Abstract
To illustrate ultrasonographic findings of unicornuate uterus with hematometra of noncommunicating rudimentary uterine horn, hematosalpinx and endometriosis in a 13-year-old girl. A unicornuate uterus with a rudimentary horn is the most uncommon uterine anomaly of the female genital tract, and it can mislead to a diagnosis of ovarian cystic tumor sonographically. Therefore we would like to describe a ultrasonographic finding of this anomaly correlating with CT and MR findings. Transabdominal and rectal ultrasonograms demonstrated a unilateral mixed echoic ovarian mass. One- and two- month follow-up ultrasonograms showed laterally deviated uterus with a gradually enlarged and thick-walled cavity-like lesion with internal fine echo in the contralateral side adjacent to the initially demonstrated ovarian mass. CT and MR findings suggested a unicornuate uterus with hematometra of noncommunicating rudimentary uterine horn, hematosalpinx and endometriosis. The diagnosis was confirmed with laparoscopic removal of the rudimentary uterine horn and endometrioma. Sonographically, unicornuate uterus with hematometra of noncommunicating rudimentary uterine horn may be suggested if a laterally deviated uterus is demonstrated with contralateral thick-walled cavitary lesion containg internal fine echo accompanying mixed echoic ovarian mass.
