Abstract

Due to transitions between community and inpatient care, family physicians are often unaware of their patients’ pregnancy history that may include pregnancy complications associated with early cardiovascular disease. A postcard containing targeted information on pregnancy risk factors was developed to determine whether patient education material could help both patients and family doctors to better manage women’s cardiovascular health after pregnancy.
