Our objective is to present the evaluation, surgical approach and outcomes of urethral reconstruction with ileal free flap. METHODS: We present a 35 year old male with obliterative stricture which was repaired with ileal free flap urethroplasty. The patient had previously undergone multiple endoscopic and open attempts including excision and primary anastomosis, buccal mucosa, gracilis flap and split thickness skin flap and perineal urethrostomy with recurrence. Reconstruction with ileal free flap was performed with success. The medical record and intraoperative photographs and video were reviewed to create this presentation. RESULTS: Urethroplasty with ileal free flap was successfully performed. At seven months post-operatively the patient reported normal erections and strong stream with low post void residual. CONCLUSIONS: Reconstruction of extensive urethral stricture after failed endoscopic and open attempts can be successfully accomplished with the use of an ileal free flap urethroplasty.
