Abstract
症例1は73歳男性．右房に達する下大静脈腫瘍栓を伴う肝細胞癌に対し，肝右葉切除と腫瘍栓摘出を施行した．術後2年5カ月で腸骨に転移を認め放射線治療と化学療法を施行，術後5年5カ月Performance Status 0で生存中である．症例2は67歳男性．肝左葉の巨大肝細胞癌で，拡大肝左葉切除と横隔膜上部下大静脈に延びる腫瘍栓摘出を施行した．残存した肝内転移に対する術後肝動注化学療法が著効，術後4年10カ月再発なく健在である．本例で経過が良好であった理由は，大きな原発巣や腫瘍栓が手術により取り除かれたこと，再発や残存病変が良好に治療されたこと，肝炎ウイルスマーカーが陰性で肝機能が良好，多中心性再発も認めなかったことである．下大静脈腫瘍栓を伴う肝細胞癌に対しては，肝機能がゆるせば肝切除と腫瘍栓摘出を先行，再発や残存病変を積極的に治療することが最も効果的な治療と思われる．手術と術後早期の補助化学療法により根治が目指せるか否かは今後の検討課題である．
