Abstract
A 44-year-old woman presented to the emergency department (ED) in cardiac arrest. Thirty minutes before arrival, she developed shortness of breath and called emergency medical services. En route to the hospital, she suffered a pulseless electrical activity cardiac arrest and arrived at the ED with chest compressions ongoing. Physical examination revealed no signs of life. A transesophageal echocardiogram was performed by the emergency physician (Figure 1 and Video E1 [available at http://www.annemergmed.com]).
