 Hailey-Hailey disease (HHD) is a rare autosomal dominant acantholytic dermatosis. It is characterized by a recurrent eruption of vesicles, erosions, and scaly erythematous plaques involving intertriginous areas and first occurring after puberty, mostly in the third or fourth decade. In 2000, mutations in the ATP2C1 gene on band 3q22.1, encoding the secretory pathway Ca2+/Mn2+-ATPase protein 1(hSPCA1), have been identified as the cause of HHD. We report the identification of two novel mutations of ATP2C1 gene in two Portuguese patients, which expands the spectrum of ATP2C1 mutations underlying HHD and provides useful information for genetic counseling.

 Folliculitis decalvans and lichen planopilaris have been considered two distinct cicatricial alopecias. However, biphasic presentation of folliculitis decalvans - lichen planopilaris in the same patient has been recently described, therefore raising the doubt if they are two distinct entities or a continuous phenotypic spectrum.We describe the case of a man who presented clinical, trichoscopic, and histopathological features of both entities. Moreover, we discuss current theories about the pathogenesis of the two diseases and their coexistence in the same patient. The recognition of similar cases allows to optimize the approach and treatment.

Trichilemmal horn or trichilemmal keratosis is a benign neoplasm, that occurs more frequently in females and in the age group over 50 years old. Clinically, it presents as a cutaneous horn and histologically shows trichilemmal keratinization at the base of the lesion.As an uncommon pathology, in this case report, we will present the case of a young man (14 years old), presenting a keratotic lesion on the forehead for eight months, which revealed the diagnosis of trichilemmal horn after lesion exeresis and anatomopathological analysis.

A criptococose é uma infecção sistémica causada por Cryptococcus neoformans, levedura oportunista encapsulada. Esta infeção ocorre principalmente em indivíduos imunocomprometidos e os sintomas variam de acordo com a integridade do sistema imunológico. A criptococose cutânea afeta aproximadamente 20% dos pacientes com criptococose disseminada, mas a criptococose cutânea primária (PCC) sem infecção sistémica é rara. Um paciente do sexo masculino de 76 anos, com doença pulmonar obstrutiva crónica, hipertensão arterial e dislipidemia, apresentou-se com placa cutânea inflamatória violácea com bolhas que progrediu apesar de ceftriaxone endovenoso por 7 dias, sem sucesso. A biópsia incisional revelou o diagnóstico histopatológico de criptococose cutânea. As lesões resolveram após tratamento com fluconazol 300 mg/dia durante 3 meses. Não se detectou doença sistémica e não havia qualquer evidência de imunossupressão. Enfatiza-se a importância da inclusão da criptococose cutânea no diagnóstico diferencial das lesões cutâneas, mesmo em pacientes sem terapia imunossupressora. As manifestações cutâneas da infecção podem ser o primeiro indício para uma doença disseminada, pelo que o seu reconhecimento precoce é fundamental para um bom prognóstico.

A 74-year-old man with past medical history of obesity and type 2 diabetes mellitus was admitted for COVID-19 pneumonia. Due to unfavorable disease progression, ten days later, the patient was admitted to the intensive care unit and intubated, requiring periods of prone position and systemic corticosteroid treatment. During the following weeks, the patient developed several infectious complications successfully managed with systemic antibiotics and herpes simplex infection of the left upper lip, treated with acyclovir 400 mg twice daily for 10 days.

Introdução: O metotrexato é utilizado no tratamento sistémico da psoríase desde 1950, continuando a ser um fármaco de primeira linha. É importante avaliar os fatores que influenciam a sua descontinuação. O objetivo dos autores foi identificar determinantes do tempo de so- brevivência do metotrexato em doentes com psoríase. Métodos: Foi realizada uma análise retrospetiva dos doentes incluídos no registo nacional DERMA.PT pelo Centro Hospitalar Universitário São João e que iniciaram tratamento para a psoríase com metotrexato entre janeiro de 2010 e janeiro de 2020. Resultados: Foram identificados 146 doentes com psoríase tratados com metotrexato em monoterapia ou em associação com fototerapia. A maioria era do sexo masculino (55%), com idade média de 51±13 anos e com psoríase há mais de 10 anos. A psoríase vulgar (49%) e a psoríase artropática (47%) foram as formas mais comuns de psoríase, com um PASI inicial médio de 10,7±5,6. A maioria dos doentes era naïve para o tratamento com metotrexato. No total, 66 (45%) doentes descontinuaram o tratamento, com um tempo médio de sobrevivência do fármaco de 18,0±15,5 meses. As razões mais comuns de descontinuação foram ineficácia (32%), má adesão terapêutica (18%) e intolerância gastrointestinal (11%). O tratamento prévio com ciclosporina e a presença de artrite psoriática associaram-se ambos à descontinuação do metotrexato (estatisticamente significativo). Na maioria dos casos, foi realizado switch para anticorpo monoclonal anti-TNFα. Discussão: Nos últimos anos, foram publicados estudos retrospetivos com enfoque no tempo de sobrevivência do metotrexato na psoríase, que revelaram um tempo médio entre 12 a 21 meses. As principais razões de descontinuação foram a ineficácia e os efeitos adversos, nomeadamente gastrointestinais. O nosso estudo está em linha com o descrito mas destaca-se pela elevada percentagem de doentes que mantiveram o tratamento. A tremenda evolução no tratamento da psoríase ao longo dos 10 anos do estudo influenciou grandemente os resultados. A principal limitação desta análise resulta do facto de ser um estudo retrospetivo. Conclusão: Os resultados estão de acordo com as caraterísticas de sobrevivência do metotrexato publicadas e reforçam a sua importância e o seu papel no tratamento desta doença.

Zoon balanitis and lichen sclerosus are both chronic inflammatory disorders of the genital mucosa that usually affect middle-aged or elderly uncircumcised men.Although the precise etiology of Zoon balanitis is still unclear, a pathogenic role of irritant and mechanical factors has been suggested. Therefore, foreskin sclerosis and phimosis caused by male genital lichen sclerosus may trigger the development of Zoon balanitis. However, until the present, only three cases with clinical and histopathologic features consistent with synchronous presentation of both disorders have been described. We report the case of a 70-year-old male who developed Zoon balanitis in association with lichen sclerosus, that cleared only after circumcision.

Cutaneous squamous cell carcinoma in situ of the eyelid and periorbital skin malignancies is common and its management is demanding. Surgical excision is considered the first-line treatment, but these techniques have limitations. Topical photodynamic therapy is currently approved for the treatment of squamous cell carcinoma in situ in other areas, but the reports of its use in this sensitive anatomic location area are scarce. Herein we report the case of a 61-year-old-man with extensive periocular Bowen disease that was treated successfully with photodynamic therapy. Photodynamic therapy may be an option as neoadjuvant or curative therapy in selected cases, especially in sensitive anatomic locations where surgery might lead to distortion of the structures or where techniques as micrographically controlled surgery are not available.

In recent years, several studies have demonstrated the involvement of the intestinal microbiota in immune-mediated diseases such as diabetes, ulcerative colitis, and multiple sclerosis. There are few data on the follicular microbiome and its role in the pathogenesis of scalp diseases. Some studies show influence of dysbiosis on these diseases, and manipulation of the microbiome may represent a possible therapeu- tic option. This article reviews current knowledge regarding the impact of dysbiosis on dermatological diseases of the scalp, such as seborrheic dermatitis, psoriasis, alopecia areata, androgenetic alopecia, lichen planus pilaris, frontal fibrosing alopecia and decalvant folliculitis. A broader understanding of this may suggest additional treatments beyond conventional therapies.
