Abstract

Introduction: Medication safety relies upon the various layers of a ‘swiss cheese model’ to prevent potential errors reaching patients. Reporting channels available are often rigid and lacking in details in the data they can provide regarding process deficiencies, and may omit data relating to ‘near-misses’ entirely. Unmasking such deficiencies requires a prospective observational study of the entire medication administration process.
