Abstract

A pioneering community-based multi-disciplinary wound care team, working alongside a tissue viability service, developed within East Lancashire teaching PCT. The drive to push the nurse-led wound care agenda forward requires powerful champions to acknowledge the value of multi-professional care, therefore this article embraces a care delivery model based on a 'spend to save' framework. This live platform, using a multi-disciplinary specialism, uses an electronic database to demonstrate measurable outcomes, reporting weekly and monthly patient and non-patient activities. The impetus for developing this service was to enable community nurses to refocus their time on patients with long-term conditions.
