Abstract

A 65 year old man presented with embolisation of an atrial septal defect closure device (Flex II, Occlutech, Helsingborg, Sweden) to the abdominal aorta. Endovascular attempts failed to recapture it within a 9Fr introducer. Despite significant friction in the aorta, the device was pulled to the right common iliac artery. Arteriography (A) and computed tomography scan (B) showed aortic dissection complicated by right external iliac artery occlusion. The device was removed surgically, and retroperitoneal iliofemoral bypass was performed. Pulling a non-covered device may cause vessel injuries; inability to recapture the device indicates the need for a larger sheath or open surgery.Image 1
