Abstract

Objective To observe and evaluate the treatment of lumbar spondylolysis with the treatment of isthmus bone grafting combined with pedicle screw system and the clinical effect of comprehensive nursing intervention. Methods Twenty-one patients with lumbar spondylolysis who were admitted to our hospital from January 2017 to May 2018 were treated with isthmus bone grafting and pedicle screw system and perioperative nursing. The clinical effects after treatment and care were observed. Results The operation time was 76-131 min, with the average of (94.8±4.3) min. The intraoperative blood loss was 68-195 ml, with the average of (122.85±11.74) ml. The healing time of bilateral isthmus was 6-12 months, with the average of (7.29±1.85) months. The patients' low back pain VAS score after the internal fixation [(1.28±0.87)] was significantly lower than that before the internal fixation [(5.57±1.39)] (t=4.382 5, P=0.000 0); the ODI index score after the internal fixation [(10.46±3.18)%] was significantly lower than that before the internal fixation [(64.98±16.06)%] (χ2=19.532 7, P=0.000 0). Evaluation of lumbar spine function Mac Nab: there were 18 excellent cases, accounting for 85.71% (18/21), 3 good cases, accounting for 14.29% (3/21). The QOL scores of patients with PDM were significantly higher than those before treatment, with statistically significant differences (P<0.05). 18 patients were very satisfied with the nursing [84.51% (18/21)], and 3 patients were basically satisfied [14.29% (3/21)]. Conclusions The isthmus bone grafting combined with pedicle screw system for the treatment of lumbar spondylolysis and comprehensive nursing intervention can significantly treat lumbar spondylolysis, with isthmus healing well, therefore, low back pain visual analog score and Oswestry dysfunction index score significantly improve, the satisfaction of nursing effectively improve.
