Use of the technology has been expanded to other complex reconstructive urologic procedures such as pyeloplasty. Robotic assisted pyeloplasty (RAP) is being performed by many surgeons without prior laparoscopic experience. Difficult situations encountered intra-operatively can increase operating room time, increase morbidity, and decrease the success of RAP. METHODS Patient selection, port placement, ureteropelvic junction obstruction (UPJO) exposure, UPJO excision and repair are reviewed and tips given to circumvent difficult situations encountered during RAP. The video demonstrates techniques to aid in cases of extreme obesity, a large floppy liver, difficult to reflect colon (trans-mesenteric pyeloplasty), crossing vessels, large calculi, and previous attempts at UPJO repair. RESULTS Techniques to aid in successful completion of RAP in the face of extreme obesity, a large floppy liver, difficult to reflect colon (trans-mesenteric pyeloplasty), crossing vessels, large calculi, and previous attempts at UPJO repair are demonstrated.
