Abstract

Case Presentation: A 47-year-old female presented to the emergency department with a 4-month history of mild abdominal pain, intermittent constipation, and a growing left maxillary mass. She reported no other relevant symptoms. Physical examination was notable for a ~3 × 3 cm firm, fixed, nontender, left maxillary mass. Laboratory evaluation was only notable for an elevated corrected serum calcium of 16.5 mg/dL (normal value, 8.4 to 10.2 mg/dL), ionized calcium of 8.7 mg/dL (normal value, 4.6 to 5.4 mg/dL), alkaline phosphatase of 186 U/L (normal value, 35 to 104 U/L), and intact parathyroid hormone of 953 pg/mL (normal value, 15 to 65 pg/mL), with a low serum phosphorous of 2.0 mg/dL (normal value, 2.4 to 4.5 mg/dL) and 25-hydroxyvitamin D of 14.7 ng/mL (normal value, 20 to 80 ng/mL). Patient's renal function was normal. Further radiographic work-up with a computed tomography (CT) scan of the maxillofacial region was performed (Fig. 1 A, B). What is the diagnosis?
