Abstract

Background/Aim: Pernicious anemia characterized by severe gastric atrophy is suggested to be a strong risk factor for decreased bone mineral density (BMD) and bone fractures. In few previous studies, there was no relationship between atrophic gastritis and bone mineral density in postmenopausal women. However, bone mineral density is strongly affected by menopause itself. Therefore, we evaluated whether atrophic change of the gastric body was associated with bone mineral density in premenopausal women. Methods: A total of 315 premenopausal women, who underwent endoscopy and bone densitometry from January 2008 through October 2009 at health promotion center were included. The grade of atrophic gastritis was endoscopically diagnosed using the atrophic pattern system described by Kimura et al. Evaluation of lumbar spine and femur mineral density was performed using dualenergy X-ray absorptiometry. Anthropometric measurements such as body mass index and waist circumference were obtained by trained personnel. Results: One hundred fifty six patients showed endoscopic finding of gastric mucosa atrophy. Although BMDs of femur were not different between the patients with atrophic gastritis and the controls, BMD of lumbar spine was significantly lower in patients with atrophic gastritis than in the controls (1.147±0.147(g/cm2) and 1.189±0.155(g/cm2), respectively, p=0.012). Moreover, patients with atrophic change of gastric body (1.124±0.147(g/cm2) showed lower BMD than patients without body atrophy (p<0.001). In multivariate regression analysis, atrophic gastritis was independently associated with the age (p=0.015) and decreased BMD of lumbar spine (p= 0.008). Conclusion: Atrophic gastritis is associated with decreased BMD of lumbar spine in premenopausal women. Additional studies are necessary to elucidate the causal relationship between atophic gastritis and BMD.
