Abstract

We investigated the hypothesis that a standardized operative technique and perioperative management can achieve excellent outcome in patients who present with urgent indications for cervical cerclage.A retrospective review of 16 consecutive patients (18 infants) between 1989 and 1992 who were transferred to a single institution was completed. All patients were managed by one operator with a consistent McDonald/Wurm technique that also included perioperative antibiotics, empiric tocolytics, and special attention to the prolapsing membranes.The mean gestational age at the time of procedure was 25 weeks (range 19-29 weeks); the mean cervical dilatation was 3.4 cm (range 1-6.5 cm). Thirteen patients were dilated greater than or equal to 3 cm and 12 of these patients had membranes prolapsed through the external os. Amniocentesis was done in 50% of the patients. Mean prolongation of gestation was 6.5 weeks (range 4 days to 15.5 weeks) and was inversely correlated to cervical dilatation at the time of cerclage. ...
