Three-dimensional deformation imaging to detect myocardial scar

Abstract Introduction Myocardial scar or fibrosis has significant implications in patient management and prognosis. Currently, cardiovascular magnetic resonance (CMR) is the reference technique for detection and quantification of scar though other imaging modalities have been proven to be useful as well. Purpose This study aims to investigate the usefulness of 3D strain echocardiography to identify myocardial scar in patients with ischaemic and non-ischaemic myocardial fibrosis using late gadolinium enhancement (LGE) CMR as a reference. Methods 149 individuals with ischaemic (89), non-ischaemic scar (26) and no scar (34) on LGE CMR underwent 3D transthoracic echocardiography. The 3D data sets were processed with the 4D LV analysis tool by TOMTEC Imaging Systems GmbH, Germany. The longitudinal, circumferential, radial, and principal tangential strain were calculated. Using a 16 LV-segment model the areas of LGE were assessed on CMR images, and a correlation between 3D strain values and LGE was investigated. Results A multivariate binary logistic regression model was built with the conditional backward elimination method using LGE as the dependent variable. The predictors made a significant contribution to the model (Wald test: 49.985, p<0.001). The Cox & Snell R-square was 0.141, and Nagelkerke R-square was 0.232, suggesting that the final model explains 14.1% or 23.2% of LGE variability. By logistic regression analysis, less negative peak principal tangential strain (P-PTS), peak circumferential strain (P-SC), and end-systolic circumferential strain (ES-CS) increased the odds, whereas increased peak radial strain (P-RS) decreased the likelihood of LGE (Table 1). The LGE extent was calculated as the average LGE transmurality of all 16 segments and was used as the dependent variable in linear regression models. The LGE extent was significantly correlated with all strain indices (Table 2). By stepwise linear regression analysis, P-PTS (B=0.029, 95%CI:0.019-0.040, P<0.001) outperformed the remaining strain variables in terms of linear association with LGE extent. Association between strain indices and the presence or absence of myocardial fibrosis in each of the 16 segments was tested by point biserial correlation (Table 3). PTS and CS indices were associated with myocardial fibrosis in 10/16 segments; RS indices were associated with LGE in 9/16 segments, and LS was not related to the presence of LGE in 15/16 segments. Septal (basal-mid-apical) and mid/apical anterior and inferior segmental strain showed more consistent associations with corresponding myocardial fibrosis. Conclusion 3D strain echocardiography is a promising and easily accessible technique, that can be used to identify areas of myocardial scar as it correlates well with LGE in many LV segments. Echocardiography is a bedside and widely available technique, and it could be used in patients with contraindications or no access to CMR or myocardial perfusion scan.Table 3

The correlation between left atrium voltage mapping and 3D late gadolinium enhancement cardiac magnetic resonance imaging findings in naive AF ablation candidate

Abstract Introduction The extent of the efficacy of late gadolinium enhancement cardiac MR (LGE-CMR) in depicting left atrial (LA) fibrosis in atrial fibrillation (AF) patients, compared to invasive Electroanatomical Mapping (EAM), is ongoing research field, while the highest degree of accuracy found when using ADAS program. Earlier studies focused mainly on post-ablation patients, employing segmentation (quality) and point-by-point (PBP) comparison methods. The non-invasive LGE-CMR-based fibrosis map shows promise for preprocedural planning and real-time guidance. Aim Evaluate LGE-CMR-based LA fibrosis precision in ablation-naïve AF patients using ADAS software, comparing it to the gold standard EAM using CARTO-3, in both qualitative and quantitative methods. Methods Patients planned for first CARTO-based AF ablation, undergoing pre-ablation LGE-CMR. 3D substrate atrial maps were generated via ADAS based on LGE-CMR (CMR-fib) and via CARTO as EAM. Normal range LA voltage was defined by Image Intensity Ratio of 1.2-1.32 on ADAS and by 0.2-0.5mV on Carto. Comparison methods, qualitative segmentation and PBP quantitative, with EAM serving as the gold standard. Results 24 naive patients included in the study (mean age 63.5 ± 10.9 yrs; 7 females, 15 paroxysmal AF, 17 maps during sinus). Segmental qualitative analysis (384 segments) showed 77% scarred segments, predominantly small scars (up to ⅓ segment). Compared with EAM, the CMR had 68.4% agreement, with 80.4% sensitivity, 78% PPV, 36.6% specificity, 32.6% NPV , kappa 0.139 AUC 0.567. Better agreement was found for dense scar. PBP quantitative comparison (41,345 points) demonstrated 50% scarred points, with 89% accuracy between EAM and CMR, with 49.6% sensitivity, 99.7% specificity, 99.5% PPV, and 73.5% NPV kappa 0.53, AUC 0.747. In both methods, agreement was worse for LA roof and best for AP and septal wall. Conclusion LGE-CMR tends to underestimate scar presence in naive patients compared to EAM, potentially due to baseline voltage scars, which are less dense compared to iatrogenic scar, the voltage criteria for scar in literature. Sensitivity is higher in segmental comparison, while specificity is superior in PBP, consistent with expectations of minimizing the size of comparison (from a segment to point). Hence, qualitative segmental method is better when assessing overall scarring, while PBP for assessing smaller specific areas. PBP comparisons exhibit less bias than segmental comparisons, which are overly sensitive to small scars. The diminished reliability of the LA roof is probably due to the margin of the CMR scan. Our study result regarding low sensitivity and high specificity of PBP aligns with a prior study investigating naïve patients through PBP comparisons, highlighting the need for a larger-scale study to assess LGE-CMR accuracy, considering various wall thicknesses and establishing scar thresholds on ADAS and CARTO software, for ablation-naïve patients with LA body scars.PBP projection on LA bodyAUC for PBP comprasion

Three-dimensional cardiac magnetic resonance allows the identification of slow-conducting anatomical isthmuses in tetralogy of Fallot.

Patients with repaired tetralogy of Fallot remain at risk of life-threatening ventricular tachycardia related to slow-conducting anatomical isthmuses (SCAIs). Preventive ablation of SCAI identified by invasive electroanatomical mapping is increasingly performed. This study aimed to non-invasively identify SCAI using 3D late gadolinium enhancement cardiac magnetic resonance (3D-LGE-CMR). Consecutive tetralogy of Fallot patients who underwent right ventricular electroanatomical mapping (RV-EAM) and 3D-LGE-CMR were included. High signal intensity threshold for abnormal myocardium was determined based on direct comparison of bipolar voltages and signal intensity by co-registration of RV-EAM with 3D-LGE-CMR. The diagnostic performance of 3D-LGE-CMR to non-invasively identify SCAI was determined, validated in a second cohort, and compared with the discriminative ability of proposed risk scores. The derivation cohort consisted of 48 (34 ± 16 years) and the validation cohort of 53 patients (36 ± 18 years). In the derivation cohort, 78 of 107 anatomical isthmuses (AIs) identified by EAM were normal-conducting AI, 22 were SCAI, and 7 blocked AI. High signal intensity threshold was 42% of the maximal signal intensity. The sensitivity and specificity of 3D-LGE-CMR for identifying SCAI or blocked AI were 100% and 90%, respectively. In the validation cohort, 85 of 124 AIs were normal-conducting AI, 36 were SCAI, and 3 blocked AI. The sensitivity and specificity of 3D-LGE-CMR were 95% and 91%, respectively. All risk scores showed an at best modest performance to identify SCAI (area under the curve ≤ .68). 3D late gadolinium enhancement cardiac magnetic resonance can identify SCAI with excellent accuracy and may refine non-invasive risk stratification and patient selection for invasive EAM in tetralogy of Fallot.

Cardiovascular magnetic resonance 3D LGE imaging for atrial fibrillation ablation

Abstract Objective To assess feasibility and reliability of cardiac magnetic resonance (CMR) imaging in visualizing left atrial (LA) structural changes that patients with paroxysmal atrial fibrillation (PAF) undergo acutely and long-term post ablation. Two different ablation methods i.e pulsed filed ablation (PFA) and cryoablation were studied. Background Pulmonary vein (PV) isolation has become the cornerstone technique for catheter ablation in patients with drug-refractory atrial fibrillation (AF). At present, almost all ablation technologies are mediated by a thermal effect. Recently, pulsed field ablation (PFA) has been introduced as a new energy source. PFA is a non-thermal ablative modality in which high voltage ultra-short pulses are applied to target tissue, while preserving the extracellular matrix architecture, nerves, and microvascular structures. Few data exist showing different structural LA changes between the two methods using Three-Dimensional (3D) Late Gadolinium Enhancement (LGE) Imaging. Methods CMR imaging was performed pre-ablation, acutely (<24 h), and 3 months post-ablation in 10 patients with paroxysmal atrial fibrillation (AF) undergoing pulmonary vein (PV) isolation with PFA (n = 5) or cryoablation (n = 5). 3D LGE and T2-weighted images were analysed. We obtained two data sets of 3D whole heart LGE imaging with high resolution, 20minutes post gadolinium injection. Patients underwent two acquisitions: the conventional respiratory navigated, electrocardiographically-gated 3D fast gradient echo (1.3slice thickness, 256x256 matrix acquisition) and the image-navigated isotropic high resolution 3D LGE with Dixon water-fat separation. 3D LGE data were analysed using a dedicated (ADAS LA) software in order to produce a 3D-LA model visualization of LGE areas. Results In the acute stage, LGE volume was 50% larger after PFA vs. cryoablation (P < 0.001), and oedema on T2 imaging was significantly smaller (visual estimation) on PFA cases. LGE rings surrounding PV ostia were more homogeneous after PFA, with no sign of microvascular damage or intramural hemorrhage as compared to cryoablation cases. In the chronic stage, the majority of acute LGE had disappeared after PFA, whereas most LGE persisted after cryo-ablation. The two 3D LGE sequences used for analysis showed complete agreement in the visualization of atrial scar with superior image quality of the novel sequence. The mean acquisition time for 3D LGE imaging was 11±4minutes for the conventional fast gradient echo sequence, and 5±2minutes for the novel 3D self-image navigated LGE sequence. Conclusions CMR imaging may help in visualization and understanding of LA structural remodeling post different ablation techniques.

Cardiac magnetic resonance - aided ventricular tachycardia substrate ablation

Abstract Objectives To assess feasibility and reliability of Cardiac magnetic resonance (CMR)-derived pixel signal intensity (PSI) maps obtained by Three Dimensional (3D)- Late Gadolinium Enhancement (LGE) images for detecting potential targets for VT substrate ablation procedures. Background Parametric imaging with color-coded PSI maps obtained from pre-ablation 3D LGE -CMR imaging can be used for identification and characterization of arrhythmic abnormal substrate. Recent publications have shown that PSI maps permit visualization of border zone areas inside the scar. Specifically, corridors of viable tissue within the scar connecting healthy myocardium define VT corridors that may be the electrical equivalent of the abnormal conducting channels on the electroanatomical mapping (EAM). Methods 8 patients with scar-dependent monomorphic VTs who underwent 3D-LGE CMR imaging prior to substrate ablation were included in the study. The studies were performed at 1.5T scanner. We obtained two data sets of 3D whole heart LGE imaging with high resolution. Patients underwent two acquisitions: the conventional respiratory navigated, electrocardiographically-gated 3D fast gradient echo (1.3slice thickness, 256x256 matrix acquisition) and the image-navigated isotropic high resolution 3D GE with Dixon water-fat separation. 3D LGE-CMR images were processed with ADAS-VT software to produce a 3-dimensional model of the heart with 10 layers from the endocardium to epicardium and PSI maps color-coded from the LGE data projected to each of the shells (Figure 1). In the LGE-CMR PSI maps, VT corridors were obtained automatically by the ADAS VT software. Subsequently, data were given to the electrophysiologists and merged with the CARTO3 system EAM data obtained for the ablation (Figure2). CMR information was used to facilitate the identification of target ablation sites in the areas identified by CMR as abnormal substrate and radiofrequency was applied in the presence of both a pathological electrogram (EGM) and a VT corridor as identified by CMR. Results There was a 95% agreement between the abnormal conducting channels as detected by abnormal electrograms in the EAM and the VT corridors identified by 3D LGE CMR imaging. In two occasions, there was one VT corridor that did not correlate with abnormal electrograms and was not ablated. The two 3D LGE sequences used for ADAS-VT corridors analysis showed complete agreement in the number and location of VT corridors. Mean acquisition time for 3D LGE imaging was 11±4minutes for the conventional fast gradient echo sequence, and 5±2minutes for the novel 3D self-image navigated LGE sequence. The image quality of 3D LGE was superior with the Dixon fat-water separation. Overall CMR-aided VT ablation reduced the procedural and fluoroscopy time by 40%. Conclusions CMR-aided VT ablation is feasible, reliable and significantly reduces the procedural time.

Evaluation of left atrial function and tissue after pulmonary vein isolation - initial results of The EVALUATE-PVI STUDY

Abstract Background Pulmonary vein (PV) isolation (PVI) by catheter ablation is a well-established treatment for atrial fibrillation (AF). Currently, the direct and long-term impact of ablation injury on left atrial (LA) function and tissue is poorly understood. Purpose The aim of this study is to evaluate the acute and long-term impact of radiofrequency (RF) PVI ablation on LA function and corresponding tissue changes as assessed with cardiac magnetic resonance imaging (CMR). Methods Seventeen patients (76.5% male, 60.8±9.7 years) with AF who underwent primo RF PVI as part of the EVALUATE-PVI STUDY underwent three sequential CMR scans prior to PVI, and <72 hours and 3 months after PVI ablation. T2-weighted turbo spin echo imaging was applied to assess post-ablation edema. High-resolution three-dimensional (3D) late gadolinium-enhancement (LGE) images were acquired to study acute LA wall injury and chronic fibrosis formation. LA volume and function were derived from two-chamber and four-chamber SSFP cine images. Maximum and minimal LA volume was indexed to body surface area to compute LAVimax and LAVimin. Longitudinal LA strain was calculated using feature tracking, and subdivided into reservoir, conduit, and contractile strain. Results Acutely post-PVI, T2w images showed increased signal intensity and thickening throughout the entire LA wall. LGE images showed diffuse increased signal intensity, without distinct focal high signal intensity areas (Fig. 1). Functional assessment at this stage showed significantly reduced LA emptying fraction (EF), LA reservoir, and contractile strain compared to pre-PVI (54.8±11.1% vs. 48.5±7.9%, p=0.01; 18.6±5.6% vs. 14.6±2.0%, p<0.01; 8.5±3.5% vs. 5.2±1.8%, p<0.001, respectively), while LA conduit strain, LAVimin, and LAVimax did not differ significantly (Fig. 2). The LGE images acquired at 3 months, displayed clear, focal, high signal intensity areas surrounding the PVs. At this stage post-PVI, a significant increase in LA strain was observed (LA reservoir strain 14.6±2.0% vs. 16.9±3.5%, p<0.01; and contractile strain 5.2±1.8% vs. 6.7±2.9%, p<0.01; respectively). LAVimax was significantly reduced at 3 months compared to acutely post-PVI (Fig. 2). LAVimin and LA conduit strain did not differ between the pre- and post-PVI stages. Conclusion PVI ablation resulted in temporary reduced LA function acutely post-PVI, likely due to edema and thickening of the LA wall. At three months, LGE images revealed clearly visible increased signal intensity in the ablated segments. At this stage, LA function improved, though full recovery did not occur as evidenced by the persistent slightly reduced contractile strain, possibly due to the ablation-induced loss of LA myocardium and formation of fibrosis. The decrease in LAVimax indicates reverse remodeling as a result of the ablation.

A patient-based realistic simulation platform to train future electrophysiologists to complex ablation procedures

Abstract Title An immersive catheter ablation platform based on patients’ 3D heart model for the simulation and training of future electrophysiologists. Background Arrhythmias are on the rise worldwide due to the ageing of the population. Projections urge to train the next generation of electrophysiologists (EPs) to perform catheter (CATH) ablation (CA) of arrhythmias. Training of future EPs relies mostly on a companionship, where teaching of CATH steerability is made on patients (pts) referred for procedures. While several other domains have established the utility of simulators to speed up learning process, increase safety and establish procedure road maps, dedicated simulators for EPs based on pts’ real anatomy do not exist yet. Purpose To speed up the handling of steerable CATH on MRI-based 3D heart models to optimize ablation road maps in pts referred for complex arrhythmias. Methods ARTS is an Artificial Intelligence and Augmented Reality-based platform comprising two key components: HeARTS and ARTSim. HeARTS is designed to automatically generate 3D heart models from pts’ MRI scans. ARTSim is a CA simulator that utilizes the 3D heart models of the pts for the planning and simulation of CA procedures. This simulator incorporates innovative techniques for digitizing and tracking the movements of a physical catheter in real time, and enabling its navigation within a completely virtual heart. With this technology, the CATH can be precisely steered to various heart locations, including the right and left (LA) atrium, the coronary sinus, and the right and left (LV) ventricles. Results Panel A of the figure shows the mannequin used for the simulator, where a real ablation CATH is introduced within the pt’s heart through a venous introducer. Panel B shows right anterior oblique (RAO) and modified RAO views of a pt suffering from a typical right atrial flutter. Note the presence of yellow tags positioned along the cavotricuspid isthmus (CTI) that the trainee must reach for a programmable duration using the shaft and the handle of the CATH shown in panel A. Panel C shows the virtual ablation CATH in green positioned on the target dots of the CTI. The pink color indicates that the trainee reached the desired target, which became red when the CATH remained stable at the same location for a duration of 5 sec (i.e. stability) with a force >5g (i.e. efficacy) and <20g (i.e. safety). Conclusions Herein, we present ARTS, a realistic CA simulation platform for the training of future EPs, that offers all the typical characteristics of 3D navigation including steerability, stability measurements and the force applied. Importantly, ARTS is based on the true anatomy of the pts, hence might be used to establish and discuss roadmaps to optimize procedures efficacy and safety. Version 2.0, foreseen early 2024, will include 3D late gadolinium enhancement and transseptal puncture for virtual LA and LV procedures, and later on activation times and myocardial voltage.Figure

The correlation between left atrium voltage mapping and 3D late gadolinium enhancement cardiac magnetic resonance imaging findings in patients with known atrial fibrillation

Abstract Introduction The non-invasive cardiac magnetic resonance imaging-based late gadolinium enhancement (LGE-CMR) of the left atrium (LA) fibrosis distribution and degree can be utilized for preprocedural planning and real-time guidance of AF ablation. There is an ongoing discussion with somewhat conflicting results regarding the accuracy of LGE-CMR of the LA as compared with the invasive electroanatomical mapping (EAM). Earlier data mainly encompassed post-ablation patients with an EAM involving a relatively small number of acquired points. Aim To evaluate the accuracy of LGE-CMR-based LA fibrosis using the ADAS 3D LA software among ablation-naive AF patients as compared with EAM using the CARTO-3 navigation system. Methods Patients with non-permanent AF, naive to previous ablation/cardiac surgery, who underwent CARTO-based AF ablation with preceding LGE-CMR scan were selected for the study. The 3D substrate atrial maps were generated based on the LGE-CMR (CMR-fib) using the ADAS 3D LA software and the CARTO EAM (EAM-fib). A fibrosis threshold of 1.2-1.32 was established for the CMR-fib using the Image Intensity Ratio whereas a threshold of 0.2-0.5mV was applied for the EAM-fib. The two 3D-maps were assessed for fibrosis using anatomically synchronized quantitative point by point comparison. The EAM was set as the gold standard. Results The study included 26,777 points acquired from 17 patients (aged 65.8±7.92 yrs with 6 females and 10 paroxysmal AF cases). CMR-fib showed 22.0% of LGE, while the EAM-fib demonstrated 48.8% of low voltage areas. The CMR-fib vs. EAM-fib point by point comparison concluded with an agreement of 73.1% and interrater reliability of 0.45 (p<0.001). The sensitivity and specificity were 45.0% and 99.9%, respectively, while the PPV and NPV were 99.8% and 65.6%, respectively. The Area Under the Curve (AUC) was calculated as 72.4. Increase of the EAM-fib threshold for scar (to 0.5-1.5mV) led to a further decrease in the assessment diagnostic performance and the interrater reliability. The lowest agreement was tested in the LA roof segments and the higher in the inferoseptal and posterior walls. Conclusion The LGE-CMR underestimates the presence of scar as presented by the EAM, however, those detected are mostly accurate. The low sensitivity that was demonstrated between the two methods may be attributed to the lower spatial resolution of the CMR as well as the superficial subendocardial layer mapped by EAM as compared with the averaged transmural mapping of the CMR-fib. A follow-up larger-scale study is required to assess the accuracy of LGE-CMR-based assessment of fibrosis, considering various wall thicknesses and establishing the scar threshold on the ADAS software, particularly for ablation-naive patients.

Single-beat mapping during programmed ventricular stimulation identifies infarct-related ventricular tachycardia isthmus sites

Abstract Introduction Complete characterization of reentrant ventricular tachycardia (VT) circuits using classical electrophysiological criteria and high-density mapping during VT is complex. Often, VT episodes cannot be fully characterized because of hemodynamic instability during the mapping procedure. This common limitation has motivated the implementation of mapping strategies during sinus rhythm or ventricular pacing aiming to identify scar regions associated with VT isthmuses or potential arrhythmogenicity. However, these strategies still require time-consuming catheter-based mapping. Purpose We aimed to develop and validate a single-beat mapping strategy during programmed ventricular stimulation (PVS) to identify scar regions associated with VT isthmus sites. We hypothesize that protected VT isthmuses within scar regions are characterized by larger activation time dispersion than surrounding scar areas not related to critical isthmus sites. Methods Experimental study including 16 pigs with established myocardial infarction (MI) in the anterior wall. Ten-to-twelve weeks after MI, pigs underwent left ventricular characterization using 3D late-gadolinium enhancement cardiac magnetic resonance imaging, and further invasive electroanatomical mapping to obtain high-density sequential activation maps during ventricular pacing and VT. High-density maps were compared with single-beat maps using a 64-pole basket catheter positioned in the left ventricle. After VT induction, high-density activation maps in well-tolerated VT episodes were used to identify VT isthmus sites. Simultaneous activation data from the 64-pole basket catheter were also obtained during PVS and VT. Off-line analysis was performed to: (i) validate single-beat activation maps during the baseline drive pacing cycle length (S1), and (ii) identify basket-catheter electrodes with the larger local activation time-dispersion score (LAT-DS) during PVS including up to 4 coupled extrastimuli (S2, S3, S4, S5) (Figure 1). LAT-DS was defined as the normalized product of the LAT of each electrode and the activation dispersion among its neighboring electrodes. Results Basket-catheter based single-beat activation maps during S1 pacing showed high correlation values with high-density sequential activation maps (N=16. R²=0.91). In 7 pigs, the VT isthmus site was fully characterized using high-density activation maps. During any of the coupled extrastimuli of the PVS protocol, basket catheter electrodes colocalizing with documented VT isthmus sites on high-density maps showed higher LAT-DS values compared to surrounding electrodes located on scar or healthy regions. Conversely, LAT values alone could not differentiate scar regions relevant for VT maintenance during PVS protocol (Figure 2). Conclusions Sixty-four pole single-beat activation maps during PVS identify left ventricular scar areas with higher than surrounding LAT-DS values that are associated with VT isthmus sites.Figure 1.Experimental modelFigure 2.A-Example. Usformer: A small network for left atrium segmentation of 3D LGE MRI

Left atrial (LA) fibrosis plays a vital role as a mediator in the progression of atrial fibrillation. 3D late gadolinium-enhancement (LGE) MRI has been proven effective in identifying LA fibrosis. Image analysis of 3D LA LGE involves manual segmentation of the LA wall, which is both lengthy and challenging. Automated segmentation poses challenges owing to the diverse intensities in data from various vendors, the limited contrast between LA and surrounding tissues, and the intricate anatomical structures of the LA. Current approaches relying on 3D networks are computationally intensive since 3D LGE MRIs and the networks are large. Regarding this issue, most researchers came up with two-stage methods: initially identifying the LA center using a scaled-down version of the MRIs and subsequently cropping the full-resolution MRIs around the LA center for final segmentation. We propose a lightweight transformer-based 3D architecture, Usformer, designed to precisely segment LA volume in a single stage, eliminating error propagation associated with suboptimal two-stage training. The transposed attention facilitates capturing the global context in large 3D volumes without significant computation requirements. Usformer outperforms the state-of-the-art supervised learning methods in terms of accuracy and speed. First, with the smallest Hausdorff Distance (HD) and Average Symmetric Surface Distance (ASSD), it achieved a dice score of 93.1% and 92.0% in the 2018 Atrial Segmentation Challenge and our local institutional dataset, respectively. Second, the number of parameters and computation complexity are largely reduced by 2.8x and 3.8x, respectively. Moreover, Usformer does not require a large dataset. When only 16 labeled MRI scans are used for training, Usformer achieves a 92.1% dice score in the challenge dataset. The proposed Usformer delineates the boundaries of the LA wall relatively accurately, which may assist in the clinical translation of LA LGE for planning catheter ablation of atrial fibrillation.

Novel Self-Calibrated Threshold-Free Probabilistic Fibrosis Signature Technique for 3D Late Gadolinium Enhancement MRI.

Myocardial fibrosis, marked by excessive collagen buildup in the heart, is a crucial severity marker of heart muscle injury in several heart diseases, such as myocardial infarction, cardiomyopathies, and atrial fibrillation (AF). It is also vital for evaluating the efficacy of induced scarring (dense fibrosis) post-interventions, such as catheter ablation for AF. Cardiac MRI emerged as the gold standard for evaluating myocardial fibrosis and scarring for diagnosis and intervention planning. However, existing 3D cardiac MRI (CMR) fibrosis analysis methods are unreliable as they rely on variable thresholding and suffer from a lack of standardization and high sensitivity to typical MRI uncertainties. Importantly, these methods quantify severity based on fibrosis volume alone while ignoring the unique MRI characteristics of fibrosis distribution, which could better inform on disease severity. To address these limitations, we propose a novel thresholdfree and self-calibrating probabilistic method named "Fibrosis Signatures" for a comprehensive and reliable fibrosis analysis of 3D MRI cardiac images. Through a novel efficient (linear complexity) probabilistic encoding of 'multibillion' MRI intensity disparities into standardized probability density function, our method derives the patient's unique fibrosis signature profile and index (FSI). Our approach goes beyond mere measuring of fibrosis volume; it encodes both the extent and the unique MRI characteristics of fibrosis distribution beyond mere entropy for a more detailed evaluation of fibrosis burden/severity. Our self-calibrating design effectively adjusts for MRI uncertainties like noise, low spatial resolution, and segmentation errors to ensure robust and reproducible fibrosis evaluation pre- and post-intervention. Validated in numerical phantom and 143 in vivo MRI scans of AF patients and compared to five baseline methods, our method showed strong correlations with traditional volume measures of pre-intervention fibrosis and post-intervention scar and was up to 9- times more reliable and reproducible, highlighting its potential to enhance cardiac MRI's utility.

Association between Left Atrial Late Gadolinium Enhancement and Atrial Fibrillation: The Multi-Ethnic Study of Atherosclerosis (MESA).

To determine the prevalence and correlates of left atrial (LA) late gadolinium enhancement (LGE) at cardiac MRI and its association with atrial fibrillation (AF) in a population-based sample from the Multi-Ethnic Study of Atherosclerosis (MESA). In this secondary post hoc analysis of the MESA cohort (ClinicalTrials.gov no. NCT00005487), participants without AF underwent LGE cardiac MRI at the fifth examination (2010-2012). LA LGE burden was quantified using the image intensity ratio technique on biplane long-axis two-dimensional (2D) LGE images without fat saturation. Survival analysis was performed with log-rank testing and Cox regression. Of 1697 participants (mean age, 67 years ± 9 [SD]; 872 men), 1035 (61%) had LA LGE, and 75 (4.4%) developed AF during follow-up (median, 3.95 years). At univariable analysis, LA LGE was associated with age (β = .010 [95% CI: .005, .015], P < .001), diastolic blood pressure (β = .005 [95% CI: .001, .009], P = .02), HbA1c level (β = .06 [95% CI: .02, .11], P = .009), heart failure (β = .60 [95% CI: .11, 1.08], P = .02), LA volume (β = .008 [95% CI: .004, .012], P < .001), and LA function (emptying fraction, LA global longitudinal strain, LA early diastolic peak longitudinal strain rate, and LA late diastolic peak strain rate; all P < .05). After adjusting for the variables in the Cohorts for Heart and Aging Research in Genomic Epidemiology (CHARGE) AF score, LA LGE independently helped predict incident AF (hazard ratio = 1.46 [95% CI: 1.13, 1.88], P = .003). The highest tertile (LGE > 2%) was twice as likely to develop AF. Although limited by the 2D LGE technique employed, LA LGE was associated with adverse atrial remodeling and helped predict AF in a multiethnic population-based sample.Clinical trial registration no. The role and implications of left atrial fibrosis in surgical mitral valve repair as assessed by CMR: the ALIVE study design and rationale.

Patients with mitral regurgitation (MR) commonly suffer from left atrial (LA) remodeling. LA fibrosis is considered to be a key player in the LA remodeling process, as observed in atrial fibrillation (AF) patients. Literature on the presence and extent of LA fibrosis in MR patients however, is scarce and its clinical implications remain unknown. Therefore, the ALIVE trial was designed to investigate the presence of LA remodeling including LA fibrosis in MR patients prior to and after mitral valve repair (MVR) surgery. The ALIVE trial is a single center, prospective pilot study investigating LA fibrosis in patients suffering from MR in the absence of AF (identifier NCT05345730). In total, 20 participants will undergo a CMR scan including 3D late gadolinium enhancement (LGE) imaging 2 week prior to MVR surgery and at 3 months follow-up. The primary objective of the ALIVE trial is to assess the extent and geometric distribution of LA fibrosis in MR patients and to determine effects of MVR surgery on reversed atrial remodelling. This study will provide novel insights into the pathophysiological mechanism of fibrotic and volumetric atrial (reversed) remodeling in MR patients undergoing MVR surgery. Our results may contribute to improved clinical decision making and patient-specific treatment strategies in patients suffering from MR.

Myocardial Scar Detection Using High-Resolution Free-Breathing 3D Dark-Blood and Standard Breath-Holding 2D Bright-Blood Late Gadolinium Enhancement MRI: A Comparison of Observer Confidence.

To compare observer confidence for myocardial scar detection using 3 different late gadolinium enhancement (LGE) data sets by 2 observers with different levels of experience. Forty-one consecutive patients, who were referred for 3D dark-blood LGE MRI before implantable cardioverter-defibrillator implantation or ablation therapy and who underwent 2D bright-blood LGE MRI within a time frame of 3 months, were prospectively included. From all 3D dark-blood LGE data sets, a stack of 2D short-axis slices was reconstructed. All acquired LGE data sets were anonymized and randomized and evaluated by 2 independent observers with different levels of experience in cardiovascular imaging (beginner and expert). Confidence in detection of ischemic scar, nonischemic scar, papillary muscle scar, and right ventricular scar for each LGE data set was scored using a using a 3-point Likert scale (1 = low, 2 = medium, or 3 = high). Observer confidence scores were compared using the Friedman omnibus test and Wilcoxon signed-rank post hoc test. For the beginner observer, a significant difference in confidence regarding ischemic scar detection was observed in favor of reconstructed 2D dark-blood LGE compared with standard 2D bright-blood LGE (p = 0.030) while for the expert observer, no significant difference was found (p = 0.166). Similarly, for right ventricular scar detection, a significant difference in confidence was observed in favor of reconstructed 2D dark-blood LGE compared with standard 2D bright-blood LGE (p = 0.006) while for the expert observer, no significant difference was found (p = 0.662). Although not significantly different for other areas of interest, 3D dark-blood LGE and its derived 2D dark-blood LGE data set showed a tendency to score higher for all areas of interest at both experience levels. The combination of dark-blood LGE contrast and high isotropic voxels may contribute to increased observer confidence in myocardial scar detection, independent of observer's experience level but in particular for beginner observers.

High-resolution structural-functional substrate-trigger characterization: Future roadmap for catheter ablation of ventricular tachycardia.

Patients with ventricular tachyarrhythmias (VT) are at high risk of sudden cardiac death. When appropriate, catheter ablation is modestly effective, with relatively high VT recurrence and complication rates. Personalized models that incorporate imaging and computational approaches have advanced VT management. However, 3D patient-specific functional electrical information is typically not considered. We hypothesize that incorporating non-invasive 3D electrical and structural characterization in a patient-specific model improves VT-substrate recognition and ablation targeting. In a 53-year-old male with ischemic cardiomyopathy and recurrent monomorphic VT, we built a structural-functional model based on high-resolution 3D late-gadolinium enhancement (LGE) cardiac magnetic resonance imaging (3D-LGE CMR), multi-detector computed tomography (CT), and electrocardiographic imaging (ECGI). Invasive data from high-density contact and pace mapping obtained during endocardial VT-substrate modification were also incorporated. The integrated 3D electro-anatomic model was analyzed off-line. Merging the invasive voltage maps and 3D-LGE CMR endocardial geometry led to a mean Euclidean node-to-node distance of 5 ± 2 mm. Inferolateral and apical areas of low bipolar voltage (<1.5 mV) were associated with high 3D-LGE CMR signal intensity (>0.4) and with higher transmurality of fibrosis. Areas of functional conduction delay or block (evoked delayed potentials, EDPs) were in close proximity to 3D-LGE CMR-derived heterogeneous tissue corridors. ECGI pinpointed the epicardial VT exit at ∼10 mm from the endocardial site of origin, both juxtaposed to the distal ends of two heterogeneous tissue corridors in the inferobasal left ventricle. Radiofrequency ablation at the entrances of these corridors, eliminating all EDPs, and at the VT site of origin rendered the patient non-inducible and arrhythmia-free until the present day (20 months follow-up). Off-line analysis in our model uncovered dynamic electrical instability of the LV inferolateral heterogeneous scar region which set the stage for an evolving VT circuit. We developed a personalized 3D model that integrates high-resolution structural and electrical information and allows the investigation of their dynamic interaction during arrhythmia formation. This model enhances our mechanistic understanding of scar-related VT and provides an advanced, non-invasive roadmap for catheter ablation.

Image Quality of Submillimeter High-Spatial-Resolution 2D Late Gadolinium-enhanced Images in Cardiac MRI: A Feasibility Study.

To evaluate the image quality of high-spatial-resolution two-dimensional (2D) late gadolinium enhancement (LGE) cardiac MRI compared with conventional normal-resolution LGE MRI. This prospective study included participants suspected of having cardiomyopathy who underwent cardiac MRI between March 2021 and December 2021. Normal-resolution and high-resolution 2D LGE sequences (inversion recovery [IR] and phase-sensitive inversion recovery [PSIR]) were performed at 3 T. Resolution was compared between normal-resolution and high-resolution images obtained in a quality assurance phantom. In vivo image quality and resolution were evaluated qualitatively using a five-point scoring system. Receiver operating characteristic curve analysis was used for LGE detection performance. Border sharpness was assessed with profile curve measurement. The contrast-to-noise ratio (CNR) between hyperenhancement and remote myocardium and LGE detection performance were calculated using normal-resolution IR images as the reference. In total, 120 participants were evaluated (mean age, 56 years ± 17 [SD]; 72 men). Features smaller than 1 mm were detectable only on high-resolution images of the phantom. In vivo, the image resolution score with high-resolution LGE was 4.14-4.24, which was higher than the normal-resolution LGE reference score of 2.99 (P < .05). Border sharpness was higher in high-resolution images (P < .001). Receiver operating characteristic curve analysis revealed no evidence of a difference in LGE detection between normal-resolution and high-resolution images. There was also no evidence of a change in CNR of LGE in IR and PSIR magnitude compared with reference images. Comparison of image quality in 2D high-resolution and normal-resolution LGE cardiac MRI demonstrated the highest resolution for high-resolution IR and high-resolution PSIR magnitude sequences.Keywords: Cartilage Imaging, MRI, Cardiac, Heart, Imaging Sequences, Comparative Studies Supplemental material is available for this article. © RSNA, 2022.

Abstract 10242: Right Atrial Function and Fibrosis in Relation to Successful Atrial Fibrillation Ablation

Introduction: Atrial remodeling, characterized by changes in atrial structure and function, is central to the development and maintenance of atrial fibrillation (AF). Besides left atrial (LA) remodeling, right atrial (RA) remodeling may also contribute to AF perpetuation, and its presence may affect clinical outcome of AF ablation. Bi-atrial remodeling in patients with AF is rarely assessed and data on the presence of RA fibrosis, the relationship between RA and LA fibrosis, and possible association of RA remodeling with AF recurrence after ablation is limited. Methods: 110 AF patients undergoing initial pulmonary vein isolation (PVI) were included in the present study. Patients were in sinus rhythm during cardiac magnetic resonance (CMR) imaging performed prior to ablation. LA and RA volumes and function (volumetric and strain) were derived from cine CMR images. The extent of LA and RA fibrosis was assessed from 3D late gadolinium enhancement images. AF recurrence was followed-up for 12 months after PVI using either 12 lead ECGs or Holter monitoring. Results: Arrhythmia recurrence was observed in 39 patients (36%) after the 90-day blanking period, occurring at a median of 181 (IQR: 122-286) days. RA remodeling parameters were not significantly different between patients with AF recurrence as compared to patients without AF recurrence after ablation, whereas LA remodeling parameters were (volume, emptying fraction and strain). LA fibrosis had a strong correlation with RA fibrosis (r=0.88, P &lt;0.001). Both, LA and RA fibrosis was not different between patients with and without AF recurrence. Conclusions: RA remodeling parameters were not predictive of AF recurrence after AF ablation. Bi-atrial fibrotic remodeling is present in AF patients and the amount of LA fibrosis had a strong correlation with the amount of RA fibrosis. The presence of bi-atrial fibrotic tissue may be an expression of disease state although no relation with AF recurrence after AF ablation was found.

The use of novel 3D dark-blood late gadolinium enhancement MRI to determine the optimal threshold for atrial scar after pulmonary vein isolation ablation

Abstract Background Dark-blood late gadolinium enhancement (LGE) magnetic resonance imaging (MRI) is proved to be superior to bright-blood LGE MRI in localising subtle subendocardial scar in the ventricles, because of improved contrast between myocardial scar and blood. However, dark-blood LGE MRI has not yet been applied to identify atrial scar in the left atrium (LA) and therefore its threshold to determine scar is unknown. Purpose To determine the optimal intensity threshold for 3D dark-blood LGE MRI for atrial ablation scar after pulmonary vein isolation (PVI) Methods Twelve re-do PVI patients with symptomatic atrial fibrillation (AF) who underwent pre-procedural 3D dark-blood LGE MRI were included. The image intensity ratio (IIR = myocardial intensity normalized to the blood pool) from the LGE MRI were calculated using ADAS-AF. High-density bipolar voltages (BiV) maps were recorded during sinus rhythm prior to ablation. All BiV locations ≤5 mm from the ADAS LA anatomy were compared with the corresponding IIR, using custom-made software in MATLAB. To achieve an equal ratio between scar (BiV ≤0.15 mV) and non-scar (BiV &amp;gt;0.15 mV) for each patient, non-scar pairs were randomly resampled to the same number as scar pairs. This was repeated 10 times and for every random selection, receiver operating characteristics (ROC) analysis was performed to determine the optimal IIR threshold (provided by the Youden's index) for scar defined as BiV &amp;lt;0.15 mV (Figure 1). All IIR thresholds and areas under the curve were averaged to determine the overall performance and optimal IIR threshold. Results Of the 12 included patients, 8 had prior cryo PVI, 2 radiofrequency PVI, and 2 surgical/hybrid AF ablation. ROC curve analysis estimated the average optimal threshold for predicting BiV &amp;lt;0.15 mV to be an IIR of 1.106, with a mean area under the curve (AUC) of 0.73 (Figure 1). Figure 2 shows two examples of the IIR map (A), BiV map (B), and the correspondence map (C) providing information on spatial agreement between IIR and BiV. This individual qualitative assessment provides insight into the spatial variation between techniques and may facilitate future studies on the pathophysiological understanding of atrial ablation scarring. Conclusion This is the first study to use the novel 3D dark-blood whole heart LGE MRI to evaluate LA ablation scar after PVI. Based on the ROC analyses, an IIR of 1.106 is the optimal threshold for atrial ablation scar, defined as high density bipolar voltage &amp;lt;0.15 mV. Myocardial scar detection in free-breathing Dixon-based fat- and water-separated 3D inversion recovery late-gadolinium enhancement whole heart MRI

The aim of this study was to investigate the diagnostic accuracy and reader confidence for late-gadolinium enhancement (LGE) detection of a novel free-breathing, image-based navigated 3D whole-heart LGE sequence with fat–water separation, compared to a free-breathing motion-corrected 2D LGE sequence in patients with ischemic and non-ischemic cardiomyopathy. Cardiac MRI patients including the respective sequences were retrospectively included. Two independent, blinded readers rated image quality, depiction of segmental LGE and documented acquisition time, SNR, CNR and amount of LGE. Results were compared using the Friedman or the Kruskal–Wallis test. For LGE rating, a jackknife free-response receiver operating characteristic analysis was performed with a figure of merit (FOM) calculation. Forty-two patients were included, thirty-two were examined with a 1.5 T-scanner and ten patients with a 3 T-scanner. The mean acquisition time of the 2D sequence was significantly shorter compared to the 3D sequence (07:12 min vs. 09:24 min; p < 0.001). The 3D scan time was significantly shorter when performed at 3 T compared to 1.5 T (07:47 min vs. 09:50 min; p < 0.001). There were no differences regarding SNR, CNR or amount of LGE. 3D imaging had a significantly higher FOM (0.89 vs. 0.78; p < 0.001). Overall image quality ratings were similar, but 3D sequence ratings were higher for fine anatomical structures. Free-breathing motion-corrected 3D LGE with high isotropic resolution results in enhanced LGE-detection with higher confidence and better delineation of fine structures. The acquisition time for 3D imaging was longer, but may be reduced by performing on a 3 T-scanner.
