Abstract

Case Presentation: A 54-year-old man with diabetes mellitus and chronic obstructive pulmonary disease presented with a 1-week of history of hematuria, dysuria, fever, abdominal pain, and elevation of blood glucose. Physical examination revealed abdominal tenderness and bilateral wheezing. Laboratory tests revealed hyperglycemia (885 mg/dL), leukocytosis (17,000/mm3), high serum level of C-reactive protein (26.3 mg/dL), mildly impaired renal function (blood urea nitrogen, 40 mg/dL; creatinine, 1.4 mg/dL; sodium,115 mEq/L; potassium, 3 mEq/L), mild acidosis, leukocyturia, and hematuria.
