Abstract

Health behavior change is often viewed as the rate-limiting step in a lifestyle medicine program. Health behavior change strategies that are tailored to an individual patient’s needs may help him/her become more successful at reaching his/her personal health goals. Based on the Transtheoretical Model of Behavior Change, this article provides an example of using “Readiness to Participate in Lifestyle Behaviors” and “Confidence to Participate in Lifestyle Behaviors” questionnaires in a community pharmacy lifestyle medicine program to tailor health communications. The questionnaires are important to the program for setting individual patient goals and for assessing a patient’s progress in the lifestyle medicine program.
