Abstract

The establishment of a "Transition medical care support center" in each prefecture was requested by the Japanese Ministry of Health, Labour, and Welfare in 2017. To date, seven centers have been set up, including the Chiba Prefecture Transition Medical Care Support Center at the Chiba University Hospital. This review article introduces the current status and perspectives for the development of the care support system and autonomy/independence in patients with pediatric-onset chronic disease in their transitional age. The specific network acting on the total care and support should be warranted to achieve sufficient and proper assistance to the corresponding patients.
