Abstract

Case Presentation: A 49-year-old male presented to our Endocrine clinic with history of progressive stiffness and limitation of mobility over the last 4 decades. He developed painful soft tissue swellings over the left hip and thigh, back, and nape of the neck; these became harder and limited his movements. Recently, he had also noticed progressive difficulty in opening his mouth. There was a history of preceding trauma to the left hip at the age of 16. Examination revealed lockjaw and multiple bony hard lesions over the left posterior aspect of neck, middle and lower paraspinal regions, and in the left hip and thigh. Both great toes were deformed with bilateral hallux valgus deformities. Movements at the neck, spine and left hip were restricted. The subject's feet with short great toes and valgus deformities are shown in Figure 1. A photograph of his back with linear swellings secondary to extensive heterotopic ossifications is shown in Figure 2. Figure 3 is the X-ray of the pelvis with femur, also showing heterotopic ossification. What is the diagnosis? Fig. 2 View Large Image Figure Viewer Fig. 3 View Large Image Figure Viewer
