Abstract

An unusual case after blunt non-penetrating orbital injury was reported. A 37-year-old female with the left globe outside the orbit was sent to the Emergency Room. Her left eye was struck by the fist of her husband. The cornea was clear, the anterior chamber had hyphema, but the details of fundus were not visible. The superior rectus, superior oblique, and lateral rectus muscles had been disrupted. The optic nerve had been disrupted at the point of 20 mm behind the globe. The right eye was normal. Orbital CT scan showed lateral dislocation of the left globe. No orbital fracture was noted. Enucleation and prosthesis were performed ad postoperative course was uneventful. The postulated mechanisms and management of optic nerve injuries are discussed.
