Abstract
Transitional cell metaplasia is a distinctive benign metaplastic change of cervical epithelium usually found incidentally in postmenopausal women. It is a potential mimic of high grade dysplasia, particularly in cytology specimens. Transitional cell metaplasia may also be seen following prolonged exogenous androgen exposure as part of gender affirmation therapy. We present a case of a 29-year-old trans male who underwent gender affirming bilateral salpingoophorectomy and total hysterectomy following 6 years of exogenous testosterone therapy.
