Abstract

Background: Type 2 diabetes mellitus (DM2) has a deleterious impact on bone metabolism. There is a lack of studies evaluating the role of diabetic status on levels of calcium (Ca), parathyroid hormone (PTH), and 25-hydroxyvitamin D (VitD). after bariatric surgery (BS). Methods: Retrospective study of patients with obesity submitted to BS in comparison to patients with and without DM before and 36 months after BS. A multiple linear regression was used, adjusted to consider the outcome of the variation of the levels of Ca, PTH, and Vit D with surgery, according to diabetic status. Results: We evaluated 827 patients (36.6% with DM2) submitted to Roux-en-Y gastric bypass (RYGB), sleeve gastrectomy (SG), or gastric banding (GB). Diabetic patients submitted to RYGB had greater PTH levels 36 months after and those submitted to SG and GB had higher total Ca plasmatic levels 36 months after surgery. Diabetic patients presented an increase of PTH levels 36 months after RYGB, opposite to nondiabetic ones. Diabetic patients had significantly higher levels of Ca before and at every time point studied after surgery. Conclusion: Type 2 diabetes status is related with a significative increase of PTH during the first 36 months after BS. This increase was not mirrored by significant variations of levels of Ca or Vit D.
