Abstract

A 52-year-old man with vascular Ehlers-Danlos syndrome (COL3A1 gene mutation) previously observed for an aortoiliac aneurysm and dissection presented with 2-month history of fever and chills. Blood cultures were positive for Streptococcus equi. Computed tomography angiography demonstrated a 5.7-cm infrarenal abdominal aortic aneurysm with periaortic retroperitoneal stranding and bilateral common iliac artery aneurysms. A fluorodeoxyglucose positron emission tomography scan was performed and demonstrated uptake in the area of retroperitoneal stranding. The patient underwent open repair of the aneurysm with in situ reconstruction with an aortoiliac cryopreserved allograft, proximal anastomosis reinforcement with fascia lata, and bilateral internal iliac bypass. The patient was discharged from the hospital on postoperative day 10.
