Abstract

Transurethral balloon laser thermotherapy (TUBAL-T) is a safe and minimally invasive method for the treatment of benign prostatic hyperplasia (BPH). A special laser balloon catheter is transurethrally introduced into the prostatic urethra and fixed, and a laser probe inside the balloon irradiates a Nd: YAG laser through 360 degrees into the prostatic tissue. This elevates the temperature in the prostate tissue to over 45°C and induces coagulonecrosis of the adenoma tissues within 15 mm around the urethral surface. The urethral mucosa remains intact because there is cooling water inside the balloon. Prostate volume is reduced and a-adrenoceptors in the prostate decrease. Therefore, urethral resistance decreases, probably resulting in an improvement in urinary flow and alleviation of the subjective symptoms. We review published basic and clinical results of TUBAL-T and its reported clinical efficacy.
