Abstract

Abstract Despite pharmacotherapy, many persons do not get relief from persistent auditory hallucinations. This paper presents a treatment approach to auditory hallucinations that may be useful as an adjunct to medication. The therapy is based on the concept of hallucinations as covert speech, misattributed to external sources. The treatment program, which includes repeated measurement with a self-report questionnaire, is described. A systematic approach to auditory hallucinations, using behavioral, cognitive, and emotional control is illustrated. The approach emphasizes teaching self-control of auditory hallucinations that will facilitate a non-delusional reattribution. Success is determined not only by complete absence of hallucinations, but also along the many dimensions used to measure them, including reductions in distress, frequency, distraction, and loudness. Case studies are presented to illustrate the use of the therapy.
