Transperitoneal Transvesical Robotic Simple Prostatectomy for a Greater Than 400G Enlarged Prostate Using the Multiport Robot

Abstract

Introduction: In this video we demonstrate our surgical technique for approaching a massively enlarged prostate through a transabdominal transvesical approach using the multiport robotic platform. Materials and Methods: Here we present a case involving a 76-year-old man with history of enlarged prostate causing urinary retention with several episodes of gross hematuria, and clot retention. His prostate specific antigen was 18 ng/mL, and a prior transperineal prostate biopsy was negative for malignancy. The patient had a medical history of hypertension. Results: Operative time was 4 hours 48 minutes. Blood loss was 300 mL. Patient length of stay was 2 days. On follow-up 3 months postoperatively, the patient's postvoid residual bladder volume was 68 cc, International Prostate Symptom Score was 3, and quality of life score was 0. Conclusions: Transabdominal transvesical approach using the multiport robotic platform to perform simple robot-assisted prostatectomy is a feasible and safe option for treatment of a massively enlarged prostate.
