Effects of air temperature on the risk of death from COPD in major microregions in Brazil: a time series study.

To evaluate the association between the risk of death from COPD and air temperature events in ten major Brazilian microregions. This was a time series analysis of daily COPD deaths and daily mean air temperatures between 1996 and 2017. Using distributed nonlinear lag models, we estimated the cumulative relative risks of COPD mortality for four temperature percentiles (representing moderate and extreme cold and heat events) in relation to a minimum mortality temperature, with a lag of 21 days, in each microregion. Significant associations were found between extreme air temperature events and the risk of death from COPD in the southern and southeastern microregions in Brazil. There was an association of extreme cold and an increased mortality risk in the following microregions: 36% (95% CI, 1.12-1.65), in Porto Alegre; 27% (95% CI, 1.03-1.58), in Curitiba; and 34% (95% CI, 1.19-1.52), in São Paulo; whereas moderate cold was associated with an increased risk of 20% (95% CI, 1.01-1.41), 33% (95% CI, 1.09-1.62), and 24% (95% CI, 1.12-1.38) in the same microregions, respectively. There was an increased COPD mortality risk in the São Paulo and Rio de Janeiro microregions: 17% (95% CI, 1.05-1.31) and 12% (95% CI, 1,02-1,23), respectively, due to moderate heat, and 23% (95% CI, 1,09-1,38) and 32% (95% CI, 1,15-1,50) due to extreme heat. Non-optimal air temperature events were associated with an increased risk of death from COPD in tropical and subtropical areas of Brazil.

Prevalence and burden of nausea and vomiting in pregnant women: Interim analysis of the PURITY survey

ObjectivesNausea and vomiting in pregnancy is a common and invalidating condition in early pregnancy. However, no data are available on its prevalence in Italy. This survey aims to evaluate the prevalence and impact of nausea and vomiting during pregnancy on the quality of life of Italian women. Study designThe survey was performed in three Italian public University Hospitals in two distinct periods: a first interview took place between the 18th and 22nd week of pregnancy, using the Questionnaire for Pregnancy Period (14 questions regarding demographic data and 30 questions about nausea and vomiting in pregnancy, including Pregnancy-Unique Quantification of Emesis questionnaire), and a follow-up interview, by telephone call, took place immediately after giving birth and in any case within 14 days of delivery, using the Questionnaire for Post-Pregnancy (9 questions). Included women were Caucasian, in physiological pregnancy and between the 18th and 22nd week (time of morphological ultrasound), able to communicate adequately with the interviewer, understand the questionnaires and able to provide valid informed consent. Twin pregnancies and women who recurred to medically assisted procreation were excluded.This is an interim report on data collected from 232 of the planned 600 women. ResultsMean age of the recruited subjects was 32.6 ± 4.6 years, with approximately 60% primiparous. The prevalence of nausea and vomiting in pregnancy in the sample examined was 65.5% overall (152 out of 232 subjects). Of these 152 women, 63 (41.4%) experienced only nausea, 6 (3.9%) only vomiting, and 83 (54.6%) reported both. Symptoms were reported to begin at 7.2 ± 2.7 weeks, lasted 10.2 ± 5.6 weeks, and persisted at the time of the interview in 32.2% of cases. Overall, over 50% of the women interviewed experienced a negative impact of nausea and vomiting in pregnancy on social relationships and work activity. ConclusionsA high prevalence of nausea and vomiting in pregnancy, 65.5% overall, was found in this interim analysis. These symptoms appeared capable of negatively influencing women quality of life. Screening procedures should be offered during pregnancy and measures that address nausea and vomiting in pregnancy impact warranted.

Nitrogen and phosphate removal from dairy processing side-streams by monocultures or consortium of microalgae

Acid-casein production generates waste streams that are rich in nitrogen (in the form of protein and nitrate) and phosphate. This makes this type of waste very difficult to treat using conventional techniques resulting in a high amount of operating cost and costly investment. In this research, the application of single culture or consortium of microalgae for uptake of nitrogen and phosphate in the wastewater of an acid-casein factory was investigated. The waste was a 1:1 mixture of nanofiltered whey permeate and dairy processing wastewater. Monocultures of Chlorella vulgaris, Tetradesmus obloquus, Nonnochlropsis ocenica and a consortium of the three microalgae were analyzed. The results showed that the consortium exhibited more efficient nitrogen and phosphate removal compared to the individual species. The consortium was able to rapidly hydrolyse exogenous protein present in the waste medium, removing 88% of protein and breaking down complex protein molecules into simpler compounds (such as nitrate) for assimilation into the biomass. In the first fourteen days of cultivation, the rate of nitrate assimilation by the consortium biomass was lower than that of nitrate formation from protein degradation, leading to a net increase in nitrate concentration in the medium. As protein source was depleted and biomass concentration increased, however, the rate of nitrate assimilation began to exceed that of nitrate formation allowing for net removal of nitrate. The microalgae consortium was shown to successfully bioremediate all nitrates by day 21. It was indicated that Chlorella and Nannochloropsis species were responsible for nitrogen removal in monocultures. Phosphate, on the other hand, was efficiently removed by Tetradesmus. The results indicated that a consortium cultivation of three species of microalgae led to effective elimination of both nitrogen and phosphate. Combined flow-cytometry and microscopy analyses revealed that Chlorella overtook Tetradesmus and Nannochloropsis to emerge as the dominant population in the consortium by the end of the cultivation cycle. It can be concluded that the application of microalgae consortium for simultaneous recovery of nitrogen and phosphate is a promising approach for treating acid-casein wastewater.

Polycarbophil vaginal moisturizing gel versus hyaluronic acid gel in women affected by vaginal dryness in late menopausal transition: A prospective randomized trial

ObjectivesDuring menopausal transition, women are frequently affected by vulvovaginal atrophy (VVA), due to the decline of estrogen levels. Resulting symptoms are itching, burning, dyspareunia, and vaginal dryness (reported in up to 85%). The aim of this trial was to verify if medical device polycarbophil vaginal (PCV) moisturizer gel is not less effective than hyaluronic acid (HA) gel in treating vaginal dryness. Material and methodsThis was a multicenter, open label, randomized, parallel group, comparative study with non-inferiority design. Female included were ≥45 to ≤55 years in the menopausal transition, with subjective dryness, any objective sign of VVA, pH > 5, and body mass index of ≥18.5 to ≤36 kg/m2. Subjects were randomized to 1 g of PCV gel twice a week for 30 days or 3 g of HA vaginal gel every 3 days for 30 days. Results53 subjects (mean age 49.45 ± 2.96 years) were analyzed. Vaginal health index showed an improvement (p < 0.001) in both groups (from 12.54 ± 1.37 to 16.36 ± 2.66 for PCV, from 12.00 ± 1.91 to 16.60 ± 2.50 for HA), but the difference between final means (95%CI: −1.66 to 1.18) evidenced that PCV is non-inferior to HA treatment. Similarly, an improvement was evidenced in vaginal maturation index (p = 0.005 for PCV, ns. for HA), female sexual function index (p < 0.001 for PCV, p < 0.001 for HA), and SF-12 (p < 0.001 for PCV, p < 0.001 for HA), with no difference between groups. Safety was optimal and no adverse events were reported. ConclusionsThe use of HA gel does not give additional benefits to those that are already provided by the moisturizing PCV.
