Abstract

A 55-year-old woman developed chest pressure the day after having pelvic surgery. An electrocardiogram showed anterior ST-elevation, a chest x-ray showed interstitial pulmonary edema, and troponin-I was elevated at 5.35 μg/L. An echocardiogram revealed a large apical region of akinesis in the left ventricle (Fig. 1; Video 1 , view video online), and an ejection fraction (EF) of 40%. A subsequent cardiac catheterization showed essentially normal coronary arteries, with minor disease in only the obtuse marginal branch, and an EF of 43%.
