Regular alternation in the direction of the QRS complexes during ventricular tachycardia has been reported by various authors, and, in the majority of cases digitalis intoxication has been responsible for the tachycardia.1-4We should like to present the following case report and electrocardiogram demonstrating alternation in the amplitude of the QRS complexes without an alternation in the direction of the complexes.
