Options include multiple ureteroscopies with holmium laser lithotripsy, percutaneous nephrolithotomy, or ureterolithotomy. We present a case of a large proximal ureteral calculus which was treated via Robotic Ureterolithotomy. METHODS Following preoperative placement of ureteral stent, a 3 trocar configuration was utilized. Complete robotic mobilization of bowel, isolation of ureter, and localization of ureteral calculus was performed. Visual cues were performed to localize the ureteral calculus, although alternative options include laparoscopic ultrasound. RESULTS Extraction of the ureteral calculus was achieved with intact extraction. Placement within an Endocatch bag avoided any fragmentation during extraction. Reconstruction of the ureterotomy was performed with a 4-0 Vicryl on an SH needle in running fashion. Patient was discharged on POD#1. EBL 10cc. There were no complications. Stent was removed at 4 weeks. CONCLUSIONS Robotic ureterolithotomy is an option for large ureteral calculi in the proximal ureter.
