Abstract

Koffier, et al.1 report a case of hydranencephaly following polycythemia in a newborn infant. In their report, the authors speculate that the elevated hematocrit level in the newborn period may have led to cerebral infarction and liquifaction, with the baby then presenting 54 days later with hydranencephaly. There are a couple of problems with this speculation based on the information presented in the case report: (1) It is well recognized that a hydranencephalic baby can have a normal neurologic examination, as well as a normal head circumference and, thus, may be completely missed in the newborn nursery.
