Abstract

To examine the reliability of corpus callosum-fastigium length (CCF) and tectal length (TL) measurements for the evaluation of fetal midbrain in the third trimester of gestation. A prospective cohort of 33 singleton low-risk pregnancies were enrolled for fetal neurosonography assessment. Neurosonography was performed between 31.6 and 32.2 weeks of gestation. Two blinded-observers measured off-line CCF and TL twice using preselected 2D images of mid-sagittal cranial plane. Intra- and interobserver agreements for CCF and TL were evaluated by using the intraclass correlation coefficient (ICC) and Bland-Altman plots. The mean values obtained were 44.44 ± 0.72mm for CCF and 10.21 ± 0.13mm for TL. The mean interobserver difference was -0.0292 ± 0.3984 for CCF (P = 0.676) and 0.0758 ± 0.2822 for TL (P = 0.133). The ICCs for interobserver and intraobserver analysis showed excellent agreement for both CCF and TL. CCF intraobserver: 0.987 (95% CI, 0.974–0.995), interobserver: 0.964 (95% CI, 0.927–0.982) and TL intraobserver: 0.967 (95% CI, 0.933–0.986) and interobserver: 0.963 ( 95% CI,0.895–0.984). Figure 1 shows mid-sagittal fetal midbrain measurements. Corpus callosum-fastigium and tectal length measurements showed an excellent intraobserver and interobserver agreement. Corpus callosum-fastigium and tectal length could be used as reproducible markers of fetal midbrain assessment.
