Abstract

Care coordination for adolescents and young adults with chronic conditions and/or special healthcare needs (SHCN) is essential to facilitate successful healthcare transitions (HCT) from pediatric to adult healthcare. Nursing care management and coordination focuses on the complexity of health conditions as children and adolescents with chronic conditions and/or SHCN and their family caregivers who have complex health literacy and social needs navigate multiple complex systems of care (Luther et al., 2019).
