patients within the acetabular cup “safe zone” of 5°-25°.

Digitalized analyses of intraoperative acetabular component position using image-matching technique in total hip arthroplasty.

AimsAppropriate acetabular component placement has been proposed for prevention of postoperative dislocation in total hip arthroplasty (THA). Manual placements often cause outliers in spite of attempts to insert the component within the intended safe zone; therefore, some surgeons routinely evaluate intraoperative pelvic radiographs to exclude excessive acetabular component malposition. However, their evaluation is often ambiguous in case of the tilted or rotated pelvic position. The purpose of this study was to develop the computational analysis to digitalize the acetabular component orientation regardless of the pelvic tilt or rotation.MethodsIntraoperative pelvic radiographs of 50 patients who underwent THA were collected retrospectively. The 3D pelvic bone model and the acetabular component were image-matched to the intraoperative pelvic radiograph. The radiological anteversion (RA) and radiological inclination (RI) of the acetabular component were calculated and those measurement errors from the postoperative CT data were compared relative to those of the 2D measurements. In addition, the intra- and interobserver differences of the image-matching analysis were evaluated.ResultsMean measurement errors of the image-matching analyses were significantly small (2.5° (SD 1.4°) and 0.1° (SD 0.9°) in the RA and RI, respectively) relative to those of the 2D measurements. Intra- and interobserver differences were similarly small from the clinical perspective.ConclusionWe have developed a computational analysis of acetabular component orientation using an image-matching technique with small measurement errors compared to visual evaluations regardless of the pelvic tilt or rotation.Cite this article: Bone Joint Res 2020;9(7):360–367.

Characterising acetabular component orientation with pelvic motion during total hip arthroplasty.

Suboptimal acetabular component position can result in impingement, dislocation, and accelerated wear. Intraoperative pelvic motion has led to surgeon error and acetabular cup malposition. This study characterises the relationship between pelvic rotation and postoperative acetabular cup orientation. A device was constructed to allow cadaveric pelvis rotation along three axes about an acetabular cup in fixed orientation. The acetabular cup was fixed in space at 40° of radiographic inclination and 15° of anteversion relative to the anterior pelvic plane to represent consistent surgeon intraoperative placement. Active marker clusters were fixed to surgical equipment while the cadaveric pelvis was cemented with passive reflective markers, both identified with the Optotrak Certus motion capture system. The reamed cadaveric pelvis was rotated along three axes from -45° to 45° of roll, -30° to 30° of tilt, and -35° to 35° of pitch. The change in component inclination and anteversion was recorded at each 5° interval. Using computed tomography 3D reconstruction, the experimental setup was duplicated computationally to assess against a greater range of pelvis and implant sizes. Radiographic anteversion and inclination showed a non-linear relationship dependent on pelvic roll, tilt, and pitch. Radiographic anteversion changed -0.59°, 0.76° and 0.01° while radiographic inclination changed 0.23°, 0.18° and 1.00° for every 1° of pelvic roll, tilt and pitch, respectively. Computationally, anteversion changed -0.61°, 0.75° and 0.00° while inclination changed 0.22°, 0.19° and 1.00° for every 1° of pelvic roll, tilt and pitch, respectively. These results were independent of cup and pelvis size. Intraoperative pelvic motion can significantly affect final cup position, and this should be accounted for when placing acetabular components during total hip arthroplasty. Based on this study, intraoperative adjustment of the acetabular component position based on pelvis motion may be implemented to improve postoperative component position.

Sagittal Pelvic Kinematics in Hip Arthroplasty.

The orientation ofthe spine relative to the pelvis-particularly that in the sagittal plane-has been shown in both kinematic and radiographic studies to be paramount in governance of acetabular alignment during normal bodily motion. The purpose of this review is to better understand the challenges faced by arthroplasty surgeons in treating patients that have concurrent lumbar disease and are therefore more likely to have poorer clinical outcomes after THA than in patients without disease. The concept of an "acetabular safe zone" has been well described in the past regarding the appropriate orientation of acetabular component in THA. However, this concept is now under scrutiny, and rising forth is a concept of functional acetabular orientation that is based on clinically evaluable factors that are patient and motion specific. The interplay between the functional position of the acetabulum and the lumbar spine is complex. The challenges that are thereby faced by arthroplasty surgeons in terms of proper acetabular cup positioning when treating patients with concomitant lumbar disease need to be better understood and studied, so as to prevent catastrophic and costly complications such as periprosthetic joint dislocations and revision surgeries.

An accelerometer-based navigation system provides acetabular cup orientation accuracy comparable to that of computed tomography-based navigation during total hip arthroplasty in the supine position

BackgroundInadequate acetabular component orientation is associated with postoperative impingement, dislocation, and accelerated polyethylene wear. Computed tomography (CT)-based navigation systems provide accuracy for total hip arthroplasty (THA) but are not available in all facilities. Accelerometer-based navigation systems are inexpensive, but their accuracy remains undetermined. This study compares the accuracy of cup orientation in THA using CT-based and accelerometer-based navigation systems.MethodsThis retrospective study included 35 consecutive patients (11 males, 24 females; mean age, 65 years) who underwent primary cementless THA via an anterolateral approach in the supine position. Both CT-based and accelerometer-based navigation systems were used simultaneously. The accuracy of cup orientation was compared between the two systems using postoperative CT.ResultsThe accuracy of cup inclination was 2.7° ± 2.0° in the CT-based group and 3.3° ± 2.4° in the accelerometer-based group. The accuracy of cup anteversion was 2.8° ± 2.6° in the CT-based group and 3.4° ± 2.2° in the accelerometer-based group. No significant difference was observed in cup inclination (p = 0.29) or cup anteversion (p = 0.34) between CT-based and accelerometer-based navigation.ConclusionsThe accuracy of cup positioning did not differ significantly between CT-based and accelerometer-based navigation systems.
