Abstract

The effects of transfusions of fresh preparations of concentrated platelets have been studied in patients with acute leukemia on 34 occasions. With the exception of melena all bleeding was arrested when the recipient platelet count was increased by more than 40,000 platelets per cubic millimeter. The percentage of transfused platelets recovered in vivo was greater in patients without infection; the in vivo survival of such platelets is short in patients with active disease. The incidence of transfusion reactions or other side effects following repeated infusions of platelets did not exceed that following administration of whole blood or other blood products. The recommended dose of concentrated platelets is 8 to 10 concentrates (each from 500 ml. whole fresh blood) per 100 pounds body weight. Repeated infusions daily or twice daily may be required for adequate control of bleeding in patients with advanced disease.
