It is a clinical challenge to treat a large target volume that exceeds the maximum field size limit. Conventionally, two isocenters, gapped fields with accepted under- and over- dose regions are used to treat volume beyond maximum field size. The potential under and over dose region could be 0% to 200% of the prescription dose. Using linear accelerator based IMRT, extended target volume can be treated with two isocenters without these under and over-dose regions. The purpose of this study is to evaluate dosimetric variations if the two isocenters of the IMRT fields are not perfectly aligned during treatment.
