CASE REPORT: SELF–EXPANDING TRANSCATHETER VALVE IMPLANTATION (ACURATE NEO 2) IN A VERY SMALL NATIVE AORTIC ANNULUS

Abstract We present the case of a 74–year–old female, symptomatic for dyspnea on exertion, evaluated by our Cardiac Team for severe aortic valve (AV) stenosis. The transesophageal echocardiography showed a stenotic tricuspid AV (valve area of 0.51 cm2, peak and mean gradients of 75 mmHg and 43 mmHg respectively), normal left ventricle (LV) ejection fraction without significant intraventricular gradient. Pre–operative thoraco–abdominal computed tomography angiography showed an AV without significant calcifications, an aortic annulus perimeter of 51.7 mm (area of 207,8 mm2) and a left ventricular outflow tract (LVOT) perimeter of 45.3 mm with elliptical shape (area of 149,9 mm2). The TAVR procedure was performed with an ‘off–label’ implantation of a SE THV with supra–annular location of leaflets (Acurate Neo 2 23 mm), without any predilation. The post–procedural transthoracic echocardiographic study highlighted the correct positioning of the bioprosthesis without significant protrusion in the LVOT and without any interference with the mitral valve apparatus. The main clinical, technical and procedural considerations have been: According to recommendations, the minimum perimeter of 56.5 mm and the minimum area of 254 mm2 represent the lower limits of the sizing guidelines for THV currently available. We used a SE valve with supra–annular location of the leaflets that offers a potential advantage over BE valves and intra annular SE valves, ensuring a larger effective orifice area. We avoided pre– and post–dilatation to reduce the risk of annular damage and/or rupture The precision of THV implantation (with minimal protrusion in LVOT) was maximized when the rapid pacing during phase 2 of the valve deployment was utilized. A short period of DAPT can be considered in order to prevent a potential increase of thromboembolic events secondary to small aortic valve area. In our opinion the main goal is to prevent high implantation and commissural misalignment in order to facilitate the subsequent coronary percutaneous interventions if needed. Even prophylactic coronary stenting (‘chimney fashion‘) should be avoided unless absolutely necessary. These technical considerations applies also to allow a subsequent valve–in–valve (VIV) procedure, if indicated. To our knowledge, this is among the smallest native aortic annulus so far described and treated percutaneously in a tricuspid stenotic aortic valve with a self–expanding (SE) THV Acurate Neo 2.

Rupture of sinus of Valsalva aneurysm in a young adult male: A case report

Rupture of the sinus of Valsalva aneurysm (SVA) is a surgical emergency. It can be fatal if not treated properly and on time. In this report, we present a 39-year-old male patient with no known disease who was admitted to the emergency department with complaints of pressure-such as chest pain, chest tightness, and an inability to breathe easily after a heavy meal at night. Thoracic and abdominal computed tomography angiography was performed to better evaluate the vascular structures; the ascending aorta diameter was 71 mm (wider than normal). Hemorrhagic fluid was observed in the pericardial space and paracardiac recesses. The preliminary diagnosis was a rupture of the right SVA, and emergency surgery was performed. The posterior aorta was found to have ruptured at the level of the right sinus of Valsalva, and anastomosis was performed on the torn portion. The patient was extubated 8 h after surgery. On the 7th post-operative day, transthoracic echocardiography was within normal limits, and he was discharged from the hospital on the 8th day in good condition with stable vital signs.

Postoperative delayed massive bleeding in gastric cancer: a case report

BackgroundPostoperative delayed bleeding of gastric cancer is a complication of radical gastrectomy with low incidence rate and high mortality.Case presentationThis case report presents the case of a 63-year-old female patient of Mongolian ethnicity who was diagnosed with gastric malignancy during a routine medical examination and underwent Billroth's I gastric resection in our department. However, on the 24th day after the surgery, she was readmitted due to sudden onset of hematemesis. Gastroscopy, abdominal CT, and digital subtraction angiography revealed postoperative anastomotic fistula, rupture of the duodenal artery, and bleeding from the abdominal aorta. The patient underwent three surgical interventions and two arterial embolizations. The patient’s condition stabilized, and she was discharged successfully.ConclusionCurrently, there are no specific guidelines for the diagnosis and treatment of pseudoaneurysms in the abdominal cavity resulting from gastric cancer surgery. Early digital subtraction angiography examination should be performed to assist in formulating treatment plans. Early diagnosis and treatment contribute to an improved overall success rate of rescue interventions.

Sonographic Features of Uterine Arteriovenous Malformation: A Case Series.

Uterine arteriovenous malformation (AVM) is very rare but potentially life-threatening. Early and accurate diagnosis is the cornerstone of its management. The objective of this study is to encourage sonographers to become familiar with a variety of grayscale sonographic features, facilitating rapid recognition of the patterns and prompting them to apply color flow Doppler for a diagnosis of uterine AVM and possible further investigations or interventions. We present six cases of uterine AVM presenting with abnormal uterine bleeding at varying degrees of severity, from abnormal menstruation to life-threatening bleeding following curettage. All initially provided some clues of uterine AVM upon grayscale ultrasound, leading to the application of color Doppler flow to support a diagnosis, with confirmation using abdominal computer tomography angiography (CTA) in most cases, resulting in definitive treatment using uterine artery embolization or other interventions. Most importantly, this study provides various sonographic features of uterine AVM, such as appearances of small tubular structures, spongy patterns, a conceptive-product-like appearance, and spaghetti-like patterns. Hopefully, familiarity with these sonographic features can facilitate practitioners to make an early diagnosis, leading to proper further investigation and intervention, and to prevent serious complications from potentially being caused by this subtle but very serious disorder.

Hepatic Artery Aneurysms in Infective Endocarditis: Report of 10 Cases and Literature Review

Hepatic artery aneurysms (HAAs), albeit rare in infective endocarditis (IE), are associated with a life-threatening morbidity. Retrospective review of 10 HAA-IE patients based on a total of 623 IE patients managed in 2 institutions (2008-2020) versus 35 literature cases. In our patient population, HAAs (10 males, mean age 48) were incidentally found during IE workup. All were asymptomatic. IE involved mitral (n=6), aortic (n=3), or mitral-aortic valve (n=1). Predisposing factors for IE were as follows: prosthetic valve (n=6), previous IE (n=2), IV drug user (n=1). Streptococcus species (spp.) were predominant (n=4), then staphylococcus spp (n=2) and E. faecalis (n=2). All patients presented associated lesions: infectious aneurysms (n=5), emboli (n=9), abscesses (n=5), and spondylitis/spondylodiscitis (n=2). HAA patterns on abdominal CT angiography (CTA) were solitary (70%), mean diameter 11.7mm (range 2-30), intrahepatic location (100%) involving the right HA in 9 out of 10 (90%) patients. In 2 patients, HAAs were complicated (rectorragia and hemobilia in 1, cholestasis in the other). Six patients underwent endovascular hepatic embolization (2 with multiple HAAs). Three HAA-IEs <15mm resolved under antibiotherapy on abdominal CTA follow-up. All patients underwent cardiac surgery. Late outcome was favorable in all followed patients (5/10). Literature review showed the preponderance of Streptococcus spp., of right lobe and intrahepatic HAA localization. Complications revealed HAAs in patients under antibiotic therapy and/or after cardiac surgery in 17 literature cases of delayed diagnosis. Abdominal CTA was pivotal in the initial IE workup. Small aneurysms (≤15mm) resolved under antibiotherapy. The usual treatment modality was HAA embolization and endovascular embolization before valve surgery was safe.

Popliteal artery aneurysmal dilatation by intimomedial mucoid degeneration causing emergency limb ischemia: a case report and literature review

ntimomedial mucoid degeneration (IMMD) is an extremely rare vascular condition characterized by mucin accumulation in the intima and media, resulting in arterial wall elastic tissue degradation and aneurysm formation. We report the case of a 34-year-old woman who presented to the emergency department with a 1-month history of left lower limb pain. On local physical examination, both lower limbs were warm with palpable regular pulses in the entire lower limbs and normally capillary refill. No palpable masses were felt. Computed tomography angiography (CTA) of the lower limbs revealed an aneurysm with a thrombosed sac in the left popliteal artery behind the knee joint. Thoracic and abdominal CTA revealed normal findings. The patient underwent posterior exploration of the left popliteal artery, which revealed a large aneurysm requiring excision, and an interposition vein graft end-to-end anastomosis was performed. Histopathological examination of the aneurysm disclosed the presence of fibrosis, scattered chronic inflammatory cells, and a hyalinized fibrin thrombus associated with marked fragmentation and distortion of the elastic tissue. Extensive mucoid deposition in the intima and media of the artery was also detected. The postoperative recovery was uneventful, and the patient was discharged home with regular follow-up during the last year in our outpatient clinic. IMMD can affect smaller vessels such the brachial, coronary, temporal arteries, and dorsalis pedis. The main histological hallmarks of IMMD include intimal and medial thickening caused by the formation of mucin pools, which leads to elastin fibers fragmentation and aggregation. The prognosis of IMMD-related aneurysms is determined by the extent of the disease and the time it is discovered. IMMD-related aneurysms should be included in the differential diagnosis of isolated popliteal artery aneurysms. Open surgical repair is the gold standard of treatment for IMMD-related aneurysms.

Splenic Artery Infectious Aneurysms in Infective Endocarditis – An Observational Study and Comprehensive Literature Review

PurposeTo determine the prevalence, the clinical and radiological features, associated factors, treatment and outcome of splenic artery aneurysms (SAAs) in infective endocarditis (IE). MethodsWe retrospectively reviewed 474 consecutive patients admitted to our institution with definite IE (2005-2020) ResultsSix patients had SAAs (1.3%; 3 women; mean age: 50). In all cases the diagnosis was obtained by abdominal CT-angiography (CTA). SAAs-IE were solitary, saccular with a mean diameter of 30mm (range: 10-90mm). SAAs-IE were intrasplenic (n=4) or hilar (n=2). Streptococcus spp were the predominant organisms (n=4).In all cases a left-sided native valve was involved (aortic, n=3; mitral, n=2, mitral-aortic, n=1). SAAs were silent in half-patients, and were revealed by abdominal pain (n=2) and by the resurgence of fever after cardiac surgery (n=1).All patients underwent emergent valve replacement. One patient died within 24h from multiorgan failure. For the others, uneventful coil embolization was performed in 4 patients after valve replacement (3 diagnosed early, one at 8 weeks). In the remaining patients, SAA-IE diagnosed at abdominal-CTA at day 16, with complete resolution under appropriate antibiotherapy alone. ConclusionSAAs-IE are a rare occurrence that may be clinically silent. SAAs-IE can be intrasplenic or hilar in location. Endovascular treatment in this context was safe. According to current guidelines, radiologic screening by abdominal-CTA allowed the detection of silent SAAs which could be managed by endovascular treatment to prevent rupture. The delayed formation of these SAAs could justify a CTA control at the end of antibiotherapy.

Segmentation of Portal Vein in Multiphase CTA Image Based on Unsupervised Domain Transfer and Pseudo Label.

Clinically, physicians diagnose portal vein diseases on abdominal CT angiography (CTA) images scanned in the hepatic arterial phase (H-phase), portal vein phase (P-phase) and equilibrium phase (E-phase) simultaneously. However, existing studies typically segment the portal vein on P-phase images without considering other phase images. We propose a method for segmenting portal veins on multiphase images based on unsupervised domain transfer and pseudo labels by using annotated P-phase images. Firstly, unsupervised domain transfer is performed to make the H-phase and E-phase images of the same patient approach the P-phase image in style, reducing the image differences caused by contrast media. Secondly, the H-phase (or E-phase) image and its style transferred image are input into the segmentation module together with the P-phase image. Under the constraints of pseudo labels, accurate prediction results are obtained. This method was evaluated on the multiphase CTA images of 169 patients. The portal vein segmented from the H-phase and E-phase images achieved DSC values of 0.76 and 0.86 and Jaccard values of 0.61 and 0.76, respectively. The method can automatically segment the portal vein on H-phase and E-phase images when only the portal vein on the P-phase CTA image is annotated, which greatly assists in clinical diagnosis.

Single-Stage Procedure of Transcatheter Aortic Valve Replacement and Endovascular Aneurysm Repair Under Local Anaesthesia and Percutaneous Access.

Abdominal aortic aneurysms (AAA) are observed in 6% of patients with concomitant aortic valve stenosis (AS) requiring aortic valve replacement. Optimal management of these concomitant pathologies is still debated. An 80-year-old man presented with acute heart failure due to a severe AS. Past medical history included AAA under regular surveillance. A thoracic and abdominal computed tomography angiography (CTA) confirmed a 6mm increase of AAA over an 8-month period (max 55mm). A multidisciplinary team prescribed a simultaneous endovascular approach of transcatheter aortic valve implantation (TAVI) followed by endovascular aneurysm repair (EVAR) under local anaesthesia with bilateral femoral percutaneous access. No intra or post-procedural complications were registered; technical success was confirmed by completion angiography and post-operative ultrasound. The patient was discharged on postoperative day 5. A 2-month post-operative CTA confirmed ongoing technical success. Combined TAVI and EVAR under local anaesthesia for AS and AAA was associated with reduced hospital stay and technical success at 2months from intervention in this case report.

Giant teratoma with isolated intestinal duplication in adult: A case report and review of literature

A combination of diseases is a rare phenomenon. Their clinical manifestations can vary, and the diagnosis can be challenging. Intestinal duplication is a rare congenital malformation, whereas retroperitoneal teratoma is a tumor in the retroperitoneal space, derived from the remaining embryonic tissue. There are relatively few clinical findings on adult retroperitoneal benign tumors. It is hard to believe that these two rare diseases can happen to the same person.A 19-year-old woman complaining of abdominal pain with nausea and vomiting was admitted. Abdominal computed tomography angiography was suggested for invasive teratoma. Intraoperative exploration revealed that the giant teratoma was connected to an isolated intestinal tract in the retroperitoneum. The postoperative pathological examination revealed that mature giant teratoma was present with intestinal duplication. This was a rare intraoperative finding that was successfully treated surgically.The clinical manifestations of intestinal duplication malformation are various, and difficult to diagnose before the operation. The possibility of intestinal replication should be considered when intraperitoneal cystic lesions are present.

P95 A RARE CASE OF INFECTIOUS AORTITIS

Abstract Aortitis is an inflammatory process affecting the arterial wall. There are infectious and non–infectious forms. While the non–infectious forms can be linked to arteritis of the large vessels, the infectious forms are a rare but fearful complication of infective endocarditis. In this case the most affected pathogenic species are gram positive bacteria such as streptococci and enterococci. An 81–year–old patient, hypertensive, with permanent atrial fibrillation, subjected to previous aortic valve replacement (with Intuity 27 mm bioprosthesis for severe stenosis in 2016) and with previous pulmonary embolism in 2021, was admitted to emergency department for oliguria and worsening ankle swelling in December 2022. The haemodynamic parameters at the hospital admission were stable. A transthoracic echocardiography was performed, which highlighted a vegetation of about 2 cm on the aortic bioprosthesis with mild stenosis and negligible intraprosthesis regurgitation. A cerebral and thoraco–abdominal CT angiography showed multiple thromboembolic outcomes involving the splanchnic vessels, kidney and spleen. The patient was then subjected to transesophageal echocardiography which highlighted the presence of vegetation on the bioprosthesis with diameters of 12x12 mm; moreover, aneurysmal dilatation of the aortic root was found with the presence, both anteriorly and posteriorly of the wall, of a thickening of about 1 cm which extended up to the sino–tubular junction. The patient‘s hospitalization was also complicated by intestinal subocclusion without urgent surgical indications.The patient‘s case was discussed collectively and it was decided, in the light of the high risk of intestinal ischemia (linked to the thromboembolic occlusion of the splanchnic vessels) not to subject the patient to cardiac surgery. In a recent publication of a case report with literature review, 12 cases were reported, all treated surgically, of which 2 died. Our patient, unable to undergo surgical therapy, represents the first case treated with medical therapy alone. To date, the patient is still hospitalized, in stable haemodynamic conditions, with a unfavorable long–term prognosis.

Peutz-Jeghers syndrome from the point of view of a radiologist and an endoscopist

INTRODUCTION: Peutz-Jeghers syndrome (PJS) is a rare (orphan) hereditary disease with a family history, primarily characterized by massive polyposis of the gastrointestinal tract and its complications. Every year, the syndrome is registered in 1 patient per 25,000–300,000 population. OBJECTIVE: To demonstrate the effectiveness of the combined use of diagnostic radiology and endoscopic imaging techniques in the diagnosis of this syndrome and its complications. A case report of a patient aged 47 years is given. A patient complained of persistent gastrointestinal bleeding, thrombosis of the arteries of the upper extremity, ventral hernia after the elimination of intestinal intussusception. A patient was admitted to the A. V. Vishnevsky NMRC of Surgery of the Ministry of Health of Russia (Moscow) to determine the tactics of treatment. Computed tomography of the abdominal organs and CT angiography of the upper extremities, esophagogastroduodenoscopy and colonoscopy were performed. CONCLUSION: The difficulties of the diagnosis and treatment of PJS and its associated complications were illustrated. A brief review of the literature (21 publications) by Russian and foreign authors on the problems of diagnosis, clinical manifestations and treatment of PJS were performed. According to the performed studies, total gastrointestinal polyposis, thrombosis of the arteries of the upper left limb, ventral postoperative hernia were revealed. Taking into account the ongoing complications of PJS, the patient was prescribed a gastrectomy.

Long-Term Propensity-Matched Comparison of Fenestrated Endovascular Aneurysm Repair and Open Surgical Repair of Complex Abdominal Aortic Aneurysms.

This study investigated the long-term outcomes of patients treated with fenestrated and branched endovascular aneurysm repair (F-BEVAR) or open surgical repair (OSR) for complex abdominal aortic aneurysms (c-AAAs). Complex abdominal aortic aneurysms are defined as aneurysms that involve the renal or mesenteric arteries and extend up to the level of the celiac axis or diaphragmatic hiatus but do not extend into the thoracic aorta. This study compares with a propensity-score matching the outcome of these procedures from 2 high-volume aortic centers. All patients with c-AAAs undergoing repair at 2 centers between January 2010 and June 2016 were included. The long-term imaging follow-up consisted in a yearly computed tomography angiography (CTA) in the F-BEVAR group. Yearly abdominal ultrasound examination and 5-year CTA were performed in the OSR group. The primary endpoints were long-term mortality, aneurysm-related mortality, and chronic renal decline (CRD), defined as estimated glomerular filtration rate reduction to <60 mL/min/1.73 m2 or >20%/de novo dependence on permanent dialysis in patients with normal or abnormal preoperative renal function, respectively. Secondary endpoints included aortic-related reinterventions, target vessel occlusion, proximal aorta degeneration, access-related complications, graft infection, and the composite endpoint of clinical failure during follow-up. After 1:1 propensity matching, 102 consecutive patients who underwent F-BEVAR and OSR, respectively, were included. The median follow-up was 67 months. There was no significant difference in long-term overall mortality (40.2% vs 36.3%; p=0.40) and aneurysm-related mortality (6.8% vs 5.8%; p=0.30), in the F-BEVAR and OSR groups, respectively. During follow-up, late renal function decline occurred in 27 (27.8%) versus 46 patients (47.4%) in the F-BEVAR and OSR groups, respectively (p<0.01). During follow-up, 23 reinterventions (23.5%) were performed in the F-BEVAR group, and 5 (5.1%) in the OSR group (p<0.01). No differences in overall and aneurysm-related mortality were observed. Chronic renal decline was significantly higher after OSR, while the reintervention rate was higher in the F-BEVAR group. These long-term results reflect the outcomes of a complex procedure performed by a single experienced operator in 2 high-volume centers, and followed with a strict surveillance imaging follow-up. Nowadays, F-BEVAR and OSR are considered two established techniques for the treatment of c-AAA. However, long-term comparative outcomes are not well studied, and concerns may rise in terms of durability of the repair, risk of reinterventions and late chronic renal decline. The present study showed, with a median follow-up > 5 years, no differences in overall and aneurysm-related mortality. Chronic renal decline was significantly higher after OSR, while the reintervention rate was higher in the endovascular group. To achieve the best possible long-term outcomes, both techniques should be performed in high volume aortic centres, tailored to the patient, and with an adequate surveillance imaging.

A Feasibility Study to Evaluate Replacing Conventional Computed Tomography at 120 KVP with Low Radiation Dose and Low Iodine Intake Based on Body Mass Index-Adapted Abdominal Computed Tomography Angiography in 291 Patients.

BACKGROUND This feasibility study aimed to evaluate replacing conventional computed tomography at 120 kVp with low radiation and low iodine dose based on body mass index (BMI)-adapted abdominal computed tomography angiography in 291 patients. MATERIAL AND METHODS A total of 291 abdominal CTA patients were divided into 3 individualized kVp groups according to their BMI: A1 with 70 kVp (n=57), A2 with 80 kVp (n=49), and A3 with 100 kVp (n=48); and 3 conventional 120 kVp groups: B1 (n=40), B2 (n=53), and B3 (n=44) BMI-matched with group A. The contrast media was 300 mgI/kg for group A and 500 mgI/kg for group B. The CT values and SD of the abdominal aorta and the erector spinae were measured, and the contrast-to-noise ratio (CNR) and figure-of-merit (FOM) were calculated. Imaging quality, radiation, and contrast media dosage were evaluated. RESULTS The CT and CNR of abdominal aorta in groups A1 and A2 were higher than those in groups B1 and B2 (P<0.05), but there was no significant difference between groups A3 and B3 (P>0.05). FOM of the abdominal aorta in group A was higher than that in group B (P<0.05). Compared with groups B1, B2, and B3, the radiation doses of A1, A2, and A3 groups decreased by 70.61%, 56.72%, and 31.87%, and contrast intake decreased by 39.94%, 38.74%, and 35.09%, respectively (P<0.05). CONCLUSIONS BMI-based individualized kVp abdominal CTA imaging significantly reduced overall radiation dose and contrast media intake while providing excellent image quality.

Optimal Virtual Monoenergetic Photon Energy (keV) for Photon-Counting-Detector Computed Tomography Angiography.

This study aimed to determine the optimal photon energy for virtual monoenergetic images (VMI) in computed tomography angiography (CTA) using photon-counting-detector (PCD) CT. Under institutional review board approval, 10 patients (abdominal, n = 4; lower extremity, n = 3; head and neck, n = 3) were scanned on an investigational PCD-CT (Count Plus, Siemens Healthcare) at 120 or 140 kV. All images were iteratively reconstructed with Bv48 kernel and 2-mm slice thickness. Axial and coronal VMI maximum-intensity projections were created in the range 40 to 65 keV (5-keV steps). Contrast-to-noise ratio (CNR) was calculated for major arteries in each VMI series. Two radiologists blindly ranked each VMI series for overall image quality and visualization of small vessels and pathology. The median and SD of scores for each photon energy were calculated. In addition, readers identified any VMIs that distinguished itself from others in terms of vessel/pathology visualization or artifacts. Mean iodine CNR was highest in 40-keV VMIs for all evaluated arteries. Across readers, the 50-keV VMI had the highest combined score (2.00 ± 1.11). Among different body parts, the 45-keV VMI was ranked highest for the head-and-neck (1.75 ± 0.68) and lower extremity (2.00 ± 1.41) CTA. Meanwhile, 50- and 55-keV VMIs were ranked highest for abdominal (2.50 ± 1.35 and 2.50 ± 1.56) CTA. The 40-keV VMI received the highest score for iodine visualization in vessels, and the 65-keV VMI for reduced metal/calcium-blooming artifacts. Quantitatively, VMIs at 40 keV had the highest CNR in major arterial vasculature using PCD-CTA. Based on radiologists' preference, the 45- and 50-keV VMIs were optimal for small body parts (eg, head and neck and lower extremity) and large body parts (eg, abdomen), respectively.

An unpredictable gastrointestinal bleed.

A 65-year-old male complained of persistent melena for 6 days, and displayed anemia symptoms without hematemesis, vomiting, and abdominal distention. He was diagnosed as ruptured aneurysm of aortic sinus Valsalva, and had received coronary artery occlusion 1 month ago. After the operation, he was continually prescribed clopidogrel 75 mg once daily. The laboratory examination showed blood hemoglobin concentration was 60 g/L without other conspicuous abnormality. Unfortunately, neither esophagogastroduodenoscopy (EGD) nor colonoscopy found no obvious bleeding lesions. And abdominal computed tomography angiography (CTA) and enhanced computed tomography (CT) showed no obvious abnormal findings. Moreover, capsule endoscopy revealed small intestinal with mucosal erosion (Figure 1A). After discontinued clopidogrel, blood transfusion, and support therapy, his symptoms was resolved with negative fecal occult blood, continued clopidogrel 75 mg once daily, and uneventfully discharged 1 week later.

Analysis of common iliac artery morphometry: A computed tomography angiography study

The present study was conducted to create reference values of common iliac arteries (CIA) for the diagnosis of vascular diseases such as aneurysm and atherosclerosis and to contribute to preoperative evaluation and the literature for open or endovascular interventions. Abdominal computed tomography angiography (CTA) images of 299 (156 F, 143 M) patients which were taken for various reasons at Gaziantep University Faculty of Medicine Hospital between 2015 and 2023 and which met the criteria were selected. The mean age of the patients included in the study was 49.85±18.74 years. Diameter measurements were made on two-dimensional images, while length and angle measurements were made on three-dimensional images. As a result of the analysis, the mean right CIA (RCIA) anteroposterior diameter was 1.12±0.66 cm and the mean transverse diameter was 1.21±0.74 cm; while the mean left CIA (LCIA) value was 1.12±0.68 cm in the anteroposterior axis and 1.13±0.73 cm in the transverse axis. In terms of length measurements, the mean right common iliac artery (RCIA) was found as 5.63±1.61 cm and the mean LCIA was found as 6.19±1.65 cm; in angle measurements of right and left CIA with the abdominal aorta (AA), mean RCIA was found as 23.3±9.84° and mean LCIA was found as 19.66±9.18°. While a significant difference was found between genders in terms of diameter values (p&lt;0.05), no difference was found in length and angle values (p&gt;0.05). In terms of correlations with age, correlation was found in diameter values of both sides and RCIA angle. It is thought that morphometric measurements of the common iliac arteries will contribute to interventional radiologists, vascular surgeons and the literature.

Abernethy Malformation: Possible Diagnosis for Patients with Congenital Heart Disease and Persistent Cyanosis.

Infant, nine months of age, female, diagnosed with congenital heart disease, with signs of heart failure associated with cyanosis and difficulty in gaining weight. Cardiomegaly with prevalence of pulmonary vascular network. Ectopic atrial rhythm with right ventricular overload and left anterosuperior divisional block. Single atrium with absent interatrial septum, atrioventricular connection with a single valve and two orifices, with increased pulmonary pressure and high Qp/Qs. Absence of portal vein and intrahepatic segment of the inferior vena cava. Infrahepatic portion continuing with the azygos system at the level of the thoracic cavity, presence of mesenteric-caval communication associated with signs suggestive of hepatic peribiliary fibrosis. Abernethy malformation is a rare condition and represents an extrahepatic portosystemic shunt that develops between the mesenteric-portal vasculature and the systemic veins. It may be associated with cardiac malformations and advance with pulmonary hypertension and even the need for liver transplantation. Persistent cyanosis after corrective surgery led to a deeper investigation and correct diagnosis of this malformation. Sternotomy with 68 minutes of cardiopulmonary bypass and nine minutes of total circulatory arrest. In the postoperative period, persistence of cyanosis was evident, even though there were no immediate complications. Patient was discharged on the 10th postoperative day. An abdominal computed tomography angiography confirmed the diagnosis of Abernethy type I malformation, and the patient was transferred for liver transplantation after congenital heart disease treatment.

Splenic hamartoma in a patient with peripancreatic arterial arcades: A case report

AbstractSplenic hamartoma is a rare benign vascular lesion of the spleen. A splenic mass was incidentally detected in an asymptomatic 65-year-old male during an abdominal ultrasound scan. The workup of the splenic lesion included an abdominal CT angiography (CTA), contrast-enhanced ultrasound (CEUS), and microvascular flow imaging Doppler sonography. The CT scan confirmed that the splenic mass had benign characteristics, and its contrast enhancement was similar to the spleen. The CEUS was able to rule out the possibility of hemangioma. Meanwhile, chronic compression of the celiac trunk and the filling of the splenic artery from collaterals were also detected on CTA. The imaging studies suggested a splenic hamartoma, and the diagnosis was confirmed by the result of a fine needle aspiration biopsy.

Clinical application of the contrast-enhancement boost technique in computed tomography angiography of the portal vein

PurposeThe aim of this study was to explore the improved image quality of the portal vein using the contrast-enhancement boost (CE-boost) technique for the improved visibility of abdominal-enhanced computed tomography (CT) scans in clinical practice.MethodsThis retrospective study included 50 patients in Group A who underwent routine abdominal-enhanced CT and 50 patients in Group B who underwent abdominal computed tomography angiography (CTA) with matched body mass index, age, and sex. Images in Group A were postprocessed with the CE-boost technique for further enhanced visibility of the portal vein. Both subjective and objective assessments of different branches of the portal vein in three types of images (i.e., Group A with CE-boost and without CE-boost, Group B) were statistically analyzed.ResultsThe subjective scores of two experienced radiologists showed good consistency (kappa value > 0.624, p < 0.001), and the score of Group A with CE-boost (mean, 4.64) was significantly higher than that of the others (p < 0.001). The liver parenchyma and most target veins in Group A with CE-boost showed the highest CT, signal-to-noise ratio (SNR), and contrast-to-noise ratio (CNR) values and the lowest standard deviation (SD), while the CNR of most portal veins in Group A without CE-boost had the lowest CNR (p < 0.001). There were no differences in the SNR of the portal vein in Group A without CE-Boost and Group B (p > 0.05).ConclusionCE-boost can significantly improve image quality in portal vein imaging without any additional scanning settings or changes in the clinical workflow.Graphical
