Abstract

Wrong-side/wrong-site, wrong-procedure, and wrong-patient adverse events (WSPEs) are a frequent topic of discussion for their roles in unnecessary deaths, disability, suffering, malpractice, and erosion of public confidence in the health care system. This study analyzed reports of WSPEs from four databases: the National Practitioner Data Bank (NPDB); the Florida Code 15 mandatory reporting system; the American Society of Anesthesiologists (ASA) Closed Claims Project Database; and the authors’ anonymous, web-based incident-reporting tool.
