Abstract

The objective of this video is to demonstrate use of the vNOTES device in removal of the adnexa during vaginal surgery. The video includes a review of the basic elements of the device, as well as research demonstrating improvement in clinical outcomes. The vNOTES device uses endoscopic instrumentation to perform routine and complex procedures vaginally. The gel single-port design, known as the GelPoint V-path transvaginal access platform, allows for the advantages of laparoscopy without abdominal incisions and results in improved cosmesis for patients. Procedures being performed with the vNOTES device include hysterectomy, salpingectomy, oophorectomy, combined salpingo-oophorectomy, and cystectomy. We review benefits to the patient, including shorter operative time, shorter length of stay, and lower pain scores, citing the HALON trial conducted between 2015 and 2017.1 The film reviews setup and use of the device and includes real-time surgical footage of a bilateral salpingo-oophorectomy procedure. In summary, this video is aimed to provide gynecologic surgeons information on the use of the vNOTES device in performing adnexal removal during vaginal surgery.
