EP38.05: The accuracy of ultrasound estimated fetal weight at 41 weeks of gestation prior to induction of labour: prospective study

To evaluate the accuracy of ultrasound estimated fetal weight (EFW) shortly before induction of labour in primiparous women induced by prologued gestation comparing accuracy of this measurement with third trimester ultrasound. Prospective study of 50 primiparous woman singleton pregnancies and cephalic presentation who were admitted to our unit for induction of labour due to prolonged gestation between April 2021 and December 2021. An ultrasound to estimated fetal weight 24h before induction was performed in all of them. Information of estimated fetal weight in third trimester was obtained from history records retrospectively. Both EFW were calculated using Hadlock formula and compared with birthweights. There was a high correlation between EFW at 41 weeks prior to labour induction and birthweight (R2 = 0.737, P < 0.001). EFW at third trimester ultrasound have not a good correlation with birthweight (R2 = 0.210, P > 0,143). The mean birthweight was 3469 ± 392 g. The mean EFW at 41 weeks was 4482 ± 414 g and mean absolute weight difference was −160 ± 293 g. The mean EFW in third trimester ultrasound was 2658 ± 262 g and mean absolute weight difference was 810 ± 423 g. Ultrasound EFW before induction of labour at 41 weeks is highly correlated with birthweight. Although, this trend used to overestimate fetal weight, their accuracy is superior than third trimester measurement. EFW should modified the way of delivery or monitorisation under labour (suspected fetal growth restriction), for that reason, we must look for the major accuracy.

Evaluation of Pharmacologic Interventions for Weight Management in a Veteran Population.

Veterans are disproportionately impacted by weight-related morbidity: 40% of veterans are categorized as obese and an additional 38.5% are overweight. Medications are recommended as an adjunct to lifestyle and dietary changes. Guidelines recommend 7 weight management medications, including orlistat, liraglutide, phentermine, phentermine/topiramate, lorcaserin, and naltrexone/bupropion. A single-center, retrospective chart review was conducted for patients who started weight management medications at Veteran Health Indiana in Indianapolis. The primary outcomes included total weight loss and weight loss as a percentage of baseline weight at 3, 6, 12, and > 12 months of therapy. Secondary outcomes included weight loss of 5% from baseline, rate of successful weight maintenance after initial weight loss of 5% from baseline, adverse drug reaction monitoring, and use of weight management medications across clinics at this site. The absolute weight difference over 12 months of weight management medication therapy was 15.8 kg. At each time point, weight loss was found to be statistically significant when compared with baseline (P < .001). Average weight change was greatest with orlistat (-25.9 kg) and naltrexone/bupropion was associated with a gain of 2.1 kg over the duration of the study. A majority of the patients analyzed lost the guideline-recommended 5 to 10% from baseline while taking weight management medication. Weight management medications in a veteran population produced initial weight loss consistent with previous studies. However, there is room for improvement in follow-up strategies to promote greater weight maintenance after initial weight loss. Considering the high health care costs, personal burden, and potential long-term complications associated with obesity, efforts to promote continued development of programs that support weight management and maintenance are imperative.

A 90-Day Oral Toxicity Study of the Ethanol Extract from Eupatorium japonicum Thunb and Foeniculum vulgare in Rats.

Eupatorium japonicum Thunb and Foeniculum vulgare are two of the most widely used folk herbs and constituents in many traditional Chinese herbal formulas. Nonetheless, little toxicological and safety information associated with following daily repeated exposure is obtained according to previous research. The present study was performed to assess the toxicity of ethanol extract from Eupatorium japonicum Thunb and Foeniculum vulgare (EFE) in male rats administered by dietary oral gavage at target doses of 0.39, 0.78, and 1.56 g/kg body weight/day for 90 days. There were no significant adverse effects on clinical signs, body weight, food conversion efficiency, and vital hematological indices. However, some hematology and biochemical indices such as WCV, MCH, MCHC, LY, MPV, T-CHO, as well as TG revealed significant changes in Sprague–Dawley rats and organ weights in lung and spleen showed diminished in male rats. Necropsy and histopathology findings suggested that no significant differences in absolute weights were found in all organs except lung and spleen, and no treatment-related alteration was identified in any organs. All results obtained in the present study indicated that the proper use of EFE in traditional medicine at oral dosages up to 1.56 g/kg/day body weight may harbor no prolonged toxicity to rats. However, further studies of EFE are still necessary to assess its oral safety in patients.

Donor-Recipient Weight and Sex Mismatch and the Risk of Graft Loss in Renal Transplantation.

Relatively smaller kidney donor to recipient size is proposed to result in higher graft loss due to nephron underdosing and hyperfiltration injury, but the potentially additive effect of sex and weight mismatch has not been explored in detail. The purpose of this study was to determine if concurrent donor and recipient absolute weight and sex mismatch was associated with graft loss in a cohort of deceased donor kidney transplant recipients. The association of kidney donor and recipient absolute weight and sex difference with death-censored graft loss was explored using a cohort of United States deceased donor recipients between 2000 and 2014 through the Scientific Registry of Transplants Recipients. Donor-recipient sex pairings (male donor-male recipient; female donor-female recipient; male donor-female recipient; female donor-male recipient) were further stratified by donor and recipient absolute weight difference (>30 or 10-30 kg [donor<recipient; donor>recipient] or <10 kg [donor=recipient]) resulting in 20 weight and sex pairings. Time to death-censored graft loss for each pairing was evaluated using multivariable Cox proportional hazards models adjusting for donor, immunologic, surgical, and recipient predictors of graft loss compared with the reference group of male donor-male recipients with no weight mismatch (<10 kg difference). Of 115,124 kidney transplant recipients, 21,261 developed death-censored graft failure (median graft survival time was 3.8 years; quartile 1 to 3, 0.0 to 14.8 years). After multivariable adjustment, the highest relative hazards for graft failure were observed for female recipients of male donor kidneys and male recipients of female donor kidneys in situations where the recipient was >30 kg larger than donor (hazard ratio, 1.50; 95% confidence interval, 1.32 to 1.70; hazard ratio, 1.35; 95% confidence interval, 1.25 to 1.45, respectively). A concurrent mismatch in donor-recipient weight (donor<recipient) and donor-recipient sex is associated with a higher risk of death-censored graft loss in kidney transplantation.

ObjectivesOur aim was to evaluate the accuracy of estimated fetal weight (EFW) by ultrasound at due date and the factors that could affect it. MethodsWe performed a retrospective study of 233 patients in 2014. An ultrasound was performed at due date consultation around 41 weeks of amenorrhea by midwives sonographer. EFW was calculated using the Hadlock's formula with 3 parameters (biparietal diameter, abdominal circumference and femur length) and then adjusted including the growth from the due date consultation to the day of delivery (25g/day) and finally compared to birth weight (BW). ResultsThe mean absolute weight difference between EFW adjusted and BW was 256g [0; 910]. The mean absolute percentage error was 7.2 % [0; 24.5] and the proportion of the EFW adjusted within 10 % of BW was 69.1 %. There was a strong correlation between EFW adjusted and BW (R=0.79). Obesity in early pregnancy or childbirth, excessive weight gain, the presence of oligoanamnios and fetal macrosomia had no influence on the estimated fetal weight. Indeed, the mean absolute percentage error of child who were macrosome and those were not, was similars (7.9 % vs 7.1 %, P=0.407). ConclusionsEFW by ultrasound at due date is performant. However, the adjustment by the effect growth does not improve accuracy. Fetal macrosomia do not decrease the accuracy of ultrasound to estimate the fetal weight at term.

Accuracy of self-reported weight and role of gender, body mass index, weight satisfaction, weighing behavior, and physical activity among rural college students

This study measured accuracy of self-reported body mass index (BMI), particularly weight, in a college population. The main purpose was to examine the role of gender, BMI, body weight satisfaction, weighing frequency and physical activity level in accuracy (weight difference, percent weight difference, and absolute weight difference). Students (N=405; 56% female) completed a questionnaire and 325 agreed to have their height and/or weight measured. Gender, BMI and activity level were significantly associated with weight difference and percent weight difference while BMI, activity level and weighing frequency were associated with absolute weight difference. However, interactions between BMI and physical activity were found. Our findings indicate that women and heavier individuals underestimate weight.

Objectives The aim of this study was to evaluate the accuracy of ultrasound fetal weight prediction at due date and to find parameters that may affect this prediction. Patients and methods We retrospectively studied 201 patients at due date in a university hospital in 2006, the fetal weight estimation being performed by Obstetric-gynecology (OB-Gyn). Estimated fetal weight was calculated with the Hadlock's formula, including biparietal diameter, cephalic circumference, abdominal perimeter and femoral length and was compared with birth weight. Results The mean birth weight was 3561 ± 415 g. The mean absolute weight difference was 261 ± 190 g (absolute range: 0 to 1183 g, actual range: −935 to 1183 g). Body mass index >30 kg/m 2 was associated with greater fetal weight inaccuracy ( p = 0,013). Fetal weight estimation was not influenced by fetal macrosomia, oligoanamnios or maternal weight gain during pregnancy. Discussion and conclusion The sonographic estimated fetal weight and birth weight are correlated with a mean absolute percentage error of 7%. However, clinicians should be aware of the risk of inaccuracy in obese women.

Accuracy of sonographically estimated fetal weight in 840 women with different pregnancy complications prior to induction of labor.

To evaluate the accuracy of sonographically estimated fetal weight (EFW) shortly before induction of labor in the presence of different pregnancy complications, and to define possible variables affecting it. The study sample consisted of 840 women with singleton pregnancies and cephalic presentation who were admitted to our unit for induction of labor between January 1999 and December 2000. All underwent detailed ultrasound assessment for EFW, amniotic fluid index, biophysical profile and placental location. Indications included previous Cesarean section, postdate pregnancy, pregnancy-induced hypertension, diabetic pregnancy, suspected large-for-gestational age (LGA) infants, suspected fetal growth restriction (FGR), oligohydramnios, decreased fetal movements, premature rupture of membranes at or before term. EFW was calculated after measuring fetal abdominal circumference and femur length. The EFW was compared with the weight at delivery, 1-3 days later. There was a high correlation between EFW and birth weight (R(2) = 0.775, P < 0.001). The mean birth weight was 3207 +/- 561 g, and mean absolute weight difference was 227 +/- 197 g; (absolute range, 0-1700 g; actual range, - 986 to + 1700 g). The mean weight difference was significantly different between the patients with LGA infants, FGR infants and preterm premature rupture of membranes (PPROM) (- 110 +/- 281 g, + 113 +/- 195 g and + 115 +/- 307 g, respectively, P < 0.01). Stepwise linear regression analysis of the effects of maternal and pregnancy characteristics on the weight difference yielded lower gestational age, higher birth weight, anterior placenta, higher gravidity, and younger maternal age as independent and significant variables associated with greater actual weight difference inaccuracy (R(2) = 0.099, P < 0.001), and higher birth weight as the only independent and significant variable associated with greater absolute weight difference (R(2) = 0.09, P = 0.018). The sonographic EFW is highly correlated with birth weight. However, clinicians should be aware of the risk of overestimation in pregnancies with suspected LGA and underestimation in pregnancies with PPROM and suspected FGR.
